FCI- BEAUMONT MEDIUM- DECEMBER 31, 2011
 
At approximately 7:00 p.m., two staff members in the Special Housing Unit were assaulted. An inmate spat on them both striking one staff member in the face and chest, and the other on his back. Both staff members were medically evaluated and returned to work.

At 8:03 a.m., control announced that staff needed assistance in the courtyard. The Operations Lieutenant responded with less lethal munitions. Staff secured all of the inmates in the courtyard. The victim of the assault was escorted through the rear corridor to Medical and then to SHU. The SIS Tech reviewed the camera system and was able to make a positive I.D. of the two inmates involved in the assault. There were no staff injuries reported.

**At 6:55 p.m., a Unit Officer hit his body alarm due to a fight in the rear corridor entrance to the unit. One inmate was assaulted by two other inmates requiring an emergency medical trip due to injuries that he received during assault. The other two inmates were escorted to SHU for assault. One staff member reported falling on left hand in rear corridor while responding the assault. The officer was assessed by medical.

At approximately 12:05 p.m., a staff member was assaulted by an inmate in front of Health Services. The staff member was struck in the face and knocked to the ground. Once on the ground the inmate began repeatedly kicking the staff member in the head and facial area. Responding staff conducted an immediate use of force to gain control of the inmate. The staff member was transported to a local hospital, where he received multiple stitches to his head and facial area.

At approximately 1:50 p.m., a unit officer called for assistance due to an one on one inmate fight that erupted over a game of cards. The inmates were separated and escorted to medical for evaluation. There were no staff injured.

At approximately 8:45 p.m., control announced a fight in a housing unit. Responding staff had to physically separate the two inmates involved. Both inmates were medically evaluated and placed in SHU. There were no injuries to responding staff.

At 8:30 a.m., a three on one inmate fight was announced in SHU Recreation. The inmates were separated, medically evaluated, and escorted back to their cells.

**At 9:50 a.m., a three on one inmate fight was announced in SHU Recreation. The inmates were separated by responding staff, medically evaluated, and escorted back to their cells. Two staff members sustained injuries during this incident.

At approximately 11:55a.m., a three on one inmate fight between two different gangs took place on the compound. The inmates were separated by responding staff, medically evaluated, and escorted to SHU.

** At approximately 12:00 p.m., an inmate was assaulted with a weapon in a housing unit. This was a result of the previous incident. The institution was placed on lock down status until further notice.

An inmate was found dead in his cell of a suspected drug overdose. The FBI is investigating the incident.

At approximately 8:45 p.m., the FA officer called for assistance when he observed two inmates striking each other with closed fists about the head and upper torso in the FA Unit common area. One of the inmates fighting approached a third inmate and began striking him in the facial area with the closed fist. Responding staff secured all three inmates and transported them to the SHU. During transportation, one of the inmates was placed on the ground by staff utilizing the minimum amount of force necessary to gain control of the inmate. The inmate attempted to pull away from staff several times and ignored verbal warnings. Housing Unit BB was placed on reduced programming status after they temporarily refused to return their assigned living quarters (beds) during the institutional emergency. They will remain inside the unit until further notice.

At 1:55 p.m. a two on one inmate fight was announced on the recreation yard. The inmates involved were separated, medically accessed, and escorted to SHU. There were no injuries to staff.

At 1:40 p.m., a two on one inmate fight was announced on the recreation yard. The inmates involved were separated, medically accessed, and escorted to SHU. There were no injuries to staff reported.

At approximately 8:00 p.m., a SIS Tech reported , while reviewing cameras he observed what appeared to be two inmates on the recreation yard behind the soft ball stop swinging closed right and left fist to the head and upper torso area. The SIS Tech was able to make a positive identification upon further review of the cameras. The two inmates were escorted to the Lieutenants Office and assessed by medical and then sent to SHU for fighting. There were no staff injuries reported.

At 7:00 p.m., an one on one inmate fight occurred in a housing unit. The inmates were separated, medically accessed, and escorted to SHU. There were no injuries to staff reported.

USP- ATLANTA- DECEMBER 9, 2011
 
Unit Officers observed two inmates chasing anpther inmate who was bleeding profusely. Surveillance reviled that two gang members cut a member of a rival gang in the face with a razor blade. All inmates were restrained by responding staff and one inmate was taken to the outside hospital for treatment.

At 8:50a.m., an one on one inmate fight with weapons involved occurred in a housing unit. The inmates were separated by responding staff. Both inmates were medically accessed and escorted to SHU. There were no injuries to responding staff.

**At approximately 1:15 p.m., an inmate was assaulted on the recreation yard. The inmate was medically accessed and escorted to SHU.

***At 1:45 p.m., an inmate was assaulted in a housing unit. The inmate was medically accessed and escorted to SHU.

U.S.P.-2 went on look-down today due to inmate stabbing another inmate with a box cutter. Earlier in the day a note was dropped by an inmate that a staff member was going to be the target of the assault.

At approximately 2:34p.m., staff called for assistance in SHU. An inmate assaulted staff by throwing a unknown liquid substance on staff. The inmate submitted to hand restraints and was removed from the cell. The staff member and inmate were both medically assessed.

At 7:52 p.m., control announced a body alarm activation in a housing unit. An inmate assaulted the unit officer by sucker punching him from behind. The inmate struck the officer about the head and face with closed fists. The opposite side unit officer conducted an immediate use of force and was able to place the inmate on the ground, until the unit officer was able to regain conciseness, at which time he assisted holding the inmate down until responding staff arrived. The inmate was placed in Special Housing. The unit officer sought medical attention at the hospital and is doing ok. He suffered a fractured orbital socket, two knots on his head, and a fat lip.

At approximately 2:51p.m., staff called for assistance reporting an inmate fight in a housing unit. Staff observed four inmates striking each other in the common area of the housing unit. One inmate was transported to the local hospital due to the injuries that he sustained in the altercation. Two inmates received minor injuries and were placed in SHU without further incident. There were no staff injuries to report.

At 10:55p.m., staff called for assistance in SHU for a one on one inmate cell fight. Both inmates were separated by responding staff.

At 5:40 p.m., a call for assistance was received by control due to an immediate use of force in a housing unit. During a visual search an inmate struck a staff member’s upper body with his elbows during his successful effort to flush suspected narcotics. Medical assessments determined that the officer and the inmate sustained superficial injuries and the officer continued duties until the end of his assigned shift.

**At 12:04 p.m., the control room officers announced a fight on the recreation yard. Upon the arrival of responding staff, the inmates involved complied with orders and got down on the ground. The inmates involved were placed in hand restraints and escorted to medical for assessments. Due to the injuries to the inmate who was the victim of the assault he was sent to local hospital for treatment. Eight inmates from various housing units were placed in SHU without further incident. No staff injuries were reported.

At 7:37a.m., staff called for assistance in a housing unit for a one on one inmate fight. Both inmates were separated by responding staff.

At 11:17a.m., the tower officer called for assistance for a fight on the compound, this was a three on one fight with weapons. One inmate was stabbed several times in the head and neck area. All inmates were restrained by responding staff and one inmate was taken to an outside hospital for treatment.

An inmate stabbed another inmate in the housing unit. The institution was placed on lock down status until further notice and no staff were injured during the incident.

At 12:52p.m., staff called for assistance in the chapel area for a one on one inmate fight with weapons. Both inmates were separated by responding staff. The weapon was recovered and one inmate was placed in SHU. The other inmate was taken to an outside hospital by ambulance for treatment.

At 7:19a.m., staff called for assistance in the green corridor for a one on one inmate fight. Both inmates were separated by responding staff and placed in SHU.

At approximately 7:45a.m., a Lt. was assaulted by a inmate in SHU. The inmate was placed on the floor and escorted to a holding cell. The Lt. sustained minor injuries.

**At 8:18a.m., the tower officer called for assistance in the recreation yard for an inmate on inmate fight. Responding staff separated both inmates involved and both inmates were placed in SHU.

At 6:51p.m., staff called for assistance on the compound for a two on one inmate fight. Responding staff were able to separate and place all three inmates in SHU.

At 1:23p.m., staff called for assistance in education for a two on one inmate assault with weapons involved. Responding staff separated all inmates involved. Staff had to wrestle with one inmate who refused to give up the weapon he had in his hand. Both weapons were retrieved and two inmates were placed in SHU. One inmate was taken to an outside hospital by ambulance for treatment for several punctures to his chest and back area. There were no staff injuries.

At 12:21p.m., staff called for assistance in SHU due to an one on one inmate cell fight. Responding staff separated both inmates involved without further incident.

**At 2:16p.m., staff called for assistance in food service for an inmate on inmate fight in the bakery area. Responding staff were able to separate both inmates involved and both inmates were placed in SHU.

FCI- HERLONG- OCTOBER 27, 2011
 
At approximately 2:05p.m., a few inmates of different races engaged in a fight inside of an upper tier cell room out of view from the unit officer. After 2 minutes of altercation, the unit officer noticed the inmates heads all looking the same direction and then noticed the fight in progress and called for assistance. Responding staff were able to separate and restrain the inmates.
**At approximately 6:30p.m., inmates of different races were grouped up on the recreation yard due to the days earlier incident, also due to a new inmate who had just got there that was a former gang member but had dropped out and found religion. Yard recall was announced and the institution was placed on lock down status so mass interviews could be conducted.

At 6:24a.m., staff was called for assistance in SHU due to an one on one inmate cell fight. Responding staff separated both inmates involved without further incident.

At approximately 6:00p.m., an inmate was having difficulties clearing the metal detector. A unit officer ordered him to submit to a pat search and was able to pull a 6 inch metal shank from his waist line. The inmate was sent to the Lieutenants office. Upon his arrival, using a closed fist, the inmate struck the Operations Lieutenant in the face. The immediate use of force was swift and effective in subduing and restraining the assaultive inmate. No radio call was transmitted and no staff were injured. The inmate was medically assessed with minor contusions and escorted to SHU.

At 3:13p.m., staff called for assistance in a housing unit for a fight in progress. It was a two on one inmate fight with weapons. Responding staff were able to separate all inmates. One inmate was taken to the outside hospital by ambulance for treatment.

FCI- HERLONG- OCTOBER 23, 2011
 
At 7:45a.m., an inmate was found dead in his cell from a suspected heroin overdose.

**At 10:45p.m., during a visual search of an inmate after receiving a drop note, a small black vial fell to the ground from between the inmates legs. The inmate assaulted the staff member and was able to flush the vial down the toilet. With the help of the operations Lt. The inmate was placed in cuffs and escorted to SHU. The item was not recovered.

At approximately 9:30a.m., there was a fight between two inmates in unit 2-B. Both inmates sustained minor injuries. No staff sustained any injuries in the incident.

USP- LEE- OCTOBER 21, 2011
 
At approximately 8:23p.m., an inmate entered the Unit Officers station and began assaulting the Unit Officer about the head and face, with a weapon. At this point two other inmates entered the Officers Station and closed the door and exited the Officers Station about 20 seconds later. During this period, the Unit Officer was able to kick the phone off the hook, which triggered a no dial alarm and Control announced staff needs assistance in the Unit. The Unit Officer then managed to fend off the assault and exited the office, with 1 inmate pursuing and continuing to strike him with the weapon, as he exited the unit. The Unit Officer exited the unit via the rear stairwell and proceeded toward the lower Unit, with the inmate pursuing. The lower Unit Officer, while responding to the assistance call, intercepted the inmate, however he tripped while backing away from a blow by the weapon. An additional staff member arrived and subdued the inmate from behind. Other staff arrived and assisted in securing the inmate. The inmate was placed in hand cuffs and leg irons and escorted to the Special Housing Unit.

One Unit Officer was medically assessed with approximately 9 stab wounds to the head and facial area. Another Officer was also medically assessed with swelling to his left wrist. Both staff were transported to the local hospital, by Government Vehicle and treated for their injuries. One Officer received 4 sutures for a puncture wound to the upper lip and 3 sutures for a puncture wound to the forehead. The other Officer was treated for a sprained wrist. Both staff were released from the hospital after treatment. USP Lee was placed on lockdown.

At 6:58 a.m., a Unit officer activated their body alarm for a fight in the unit between two inmates. Staff separated the two inmates and placed them in hand restraints. They were escorted through the secured corridor to medical for their assessment and SHU without incident. SIS reviewed the cameras and noted another inmate was involved. One staff member received injuries responding to the fight.

At 1:20p.m., approximately 10 minutes after being released from SHU, an inmate assaulted another inmate in his housing unit. The unit officer called for assistance and with the help of responding staff was able to separate the inmates. Immediately upon the call for assistance, SIS Lt called for the housing unit officer of the other unit that received a SHU release of the same gang affiliation to find that inmate and secure him in an area. The 2 inmates of a certain gang affiliation had been in SHU because that gang can't walk the yard but had filed complaints that they were "unable to program" from SHU.

There was a staff assault last night on evening watch, an inmate came out of food services with food and was told no food could come out. The inmate was going to return inside, then was told by another staff member that he could not come back into food service. Then the inmate proceeded to walk back out the exit door and punched an officer in the face. The inmate was sent to SHU and will be transferred to the medium institution.

At 7:08 p.m., a compound officer requested assistance upon observing three inmates engage in a fight in the courtyard with weapons. The inmates were separated by responding staff. The inmates were medically assessed, photographed, and admitted into SHU. One staff member sustained injuries responding to this incident.

**At 8:08 p.m., a compound officer requested assistance upon observing two inmates fighting in the courtyard. The inmates were separated, both sustained minor injuries only. Inmates were medically assessed and admitted to SHU. Two staff members were injured during the incident.

 
 

FCI- FT. WORTH- OCTOBER 5, 2011
 
An inmate was murdered at the institution. Two inmates became involved in a fight. The fight cause serious injuries to one of the inmates and he died on the unit. The FBI was contacted and the aggressive inmate was placed in AD for the investigation.

At 9:00a.m., staff called for assistance in SHU for a two on one inmate fight in the outside recreation cage. All three inmates were separated by responding staff without further incident.

At 8:48a.m., a call for assistance was made in the facilities department for an one on one inmate fight in progress. Responding staff were able to secure all inmates in the facilities department and separate both inmates involved in the fight without further incident. Both inmates were placed in SHU.

At approximately 10:30a.m., it was discovered that an inmate escaped from the prison being the third escape this year.

At 1:30p.m., staff called for assistance in SHU for an one on one inmate cell fight. Both inmates were separated by responding staff without further incident.

At 7:04a.m., an assistance call was made in the food service dinning hall for an one on one inmate fight during the morning meal. Responding staff were able to separate both inmates involved in the fight and both inmates were placed in SHU.

**At 8:05a.m., a call for assistance in a housing unit was made for an one on one inmate fight with weapons in progress. Responding staff were able to separate both inmates involved without further incident and both weapons were recovered.

FCI- WILLIAMSBURG- SEPTEMBER 29, 2011
 
Approximately 200 gang rival inmates engaged in an altercation on the recreation yard. The inmates utilized horse shoes, balls, and other recreation equipment to assault each other. Less-than-lethal munitions were used to quell the disturbance. The institution was placed on lock down status. One officer was injured and may have suffered a break to his limb.

At 3:44p.m., an inmate walked up to a housing unit officer and stated he had been assaulted by other inmates. Upon review of the cameras it was found that three inmates assaulted the one inmate. All 4 inmates were placed in SHU.
 
**At 4:42p.m., staff called for assistance in SHU for an one on one inmate cell fight. Both inmates were separated by responding staff without further incident.

***At 11:10p.m., staff called for assistance in the holdover unit because an inmate was caught trying to swallow drugs in his cell. A use of force team was used to secure the inmate from his cell and the inmate was placed on dry cell. There were no staff injuries reported.

****At 11:55p.m., staff called for assistance in SHU for an one on one inmate cell fight. Both inmates were separated by responding staff without further incident.

FCI- SEAGOVILLE- SEPTEMBER 28, 2011
 
At approximately 2:46p.m., the Control Room Officer announced a inmate on inmate fight in Unit 2. Upon arrival of additional staff the unit was secured and both inmates were escorted to medical and evaluated . Both inmates were interviewed and placed in SHU.

At 12:26p.m., staff called for assistance on the compound. A staff member was conducting a pat search on the compound when the inmate reached into his pants and pulled out a homemade knife. The inmate tried to throw the knife onto the housing units roof without success. The inmate was placed on the ground by responding staff and placed in SHU.
 
**At 9:23p.m., staff called for assistance in SHU for an one on one inmate cell fight. Both inmates were separated by responding staff without further incident.

Two inmate on inmate fights occurred between rival gang members. Staff members secured the units and did mass interviews. Approximately thirteen inmates were placed in SHU.

At 1:59p.m., a call for assistance was announced in a housing unit for a one on one inmate fight in progress. Responding staff were able to separate both inmates involved without further incident. Both inmates were placed in SHU.

At 5:10p.m., two inmates were being moved to the recreation yard when they began arguing. The argument then escalated into a fight. At this time other inmates started joining in on the fight. Responding staff subdued all inmates and the institution was placed on lock down status.

**At 11:19a.m., staff called for assistance in the dinning hall during the noon feeding for an inmate fight in progress. Responding staff separated both inmates involved and placed them in SHU.

At 6:05p.m., staff called for assistance in a housing unit due to an three on one inmate assault in progress. Responding staff were able to separate all inmates involved.

At approximately 7:45a.m., a call for assistance was made from the food service department. Two inmates in the dish room were arguing over the quality of one inmate’s work. One inmate picked up a dirty pan and began striking the other inmate in the head and face area. Responding staff were able to separate the inmates in a timely fashion. One inmate was escorted to SHU, the other inmate was taken to the local hospital. No staff were injured in the response.

At 11:28a.m., staff called for assistance in the green corridor because an inmate took a swing at a staff member. Responding staff took the inmate to the groung restrained him and placed him in SHU.

At 9:00p.m., a use of force team had to be assembled in SHU to remove an inmate from his cell who broke his sprinkler head. The inmate refused staff orders to cuff up. The team was able to secure the inmate without any problems.

At 9:46a.m., staff called for assistance in the visiting room because an inmate became hostile after the inmate was caught having a sexual act with his visitor. The visit was terminated and while the inmate was being escorted to SHU the inmate became agitated and bucked on staff. The inmate was taken to the floor and once under control he was placed in SHU.

The institution was placed on lock down status after a fight broke out in a housing unit involving multiple inmates with weapons. One inmate was sent to the local hospital for treatment of stab wounds. The lock down was lifted on Wednesday, September 7, 2011.

At 12:40p.m., staff called for assistance in an unit. Four inmates assaulted one inmate. The four assaultive inmates ran and dispersed when staff arrived at the scene. Later identified by video and staff recognition, all inmates were collected and sent to SHU. The victim was escorted to a outside hospital for facial and head injuries. An emergency count was conducted so staff could identify inmates and ensure all were escorted to SHU.

At approximately 2:00p.m., the DTS at USP Tucson was escorting an inmate back to his unit and escorting a volunteer out of the institution, when the inmate turned and punched the DTS in the face. A call for assistance went out. The DTS was able to put the inmate against the recreation fence and place him in hand restraints. Reports thus far indicate that the DTS sustained a broken nose and swollen upper lip. The volunteer was safely escorted out of the facility. The institution was placed on lock down.

At approximately 6:03a.m., control announced a fight in SHU. Responding staff were able to restrain both inmates and separate them without further incident. After initial interviews it turns out that inmate #1 was attempting to commit suicide and tied the larger inmate #2 to his bed in hopes that he would be unable to intervene. Inmate #2 was able to break free from his ties and activate the duress alarm. Inmate #1 continued to attempt suicide and inmate #2 pinned him to the ground. Inmate #1 is currently on suicide watch.

At 8:47a.m., an inmate was in a recreation cage. At that time, he had just had his wrist restraints removed to begin his recreation period. As his restraints were removed he stepped forward to allow the other inmates to be unrestrained. Another inmate placed his hands into the slot to be unrestrained. As this happened, the unrestrained inmate approached the restrained inmate from the front and began to punch him in the face, head, and upper body with closed fists. Responding staff arrived and controlled the situation and placed the inmate on the ground to gain control. One staff member fell hitting the concrete and the other staff member was kicked by the other inmate at this time. The inmates were removed from the recreation cage without further incidents.

At 12:30p.m., A cook foreman noticed an inmate trying to steal a bag of chicken from food service. The foreman asked the inmate to give back the bag of chicken but the inmate refused and became very loud and started cursing at the cook foreman. The cook foreman then gave the inmate an direct order to give him the bag of chicken. The inmate again cursed at the foreman then pulled the bag of chicken he had hidden in his cloths and slammed it into the foreman's out stretched hand. The inmate was escorted to the Lt's office then placed in SHU. The staff member was not hurt.

Both inmates refused orders to stop and lay on the ground along with other inmates in the recreation yard. The tower officer dispersed munitions to control the situation at which time all inmates laid on the ground. Responding staff were able to secure both inmates involved and placed them in SHU.

Both inmates did comply with staffs orders to cease their actions and lay on the ground. Both inmates were medically assessed with minor injuries and placed in SHU without further incident. There were no staff injuries.

**At approximately 2:07p.m., staff called for assistance in the in the unit team alley way. An inmate threatened and attempted to assault staff. Staff in the immediate area were able to stop him as he attempted to cross a desk to reach the staff member. The inmate was medically assessed with no injuries and placed in SHU without further incident. There were no staff injuries.

At approximately 8:05 pm, an officer was escorting an inmate back to his cell from the recreation yard. While walking past the inmate’s cell to get off the range, he grabbed the officer’s key chain from his food slot and struck him in his right thigh with a clear homemade shank. The officer was able to get away from the inmate. The inmate then stated " It’s on now, I am going to get my recreation time." The officer notified staff on the range of the situation and told them to stay away. The inmate then started throwing food trays and other items out of his cell.

At 6:55p.m., staff called for assistance in front of a housing unit. An inmate was being pat searched when the staff member found a weapon. The inmate pulled the weapon out and threw the weapon to another inmate. The first inmate was taken to the ground and responding staff took the second inmate to the ground after the inmate had thrown the weapon down a storm drain. Both inmates were placed in SHU. The weapon was retrieved by maintenance staff. One staff member had to go to outside hospital for stitches in their elbow the officer is doing fine.

**At 9:28p.m., staff called for assistance in the green corridor. An inmate tried to assault a staff member. The inmate was talking to the Ops Lt when the inmate became disruptive and took a swing trying to punch the Lt in the face. The inmate missed and was taken down to the ground, cuffed, and placed in SHU.

The staff member told the inmate to drop the weapon and the inmate started to run. The staff member was able to take the inmate to the ground and cuff the inmate. The weapon was retrieved and inmate was placed in SHU.

At 10:15a.m., the Operations Lt. was notified by C-Unit Case Manager of a possible assault that occurred on the evening of August 3, 2011. The inmate admitted during interview he had been bumped by another inmate. He identified the other inmate through a photograph. The inmate was taken to Health Services for medical assessment. The assessment revealed bruising on the inner lower lip and multiple abrasions on the back. The accused inmate denied the incident and refused to provide information. Both inmates were placed in SHU pending investigation.

At approximately 6:47p.m., an inmate spit on the Lieutenant as she walked by the recreation cage.

**At 6:53p.m., and inmate spit on an officer then threw an unknown liquid substance on him as he walked by.

***At 7:00p.m., the same inmate that was involved in the above incidents began to bend then metal back and forth that connects the fencing to the pole near the door of the recreation cage. The officer that noticed this called for assistance and notified the parameter patrol officers of the situation. The inmate then breached the cage and was walking freely in the common area of the recreation yard. The inmate then started to verbally assault the officer and stated that he wants to kill a staff member. A Use of Force team was called in and the inmate was placed in restraints with no future incidents.

An inmate that was released from SHU was returning to his housing unit when another inmate came up to him and the two inmate began fighting other inmates started to group around the two inmates fighting and refusing staff orders to get down. Flash bangs were dispensed from the tower. Responding staff were able to separate both inmates. The inmate that started the fight assaulted staff by pushing and officer in the chest. The inmate was taken to the ground and hand and leg restraints were applied both inmates were placed in SHU. The officer is fine.

At approximately 6:20p.m., recreation staff notified control that the inmates on the yard were grouping up.  Upon the arrival of additional staff, the inmates refused to disperse.  Less-lethal and chemical munitions were requested.  When they arrived the inmates began to comply.  A yard recall was conducted, and the institution was placed on lockdown.  On July 19 the institution came off of lockdown and operated under a modified movement schedule.  On July 21, the institution resumed normal operations.

At approximately 2:20p.m., an inmate approached the Unit Officer with injuries consistent with being in a physical altercation. It was determined that the inmate was assaulted in a cell by two other inmates. All three inmates were medically accessed and escorted to SHU.

**At approximately 7:00p.m., an inmate was assaulted on the recreation yard. Yard recall was announced. Head and upper body checks were conducted of all inmates exiting the recreation yard. The assaulted inmate was medically accessed and escorted to SHU.

At approximately 2:20p.m., an inmate assaulted another inmate in a housing unit with a lock-in-sock. This turned into a fight between three inmates. Responding staff were able to restrain the inmates and escorted all three to medical then placed them in SHU. The unit was locked down and breathalyzed. Three more inmates tested positive for intoxicants and were taken to SHU. No staff were injured during the response.

The institution was placed locked down status Sunday, July 10, 2011 following a gang related fight on Saturday, July 09, 2011 and then another fight involving the same gangs on Sunday, July 10, 2011. The prison remained fully locked down until Wednesday, July 13, 2011. Approximately 15-20 inmates have been placed in SHU resulting from the incident(s). There were no staff injuries were reported.

At 6:45p.m., staff responded to D Block when an inmate assaulted staff. While attempting to remove the restraints from an inmate, the inmate pulled the staff into the door of the cell. The staff member suffered an abrasion to his right hand.

At 1:05p.m., a fight involving 15-20 inmates erupted in B-2 housing unit. Three staff were injured responding to the incident. One staff member separated his shoulder, the second staff member injured his elbow, and the third was kicked in the head. The USP is currently locked down until further notice.

At 8:25a.m., staff responded to B Block for an inmate on inmate assault. Upon removal of restraints from the first inmate, he began hitting the other still restrained inmate. He then pulled a razor blade from his mouth and began slashing the restrained inmate. Staff were able to restrain and separate. Both were examined and treated by Medical.

**At 11:10a.m., staff responded to D Block when an inmate grabbed an Officer through the food slot and attempted to pull him in, slamming the officer into the door. Staff was examined and suffered no injuries.

***At 8:43p.m., staff responded to D Block for a fight involving two inmates. Responding staff were able to restrain and separate the two inmates.

At approximately 9:15a.m., three unrestrained inmates assaulted one restrained inmate inside of the SHU recreation cage. Less lethal munitions were called for but prior to their arrival the assault had stopped and staff were able to restrain the three inmates. Medical staff responded quickly and EMS was began with local services called. The inmate was air lifted to a local hospital with head trauma, several broken facial bones among other injuries. The inmate has been admitted to the hospital and remains there. This was directly related to the lock-in-sock assault that occurred June 19th.

Upon the arrival of additional staff, the door was opened and both inmates were restrained, medically accessed, and placed into separate cells. There were no staff injuries reported.

At 5:50p.m., staff observed two inmates assaulting one inmate with their fists. One inmate attempted to flee and he was chased. The other two inmates refused to cease and separate from the victim and staff placed them onto the floor. There were no weapons involved and only minor injuries. Two staff members sustained one abrasion each to their elbows sustained during the process of placing inmates onto the floor. Following this incident, a third Officer sustained a puncture from a sewing needle which was located within sheets.

The inmate began jerking away from the staff member then struck her with an elbow. The inmate was taken to the floor to regain control. Staff were examined by medical and no injuries sustained.

At 8:00p.m., staff called for assistance in a housing unit for an inmate on inmate fight in progress. While responding staff were trying to separate both inmates that were fighting other inmates in the housing unit tried to surround all the staff. One officer was hit in the back of the head with a gatorade bottle filled with water. The inmates then disbursed to their cells and the unit was placed on lock down status until further notice. Both inmates involved in the fight were placed in SHU, The officer that was hit in the head with the bottle of water was taken to an outside hospital for treatment. The officer is doing fine but has a lump on the back of his head. After further review of the cameras staff were able to identify the inmate that assaulted the staff member with the bottle of water inmate was placed in SHU.

At 12:18p.m., staff responded to D Block for an inmate assault on staff. As staff was attempting to pick up food trays from a cell, an inmate grabbed the officer and attempted to pull him in, ripping the officer's shirt. The officer suffered no injuries.

**At 1:07p.m., staff responded to R & D when an inmate began hitting staff with elbows and had to be taken to the floor. Staff were examined by medical. One staff member suffered a concussion and another staff member received a minor scratch to his hand.

The other inmate was placed in SHU. Injuries consist with a lock in a sock. Before count, the unit officer was notified by an inmate that something was going to happen. The officer notified the Operations Lieutenant and was instructed to start count. There were no staff injuries reported.

**At 1:33p.m., staff responded to J Block for an inmate on inmate assault. After staff removed the restraints from the first inmate, he began hitting the restrained inmate. Staff were able to restrain and separate the inmates.

At approximately 3:50 p.m., an inmate refused several orders to return to his unit. When approached by staff the inmate became combative, and attempted to assault staff. An immediate use of force was conducted. The inmate was placed on the ground and restrained. The inmate was medically accessed and placed in SHU. There were no staff injuries reported.

At approximately 1:20p.m., an officer asked an inmate to step out of the band room with his I.D. in hand , the inmate was told to stay out previously. When the inmate stepped out of the band room he swung and punched the officer in the side of his face. The officer activated his body alarm and the institution was placed on lock-down and a emergency count was conducted. At 4:30p.m., after a good verbal count, a conference call was announced and the institution went back to normal operations.

At 9:40a.m., a counselor was seriously assaulted by an inmate at the USP. The inmate entered her office and attacked without warning. The inmate sucker punched her, then grabbed her by the hair and throat and punched her several times before staff arrived and restrained the inmate. The counselor was treated at an outside hospital with extensive swelling and bruising to her face. She was released later the same day. The institution was placed on lock down status until further notice.

At 8:48a.m., staff called for assistance in the green corridor for an inmate fight. Two inmates were fighting in the green corridor in front of health services both inmates were separated by responding staff without further incident. Both inmates were medically assessed and placed in SHU.
 
**At 6:26 p.m., staff called for assistance from a housing unit inmate due to an assault in progress. A group of inmates were assaulting one inmate. Responding staff were able to separate all inmates involved when they arrived. Two gang members were placed in SHU. The inmate that was assaulted was sent to an outside hospital for treatment. Upon further investigation and review of the cameras three more gang members were placed in SHU for the assault. There were no staff injuries reported.

The institution was placed on lock down status for multiple fights and assaults on the recreation yard. At approximately 8:00 pm staff called for assistance in recreation. A fight broke out at the recreation entrance/exit gate which lead to other fights all over recreation. Inmates were using rakes, shovels, weight bars, hoes as weapon against each other. After several orders to stop fighting the perimeter officers fired off one round. The SIS Lt. enter the recreation area with several non-lethal weapons to help get control of the inmates. A staff recall was done. The control room officer call 911 and local law enforcement responded to institution to assist. Approximately 9 inmates where taken to local hospital. There were no staff injuries reported. The lock down continues.

At approximately 4:55 p.m., an inmate became disruptive. Specifically, the inmate threw a food tray and struck an Officer in the groin area. The inmate displayed signs of imminent violence by stating he would assault more staff. Due to the inmate’s assaultive actions and display of imminent violence, the Warden was notified and authorized a Use of Force Team to be assembled to place the inmate in ambulatory restraints. At approximately 6:04 p.m., a Use of Force Team was assembled and confrontation avoidance procedures were initiated with positive results. The inmate was removed from his cell and escorted to the shower. The inmate was placed in ambulatory restraints at approximately 6:22 p.m., and was escorted to another cell without further incident. The inmate was medically assessed and sustained no injuries.

At approximately 1:16 P.M., a SHU Officer activated his body alarm as two inmates became disruptive as they were removed from their cell in order to conduct a cell search for contraband. During the escort one inmate attempted to assault the SHU Officer by pushing him in the chest. An immediate use of force occurred. The SHU staff placed both inmates on the ground and applied leg irons due to their disruptive behavior. Both inmates were searched, metal detected and escorted off the range. Both inmates were placed in separate holding cells and medically assessed by health services. One inmate sustained a small skin abrasion to his left cheek. The other inmate sustained redness to his chest. There were no staff injuries.

At approximately 4:30 p.m., while staff were delivering the evening meal food trays an inmate became disruptive and spit at a staff member through the inner grill of his assigned cell, striking staff in the back of the head as they was exiting the cell sallyport. Staff immediately secured the inmate’s outer cell door. Medical assessments were conducted on both the staff and inmate with no injuries noted. The inmate was escorted to the SHU without further incident.

At approximately 8:30am, an inmate in SHU became disruptive by covering his window, breaking the fire sprinkler head, and threatening to commit suicide. A Use of Force Team was being prepped for entry to place the inmate in a safer environment when the inmate decided that willingly being cuffed up through the food slot was the better option. He was then placed on suicide watch with very little resistance or disruption after that.

At approximately 7:34 a.m., the SHU nurse notified the SHU Lieutenant that an inmate had assaulted staff. Specifically, staff passed medication to cell which housed the inmate. As staff attempted to close the food slot the inmate pushed the food slot open and caught the right wrist of the staff member. The Lieutenant arrived to the cell and ordered the inmate step away from the food slot so staff could secure it, to which he complied. The inmate was medically assessed with no injuries noted. The staff member was medically assessed and referred to a local hospital for X-Rays.

At approximately 5:50 p.m., as Commissary staff were issuing commissary on F-Range in the SHU, an unknown liquid substance struck staff on the leg. Staff observed the substance coming from a cell occupied by two inmates. Both inmates were interviewed and one inmate admitted to throwing milk at staff. Medical assessments were conducted on both staff and inmate. An incident report was written.

The institution came off lock down status around noon. At 2:30p.m., a group of inmates when out on the compound on the basketball court and sat down, refusing to go in. After the 4:00pm count the FCI was locked down again.

At approximately 7:15p.m., an inmate saw an officer go to bathroom and when the officer came out the inmate sucker punched him. The officer sustained a broken nose and had to have nine stitches applied to his forehead. Six other inmates where also locked up. This was a planned attack. The officer who was attacked was a good officer who did regular shake downs and was a constant in confiscating hooch. This was a retaliation move by the inmates. The instituiton was placed on lock down status until further notice.

At approximately 6:05 a.m., an inmate became verbally assaultive to the Food Service Forman after receiving his food tray then forcefully slammed his food tray on the serving line and began to walk away yelling. Food Service staff followed the inmate out of the Dining Room where they ordered him to get against the wall to submit to a pat search. As staff attempted to search the inmate, he spun around and elbowed a Food Service staff member in the face. Responding staff immediately placed the inmate on the ground, placed him in hand restraints and escorted him to the Lieutenant's Office. The inmate was examined and treated for scrapes to his right elbow and left knee. One Food Service Foreman were examined and treated for a bruises under the left eye, left elbow and back of neck. Another Food Service Foreman was examined and treated for abrasions to the left elbow. The inmate was placed in Administrative Detention without further incident.

At approximately 7:26 p.m., an inmate assaulted staff as the staff member was securing the food trap. The inmate was observed hitting the food slot with his fist which in turn struck the officer’s left hand. Orders were given to the inmate to step away from the door so the food slot could be secured. The food slot was re-secured without further incident. The inmate refused photographs and medical assessment.

At approximately 6:00pm, during a rainbow soccer game, one inmate was body checked by another inmate of a different race. This led to a stand off between approximately 50 inmates of each race. Inmates were refusing to disperse and ignoring staffs orders to return to their housing units for approximately 10 minutes when the less lethal munitions arrived on scene. Several inmates were identified as instigators and placed in SHU. The institution was placed on lock down status for the inmates safety and so SIS could conduct mass interviews and determine that tempers had receded and further and worse problems would not occur. A modified feeding and recreation schedule was set in place and ran smoothly until May 23 where the institution was brought back to normal operations.

Staff received intelligence that a food strike was planned. "B" unit inmates were recalled and interviewed. At the noon meal approximately 20 inmates went to the dining hall. All inmates were returned to their housing units and the lock down is planned to last at least until Saturday.

At approximately 7:30 a.m., staff were attempting to remove the hand restraints from an inmate when he grabbed the Officer’s hand and attempted to pull it through the food slot. Staff were able to secure the food slot, and the East Lieutenant was notified. Due to the inmate’s assault on staff, the Warden was notified and authorized a Use of Force Team to be assembled to place the inmate in ambulatory restraints. At approximately 8:10 a.m., a Use of Force Team was assembled, and confrontation avoidance procedures were initiated with positive results. The inmate was removed from his cell and escorted to the shower. He was placed in ambulatory restraints at 8:27 a.m., and escorted to D-Block, without further incident. The inmate was medically assessed, with no injuries.

The institution was placed on lock down status for multiple fights and assaults. There was a large group of inmates that refused to leave the recreation yard or comply with staff’s orders. Once less lethal munitions were brought to the recreation yard and the inmates became compliant and were locked down. The investigation continues, but the incident was gang and race related.

At approximately 9:05 a.m., after the Operation's Lieutenant removed an inmate from ambulatory restraints, he became combative and belligerent. The inmate dropped to the floor, kicking and cursing, refusing to cooperate with staff. Additionally, he proceeded to assault staff by spitting on two staff member's facial area. Staff maintained control of the inmate and reapplied ambulatory restraints. Both staff and inmate were medically assessed.
 
**At approximately 8:30 a.m., while SHU staff were collecting Food Service trays an inmate assaulted staff by throwing a Food Service tray and books at staff, striking staff on her left knee. The Operation's Lieutenant was notified. Both staff and inmate were medically assessed.

At approximately 8:30 p.m., an inmate assaulted the SHU Officer by throwing a cup of water, hitting the officer in the mid section and groin area. The SHU Officer and the inmate were examined and did not sustained any injuries. The inmate will remains in SHU pending further investigation.

**At approximately 6:40pm, a newly arrived inmate was assaulted by 3 rival gang members on the recreation yard. Responding staff issued direct orders to stop fighting and the inmates complied. They were restrained and escorted to SHU where all 4 were medically assessed.

At approximately 12:40p.m., the unit officer called for staff assistance via radio due to an inmate running in the unit. The unit officer was conducting rounds on the second floor and conducting random pat searches. The Unit Officer observed an inmate exiting the second floor bathroom looking suspicious. When staff approached the inmate, he gave him a direct order to submit to a pat search. As the Unit Officer attempted to pat search the inmate he turned toward his right, and struck staff with his right elbow into his right shoulder to avoid being searched. The inmate ran through the unit and was located on the second floor. Both inmate and staff was medically assessed with no injuries. The inmate was escorted to SHU without incident.

At approximately 3:10 p.m., while an inmate was in a Special Housing Unit Recreation Cage, he asked the SHU OIC to call the SHU Lieutenant and stated he wanted to talk to him. Upon the Lieutenant's arrival to the recreation cage, the inmate began yelling at the SHU OIC and proceeded to assault the officer by spitting on him, striking him in the face. The inmate continued to yell racial slurs and assaulted the officer a second time, by spitting on him, striking him in the face. The SHU Lieutenant contacted the Captain and notified him of the inmate’s assaultive behavior. A Use of Force Team was authorized, however, before the team was assembled, the situation de-escalate and the inmate was escorted back to his cell without further incident.

At approximately 7:15 p.m., as staff escorted an inmate to the Lieutenant’s office. He threatened to stab escorting staff and turned toward staff in an aggressive manner, staff placed him on the floor to gain control. While being escorted to SHU, the inmate attempted to pull away from escorting staff. He again was placed on the floor, leg restraints were applied, and he was assisted into the ambulatory chair and escorted to SHU. He was medically assessed and placed in the holding cell without further incident. The FBI was notified and an investigation continues.

Multiple (total of 11) inmates were fighting on recreation yard.. The institution was placed on modified lock down status. The modified lock down was lifted on May 16, 2011.

At approximately 6:33 p.m. staff called for assistance reporting an inmate fight behind the backstop on the recreation yard. Staff observed two inmates striking a third inmate in the head and upper body. All three inmates had to be placed on the ground due to refusing staff orders to cease their actions. The inmates were medically assessed with minor injuries and placed in SHU without further incident. Tower #3 Officer fired 3 ALS Hard Bangs and the Tower #7 Officer fired 2 ALS Hard Bangs, and 3 .223 rounds into the ground, when the inmates would not cease their actions

**At approximately 7:35 p.m. staff called for assistance reporting an inmate fight on the softball field. Staff observed 4 inmates striking each other in the head and upper body with closed fists. The inmates did comply with staffs orders to cease their actions and lay on the ground. The inmates were medically assessed with minor injuries and placed in SHU without further incident. All inmates were recalled to their assigned housing unit and secured in their cells. An emergency count was conducted at that time. The Tower #3 Officer fired 4 ALS Hard Bangs and the Tower #7 Officer fired 2 ALS Hard Bangs and 2 .223 rounds into the ground when the inmates would not cease their actions.

At 12:52p.m., staff responded to J Block for an inmate cutting his wrists with a piece from a broken mirror. Staff restrained the inmate and escorted him to the shower. Once in the shower, the inmate head butted the officer. The officer put the inmate up against the wall. Both were examined by medical staff.

At 8:40p.m., staff responded to a cell fight in Z Block involving two inmates. The inmates were restrained and separated.

At approximately 7:30 a.m. an inmate assaulted the Mental Health Unit Nurse by throwing dry cereal and hitting the nurse in the facial area. Staff in the area gave an inmate an order to stop and submit to hand restraints and he complied. The inmate was escorted to 1-E, medically assessed, and placed on Administrative Detention without further incident. The mental health nurse was medically assessed with no injuries noted.

At 1:44p.m., staff responded to an assault on staff in D Block. While escorting the inmate back to his cell, he twisted away and kicked staff. The two escorts took the inmate to the ground and called for assistance. All were medically examined with no injuries noted.

At 11:18a.m., staff called for assistance in food service. During mainline feeding staff witnessed two inmates striking each other in the face and body area's. Responding staff were able to separate both inmates and place them on the ground. Staff found a weapon on the one inmate during a pat search. Both inmates were medically assessed and placed in SHU.

**At 11:53a.m., staff needs assistance call was made in the Lt's office. An inmate refused a pat search in the green corridor when he bucked on staff. The inmate was cuffed and taken to the Lt's office to be strip searched. While being strip searched the inmate was found with a weapon on his person and bucked on staff again. The inmate was placed on the ground cuffed and escorted to SHU.

***At 2:40p.m., staff called for assistance in the Lt's office because an inmate refused to be pat searched by a female staff member in the green corridor. The inmate was taken to the Lt's office. Once there the inmate was told to submit to a strip search. The inmate refused and lunged at staff the inmate was taken down to the ground cuffed and placed in SHU.

At approximately 6:11 p.m.,two inmates were involved in a physical altercation with one another in the Multipurpose Room in recreation over the recently completed basketball game. The altercation escalated in the gymnasium, where inmates from both factions (Massachusetts and D.C.) began fighting. During staff's attempts to subdue the inmates involved, an officer was hit in the head by one inmate. Several other inmates were observed on the camera recordings kicking and/or throwing punches in the gym. All inmates noted above were medically examined and placed in SHU pending further investigation.

At approximately 7:55 a.m., the SHU Lieutenant was informed an immediate use of force took place on the 2nd Floor Range in the SHU. An inmate was being removed from his cell and to have a pat search conducted on him by the SHU Officer when the inmate reached and grabbed the officer in his groin area during the escort. The inmate was placed on the wall and control was gained by the Officer. Medical assessments were conducted and the Officer sustained a laceration to his upper lip. The inmate sustained no injuries.

At approximately 7:35 a.m., a call for staff assistance in the Laundry Department was announced over the radio system. Upon entering the Laundry Department area, responding staff observed the Laundry Foreman holding an inmate against a row of shelves. Further investigation revealed the inmate became disruptive and displayed combative behavior with the Laundry Foreman when he asked the inmate to return some missing photo tickets. Specifically, the inmate turned and pushed the Laundry Foreman, striking him on the left side of his chest. The Laundry Foreman utilized an immediate use of force in an attempt to secure the inmate by holding onto the inmate’s torso and placing him against some shelves, to gain control of the inmate, until responding staff arrived. Once responding staff and the Operations Lieutenant arrived on the scene, they were able to gain control of the inmate and place him in hand restraints without further incident. The inmate was medically assessed and then placed in SHU. The inmate did not sustain any injuries from the incident. The Laundry Foreman was also medically assessed and was found to have sustained a contusion on his left anterior forearm. An investigation continues.

An assistance called was made for an assault on staff in a housing unit. An inmate entered an unit officer's office and reached in his back pocket, giving indications he was about to assault the officer. The inmate was restrained and a combination lock tied to a belt was found in his back pocket. The staff member suffered a dislocated shoulder.

At approximately 9:40 p.m., SHU staff were picking up food trays from cells. As staff opened the food slot, an inmate assaulted staff by throwing feces on staff, striking staff in the face, front and back of staff's shirt, pants and shoes. Medical assessments were conducted on both staff and inmate. An incident report was written.

At approximately 3:00 p.m., an inmate became assaultive by striking a Teacher in the Education Department. Specifically, the inmate charged at the staff member striking him in the chest with his body, after the staff member opened the front door for an open move. The inmate was ordered to report to the Lieutenants' Office and he complied. As the Education staff member was leaving the Lieutenants' Office, the inmate began to verbally assault and threaten him. Staff attempted to calm the inmate and ordered him to be quiet and sit down, but he became assaultive, charging the closest staff member with closed fists. An immediate use of force was conducted, and the inmate was placed on the ground. Restraints were applied and the inmate was escorted to SHU. While under escort, the inmate continued to be combative, by resisting and attempting to head butt escort staff. Once inside SHU, the inmate was placed in a visual search cage. He was interviewed by Psychology and medical staff, and asked to accept prescribed medication, but refused and continued to display bizarre and delusional behavior. Due to his current mental state, and history of non-compliance with psychotropic medication, a decision was made by the Clinical Director to involuntarily medicate the inmate. A Confrontational Avoidance Team and Use of Force Team was authorized by the Acting Warden, and assembled in SHU. At 4:25 pm, confrontational avoidance proved unsuccessful, as the inmate refused numerous attempts by staff to get him to accept prescribed medication. The Force Cell Move Team entered the visual search cage, and escorted the inmate from the cage to the observation cell. The inmate was placed on the bunk in the cell, and administered 2mg of Lorazepam, and 5mg of Haloperidol by Health Services Staff. A medical assessment was conducted and the inmate was noted with a small abrasion on the left side of his forehead, and a small abrasion on his left shoulder and left knee. Staff conducted a clothing exchange with the inmate, and he was escorted to his cell without further incident. A medical assessment was conducted on the Operation's Lieutenant who was involved in the immediate use of force, and was treated for minor bruising to the left knee and redness on the left wrist.

At approximately 1:15 pm, an inmate was being placed in the Secure Mental Health holding cell when he became assaultive. Specifically, the escorting officer's had requested that the inmate remove his shoes, which is protocol for entry into the holding cell, when he became violent and attempted to kick both officer's and he head-butted one of the officer's on the chin. The inmate had to be placed on the floor to gain control of him. Once control was gained, he was placed in the holding cell without further incident. The inmate was given a medical assessment with no injuries noted and placed on administrative detention pending disciplinary action. Both officer's received medical assessments with one citing pain to his lip and chin area and the other having soreness to his hand and pinkie finger.

At approximately 5:45 p.m., in SHU an inmate assaulted the SHU Officer. Specifically, the inmate was in recreation cage, requested for staff to step over to the recreation cage, staff arrived at the recreation cage the inmate spat in the face and upper torso area of the staff member. Staff proceeded from the recreation area without incident. The inmate submitted to hand restraints and was escorted from the recreation cage. Medical assessments were completed on staff and inmate. The inmate remained in his current housing assignment.

At approximately 9:10 a.m., an inmate entered the officer's station and walked to the side of the desk. Staff ordered the inmate several times to exit the office, but he refused. When staff approached the inmate, he reached for an item in his right rear pocket. Staff attempted to place the inmate against the wall to gain control; however, a struggle ensued. During the struggle, the inmate hit the officer with a closed fist in the facial area and placed his arm around the officer's throat in attempt to choke the officer. Additional staff arrived and separated the inmate from the staff member. The officer was transported to an outside hospital for treatment of his injuries. The inmate was medically assessed and treated for his injuries at the institutional hospital. The inmate was later transferred to another institution.

At 8:11a.m., a body alarm was activated in the corridor for an inmate assault in progress, staff witnessed an inmate walk up to another inmate and start to strike the other inmate in the face. Responding staff had to use force on the inmate attacking the other inmate. Both inmates were medically assessed and placed in SHU.

**At 11:51a.m., staff called for assistance in SHU for an inmate on inmate cell fight. Responding staff were able to separate both inmates without further incident.

***At 8:21p.m., a unit officer called for assistance in a housing unit for an inmate on inmate fight in progress. Responding staff were able to separate both inmates involved. One inmate had to be taken to an outside hospital and was admitted, the inmate had his ear bitten off by the other inmate involved in the fight. One inmate placed in SHU and the other admitted to outside hospital.

At 9:55a.m., staff responded to the East Side Recreation cages for an inmate on inmate assault with a weapon. Responding staff were able to restrain and separate the inmates. Both were examined and returned to a cell. The weapon, a homemade sharpened plastic knife, was recovered.
 
**At 4:36p.m., staff responded to D Block for a cell fight involving two inmates. Staff were able to restrain and separate the inmates.

At approximately 8:40 a.m., an inmate who is housed in SHU, assaulted staff by throwing a yellow colored liquid on their person. The liquid struck the staff member on the back and side. The inmate submitted to hand restraints and was relocated to a cell containing an inner grille where he could not repeat his actions. The inmate was medically assessed with no injuries. The officer was medically assessed with no injuries.

At approximately 7:13 p.m., while conducting a random cell search of a cell, the officer attempted to turn on the cell light by using the light switch. The circuit to the light tripped and electrocuted the officer. An inmate admitted to hooking up a stinger wire. He stated he detached the light switch cover and tampered with the wires to use as a heating source for food. Due to the wire being removed, it placed the breaker to the light switch in a fault mode. The officer turned the light switch on and the breaker released a surge of electricity. The officer was sent to the local hospital for medical evaluation. The inmate was photographed, medically assessed and placed in SHU pending disciplinary sanction.

At approximately 10:55 p.m., a body alarm was activated at the Camp. The Operations Lieutenant arrived on the scene and observed the Camp Officer with an inmate in custody. According to the Camp Officer, he was conducting rounds near the laundry room and ordered the inmate to submit to a pat search. The inmate ran into the officer and tackled him to the floor. The Camp Officer grabbed onto the inmate’s torso in an attempt to control the inmate. The inmate refused the officer's orders to stop resisting. The struggle continued and the Camp Officer was able to activate his body alarm. The inmate and the Camp Officer were medically assessed. The inmate was escorted to the SHU for assaulting another person and possession of a hazardous tool.

At approximately 11:26 a.m., SHU staff members were escorting an inmate in hand restraints, from the B range holding cell to the A range holding cell, when the inmate turned towards a staff member and head butted him in the side of his head. The inmate attempted to pull away from staff and was then placed on the floor outside of the holding cell of A range to regain compliance. The inmate was placed inside of the holding cell of A range without further incident. He was escorted to the Health Services Department for a medical assessment where he did not report or sustain visible injuries when assessed by medical personnel. No other staff injuries were reported.

At 8:25a.m., while performing cell rotations on J Block, staff discovered two inmates with injuries consistent of being in a cell fight. The inmates were medically examined and placed in different cells.

At 6:49a.m., staff called for assistance on the compound for a two on one inmate assault in progress. Two inmates were attacking one inmate with weapons. The tower officer fired 2 rounds of munitions and the inmates complied and layed on the ground. Responding staff cuffed and placed all three inmates in SHU.

At approximately 8:45 p.m., the SHU Officer was attempting to remove the hand restraints from an inmate to conduct a visual search. The inmate then reached out of the food slot and grabbed the officer's hand, pulling the handcuffs and keys away from the officer. The inmate was ordered to submit to hand restraints which he complied. Staff and inmate were medically assessed with no injuries noted. Subsequently, the handcuffs and keys were recovered and the inmate was returned to his cell without further incident.

At approximately 11:25 a.m., as an inmate walked past the an Unit Officer, he used his shoulder to intentionally bump into the officer, striking the officer's right shoulder. The inmate was placed in restraints, escorted to Health Services for a medical assessment and placed in SHU.

A medical staff member was physically assaulted by an inmate during the morning pill line. The inmate turned over the pill cart and hit the staff member in the head with the housing unit phone when the staff member attempted to activate 222 for assistance. Responding staff arrived and were able to gain control of the inmate. The inmate was taken to SHU. The medical staff member received a contusion to his head and was sent to see his private physician.

At 7:51a.m., staff called for assistance in the red corridor inmate due to a fight with weapons involved. During the morning feeding inmates were returning to their units using the corridors because of adverse weather. One inmate was assaulted by numerous inmates the inmate attacked was stabbed several times in the chest and side. Responding staff were able to separate all inmates and recover two weapons involved. After further review of the cameras it was found that more inmates were involved in the incident. All inmates were placed in SHU. The inmate attacked and stabbed was transported to a local hospital by ambulance and was admitted. The institution was placed on lock down status until interviews were completed. The lock down was lifted at 11:00 Am.

**At 9:28a.m., staff called for assistance in SHU for an inmate on inmate cell fight in progress. Responding staff were able to separate both inmates without further incident.

At approximately 11:00 a.m., the Assistant Food Service Administrator was monitoring the inmate serving line in the inmate dining room when he observed an inmate open a pack of mayonnaise with his mouth, and spit part of the packet on the floor. The AFSA ordered the inmate to pick the packet up from the floor. The inmate slammed his food tray down and swung at the AFSA with a closed right fist without contacting the AFSA. Responding staff placed the inmate in hand restraints and he was escorted from the area. The AFSA and the inmate were assessed by medical staff which revealed no injuries to either staff or inmate. The inmate was placed in Administrative Detention without further incident.

At 10:30a.m., while participating in a use of force on an inmate who is currently on a hunger strike, a staff member was assaulted. The staff member was attempting to control the inmates left leg when the inmate became combative and kicked the staff member in the groin. The staff member was examined and returned to work.

At approximately 4:20 p.m., in SHU staff witnessed an inmate start a fire and break a sprinkler head. Responding staff ordered the inmate and his cell mate to submit to hand restraints. Both inmates complied and were escorted from the range. During escort, one inmate turned his body aggressively and assaulted staff. Specifically, the inmate became combative and aggressive and spat striking staff on the head and upper torso area. An Immediate Use of Force was conducted on the inmate using minimal amount of force necessary to gain positive control. The inmate was visually searched, given a new issue of clothing, and placed in ambulatory restraints. Medical assessments were completed on staff and inmate with no injuries noted. The inmate was escorted to the observation cell within SHU. The other inmate was placed in a different cell assignment without incident.

A medical emergency was called for a male inmate in the SHU. The inmate was brought to the Health Services area because he was supposedly was having an asthma attack. While the female nurse was getting a nebulizer ready the inmate became aroused and intentionally rubbed his erection against the female nurse. The Operations Lieutenant was immediately notified and the inmate was returned to SHU without further incident.

At approximately 6:00 p.m., an inmate struck a SHU staff member in the groin area through the food slot. Staff used immediate force by grabbing the inmate's arm to stop the assaultive behavior, and to secure the food slot. The inmate attempted to assault staff by striking the staff member in the arm, while staff was controlling the inmate’s arm. The inmates then complied with staff orders to cease their actions and remove their hands from the food slot. Both inmates were medically assessed and returned to their cell without further incident.

At 7:35 p.m., the Unit Officer was monitoring the recreation move at the entrance door when he attempted to conduct a pat search on an inmate. Several orders were given to the inmate to submit to a pat search, but the inmate refused and walked away from the Officer. The Housing Unit Officer followed the inmate on the compound in between the E-2 and D-1 Housing Units and where he noticed the inmate emptying what appeared to be homemade intoxicants into a drain. He again ordered the inmate to submit to a pat search. The inmate immediately turned on the Officer in an aggressive manner and at which point the Officer placed the inmate on the ground with the least amount of force necessary to gain control of the inmate. The Tower Officer immediately radioed for staff needs assistance in front of a Housing Unit and turned on the auto alert tone for Tower #7. Responding staff observed the inmate being resistive towards the E-2 Housing Unit Officer by wrestling with him on the ground. Responding staff assisted placing the inmate into hand and leg restraints. The inmate was escorted to the Lieutenant's Office and breathalayzed with a reading of .000 with the ALCO SENSOR IV. Inmate was escorted to SHU where he was visually searched, placed into a new issue of clothing and placed into hard ambulatory restraints at 8:42 p.m. The inmate was medically assessed and placed into a cell without further incident.

At approximately 10:45 a.m., a Calculated Use of Force Team was authorized due to the disruptive behavior of an inmate. Specifically, the inmate refused direct orders of SHU staff to submit to hand restraints for the purpose of a safety and sanitation inspection of his cell. At approximately 11:00 a.m., confrontational avoidance proved ineffective, the Use of Force team entered the cell and applied hand and leg restraints to the inmate and escorted him from the cell. As the inmate exited the cell, he spit several times in the direction of staff, striking one team member in the left shoulder, and attempted to pull away from the escorting staff. The inmate was placed on the ground to gain control. Once the inmate was compliant, he was moved to the D-Range holding cell. The inmate was visually searched, scanned with the hand held metal detector and placed into ambulatory restraints. He was medically assessed and no injuries were found. A safety and sanitation inspection of the cell was completed and the inmate was placed back in his assigned cell without further incident. No staff injuries were reported during the Use of Force.

At 7:30p.m., an officer came across an inmate in possession of homemade intoxicants. When the officer attempted to confiscate the contraband the inmate attacked the officer. In the process of taking the inmate to the ground, the officer sprained his ankle. An additional officer was injured coming to the aid of the first officer.

At 9:04a.m., staff responded to the East Side Recreation cages for an inmate on inmate assault with a weapon. Staff were able to restrain and separate the two inmates. The victim was taken to Health Services where he was treated and returned to his Housing Unit. The weapon was recovered by staff.

At approximately 9:00 a.m., the SHU Officer was removing the hand restraints from an inmate while he was in a cell. Upon the removal of hand restraints from one hand, the inmate pulled away from the staff member causing the staff member's hand to hit the food slot. The restraints were then removed by another officer and the food slot was secured. Subsequently, staff and inmate were medically assessed with minor injuries noted to staff. The inmate was returned to his assigned cell without further incident.

**At 11:55a.m., staff called for assistance in the green corridor. During the lunch feeding two inmates began to argue in the green corridor. One inmate then sucker punched the second inmate in the face both inmates then began to fight. Responding staff were able to secure food service and both inmates involved without further incident. Both inmates were placed in SHU.
 
***At 12:54 p.m., staff called for assistance in a housing unit team area when an inmate began arguing with his unit team members. The inmate sucker punched the staff member in the face splitting his upper lip open, a second staff member that responded to try and secure the inmate had one of his fingers broken when the inmate grabbed it. The third staff member received bruising and swelling to the cheek area while the staff member was trying to secure the inmate on the floor. Responding staff were able to secure the inmate with hand and leg restraints but the inmate continued to fight staff and refused to walk or stand. The inmate was placed in the boss chair and escorted to SHU. One staff member received bruising to her cheek area and treated at the institution hospital. One staff member sent to outside hospital for X-Rays on his broken finger. The third staff member was taken to outside hospital and received sutures in his upper lip, the housing unit that this incident happened in was placed on lock down status and the rest of the institution went back to normal.

At approximately 7:35 p.m., in a SHU cell an inmate assaulted Special Housing staff with an unknown liquid substance, striking the officer in the upper torso, legs, and feet. Specifically, the officer was collecting food trays on B-Range when an inmate threw the liquid through the food slot. A medical assessment was completed on the officer and inmate. The inmate was placed in SHU.

While picking up food trays on the SHU range, a USP/Pollock holdover reached through the food slot and cut an officer's left forearm with a razor blade. He was taken to the hospital and had surgery to repair the cut. It was an approx. 12 inch cut, and the doctors expect the officer to make a full recovery. The inmate was immediately shipped to Pollock.

At approximately 1:23 p.m., the Control Center announced a body alarm from an officer. The unit officer was conducting a pat search of an inmate and felt a rectangular heavy object in his right sock. The inmate turned and pushed past the unit officer, the unit officer then pushed the inmate back off of him and ordered the inmate to get back. The inmate ran into the unit officer, knocked the officer into the wall and onto the ground. The unit officer attempted to secure the inmate by holding onto his torso. The inmate fled the immediate area and the unit officer gave chase. Responding Staff and the Operations Lieutenant were able to place the inmate in hand restraints without further incident. The inmate and the unit officer were medically assessed with injuries and medical treatment was provided. The inmate provided a urine sample and was given a breathalyzer, both yielded negative readings. The inmate was escorted to the SHU for assaulting another person.

At 6:45p.m., staff called for assistance in the food service dining hall while the evening meal was being served. Two inmates began fighting each other in the dining hall. Responding staff were able to separate both inmates and secure the dining hall without further incident. Both inmates were placed in SHU.

At approximately 12:30 p.m., an inmate was ordered to submit to restraints in SHU after having flooded his cell and range. The inmate covered the window to his cell door and threatened to harm staff if they attempted to enter the cell. Confrontational avoidance was started at 1:10pm, and after repeated efforts to gain the inmates voluntary compliance, he submitted to hand restraints. The cell door was opened, and as staff attempted to escort him from the cell, he immediately tried to pull away and run towards the camera operator. SHU staff placed the inmate against the wall with the assistance of other staff. The inmate continued to resist, and then spat in an officer’s face who was attempting to restrain him. Escorting staff then placed the inmate on the floor to control his actions and assaultive behavior. Leg irons were applied as the inmate continued to kick at staff after he was placed on the ground. Once the inmate appeared to calm down, he was escorted to the observation cell on B-range. His clothing was exchanged, and he refused examination by medical staff. It should be noted that the inmate was observed with no visible injuries and due to his continued assaultive behavior, the Warden authorized the use of ambulatory restraints. There were no injuries reported by staff who participated in the immediate use of force.

At 09:02 a.m. staff assistance was requested to the Health Service Department regarding an inmate who became assaultive toward staff. Specifically an inmate broke his wooden crutches on a water fountain and then raised them up in a threatening manner, during this incident pieces of the broken crutches hit staff in the face and chest area. At that point staff placed the inmate on the ground and restraints were applied. The inmate was medically assessed and was placed in SHU without further incident.

At 9:00a.m., an inmate walked away from the LEC. The inmate was later found at 1:24a.m., on 4/22/11 attempting to return. U.S. Marshals were notified of the escape and of the return. The inmate was brought inside the U. S. P.

At approximately 3:12 p.m., in the secure area of the south compound metal detector, an inmate attempted to clear the metal detector and failed. Staff attempted to conduct a pat search on the inmate and he assaulted staff. Specifically, the inmate turned around and struck staff on top of their head with his right elbow during the pat search. The inmate then stood in a fighting stance, striking the staff member on the right arm. Subsequently, staff conducted an Immediate Use of Force and placed the inmate on the ground to gain positive control. Responding staff placed the inmate, who continued to be combative, in hand restraints. The inmate was visually searched, breathalyzed, and medically assessed. Two staff were also medically assessed. Due to his ongoing combative and disruptive behavior, the inmate was placed in ambulatory restraints, medically assessed again, and placed in SHU.

At approximately 10:10 p.m., an unit officer was securing the unit for count. He approached a cell and the window to the cell was covered. The officer opened the cell door and witnessed an inmate with a homemade weapon in his hand. The officer ordered the inmate to hand over the weapon. The inmate pushed through the officer and ran to the shower, where he threw the weapon into the drain. Once responding staff arrived, the inmate was pat searched and escorted to the Lieutenant's Office. The inmate was scanned by a metal detector, visually searched, medically assessed and placed in SHU pending disciplinary action. There were no staff injuries.

At approximately 6:40 p.m., SIS staff entered a cell that was occupied by two inmates one inmate was out of bounds. Both inmates were observed with a green, leafy substance in their hands. One inmate jumped up from the table with a green, leafy substance in his hand. The SIS officer called for staff assistance. As the SIS officer stood in front of the toilet, the inmate aggressively moved toward the toilet with the green, leafy substance and pushed the SIS officer in the chest, causing her to fall to the floor. Responding staff arrived and both inmates were placed in hand restraints. The inmate was escorted to Health Services for a medical assessment, and later escorted to SHU where he was placed in Administrative Detention. The other inmate was escorted to SHU and placed in Administrative Detention. The one inmate was successful in flushing most of the green, leafy substance in the toilet after pushing the officer down to the floor. However, enough of the green, leafy substance was recovered in the cell, and staff were able to conduct a test of the green, leafy substance utilizing the NIK testing kit E. The green, leafy substance tested positive for marijuana.

At approximately 7:55 p.m., staff placed an inmate in hand restraints so the Mid-level Practitioner could conduct a medical assessment on him in the Psychology Services suicide-watch cell due to possible injuries incurred while kicking and striking the cell wall with closed fists. The inmate sustained minor bruises and bleeding to his hands and right toe areas. Upon the completion of the medical assessment, staff attempted to remove the hand restraints through the food slot of the cell door; however, due to a malfunction with the locking device, they were unable to remove the restraints. Staff entered the cell to determine the malfunction of the hand restraints and once the malfunction was corrected, the restraints were removed from the inmate. At that time, the inmate became extremely violent and aggressive toward staff by striking a staff member with closed fists in the neck area. Staff immediately placed the inmate on the ground to regain control and applied hand and leg restraints to the inmate without further incident. An additional medical assessment was conducted on the inmate which revealed a minor nose bleed. Two staff were injured during the assault: the first staff member sustained a contusion to the left neck area, swelling and abrasions to the left elbow, and the other staff member sustained superficial abrasions to both hands.

At 9:42a.m., staff responded to D Block for an inmate on inmate assault. After placing both inmates back in their assigned cell, staff removed the restraints from the first inmate. Upon completion, the inmate turned and began kicking and hitting the still restrained inmate. Staff were able to restrain the assaultive inmate and separate the inmates.

**At 1:45p.m., staff responded to an assault on staff in G Block. The inmate was in the shower and threw a razor at a staff member, striking the staff member in the leg. The inmate then began threatening the staff member.

At 8:23a.m., staff responded to E-Block for a fight involving six inmates on the range during their 2 hours allowed out of their cells. Responding staff were able to restrain and separate the inmates. All six inmates were examined by medical staff and escorted to a Phase 1 Block where they are now housed. There were no staff injuries reported.

At 2:40 p.m., staff observed two inmates fighting with each other in front of an unit. Specifically, staff saw one inmate brandishing a prison made weapon as he was attempting to stab the other inmate. Both inmates refused orders to cease their actions. As responding staff were placing the inmates on the ground, one officer received a puncture wound to his right forearm. Both inmates were placed in restraints and escorted to separate holding cells where they were breathalyzed and medically assessed on video camera. One inmate sustained three puncture wounds, a contusion to his lip and minor abrasions. The other inmate received minor abrasions. Additionally, one inmate had a positive Alco Sensor reading of .085 at 4:24 p.m. and refused the confirmation breathalyzer test. The other inmate had a negative reading. The injured staff member was transported to an outside medical facility where he was treated for his injuries and released. Both inmates were placed in SHU without incident.

At approximately 7:50 p.m., while being escorted on the B-2 range of SHU, an inmate assaulted a staff member by kicking the staff member in the right leg. An immediate use of force was conducted by the staff member, placing the inmate on the ground to control his actions. Once on the ground staff applied leg restraints and the inmate stopped his disruptive behavior. He was then escorted from the range, medically assessed and returned to his cell without further incident.

At approximately 8:10 p.m., the Compound Officer was conducting a visual search of an inmate in the Lieutenants Office holding cell. After advising the inmate to remove his boxer shorts, the inmate proceeded to assault staff by taking them off and throwing the boxer shorts in an aggressive manner, striking the officer in the chest area. The officer and inmate were medically assessed and photographed. The inmate was escorted to SHU without further incident.

At 2:40p.m., a staff member was injured after he tried to break up a fight between two inmates. The rest of the staff quickly gained control of the fight and temporarily confined all of the inmates to their cells. The staff member was treated for non-life-threatening injuries.

At approximately 3:40 p.m., the Camp Visiting Officer and Visiting Room Officer were conducting visual body searches in B-Unit at the Federal Detention Center. Upon unlocking a cell, both officers instructed an inmate to remove his shirt. The inmate became verbally abusive and refused direct orders from the officers. The inmate approached the Visiting Room officer in an aggressive manner, and bumped the officer's chest with his chest. A call for assistance was announced both officers gained control of the inmate and placed him in restraints. The inmate was medically assessed and placed in SHU without further incident. Both officers were medically assessed with no injuries noted.

At 1:45p.m., an officer was shaking down an inmate's cell and confiscated pornographic pictures. The inmate punched the officer repeatedly until additional staff arrived. The officer was taken to the local hospital. The officer is now reportedly doing well. He sustained a major laceration around his left eye and trauma to his mouth. The institution was placed on lock down status.

At 3:45p.m., a body alarm was activated by a housing unit officer. A large amount of the inmates within the unit were squaring off with each other, and refusing to lock down. Responding staff arrived, and requested chemical munitions to assist on gaining compliance. Upon the arrival of the munitions, the inmates complied and returned to their cells. The housing unit was placed on lock down status for the remainder of the day.

One inmate was transported to the local hospital due to the injuries he sustained. USP Lee resumed normal operations on 4-12-11.

At approximately 7:30p.m., an officer was assaulted at a local medical center. Two hospital escort officers were walking in a hallway with an inmate when another individual approached the officer from behind and poked his fingers into the officer’s kidney area, stimulating he had a weapon. The individual stated "give me the gun" and the officer used immediate force to strike the individual away from him. The officer was able to get a safe distance away from the individual. The individual fled and was later found and identified as a patient at the medical center.

**At 12:30p.m., during mainline feeding staff became aware of an inmate trying to hide injuries. Upon further investigation the inmate had a swollen jaw and bleeding from his head. Staff examined the cameras from the housing unit and discovered that two other inmates assaulted the one inmate in his cell and taken his property. The inmate was taken to outside hospital for treatment and returned to institution. All inmates were placed in SHU.

Both inmates were trying to stab each other with homemade weapons. Responding staff were able to separate and control both inmates. Other staff secured the rest of the inmates in the dinning area. Both weapons were retrieved by staff. One inmate was slashed in the facial area and received treatment at the institution hospital. Both inmates were placed in SHU.

At 11:00p.m., a staff member was making rounds in the range. The staff member looked over the top of a toilet stall and observed an inmate holding a cell phone. The staff member ordered the inmate to hand over the cell phone and the inmate refused. The officer then ordered the inmate to open the stall door and he complied, but attempted to push past the staff member. The inmate then dropped the phone and kicked it across the floor. The staff then called for assistance. The inmate then attempted to push the staff member in order to get by. Staff gain control of the inmate and he was placed in SHU.

**At 12:06p.m., staff responded to an attempted assault on staff. When staff opened the food slot, the inmate threw his trays at staff and turned around to retrieve more items to throw. A Lieutenant was present and utilized gas. The inmate was restrained and removed from the cell.

At approximately 3:15 p.m., an inmate assaulted the SHU Officer by spitting on her as he was being taken to the shower. The inmate was escorted to the Health Services Medical Examination Room to be medically assessed and photographed. As the SHU Officer was attempting to photograph the inmate, he assaulted the officer by turning and spitting on him. He attempted to spit on the staff member again, so staff placed him against the wall to gain control of him. Videotaping was initiated due to the use of force. As staff were waiting for medical to arrive, the inmate became upset because staff would not remove his hand restraints. The inmate attempted to pull away from staff. When he was unsuccessful, he began hitting his head against the wall. Due to his disruptive actions, staff placed him on the ground to maintain control of him. Once he regained his composure, staff lifted him and placed him into his wheel chair. He was medically assessed and placed back into his cell without further incident.
 
**At approximately 11:30 a.m., as SHU staff were collecting the food service trays, an inmate assaulted the SHU Officer by throwing an unknown liquid substance through the food service slot, striking the officer on the lower right leg. The inmate was restrained and escorted to the Health Services Exam Room inside SHU and medically assessed. He was placed back into his cell without incident.

At 11:00 a.m., as staff was conducting a random search of a cell an inmate became agitated and swung a pair of government issued pants striking the staff member on the left leg. The inmate was placed in hand restraints and escorted to SHU without incident. Medical assessments were completed with no noted injuries to the staff member or inmate.

At approximately 4:05 p.m., the Food Service Foreman observed an inmate stealing two pieces of beef slices from the serving line. The Food Service Foreman then retrieved the food items from the inmate and asked for his identification card, at which time the inmate threw his ID card, striking the Foreman in the chest. The Operations Lieutenant was notified, reported to Food Service and escorted the inmate to SHU without any further incident. Both the staff member and inmate were medically assessed and found to have no injuries.

At approximately 12:00 p.m., an inmate refused to close the food slot and insisted on speaking to the Special Housing Officer in Charge. When the OIC and an additional staff member approached the cell, the inmate proceeded to assault both staff members by throwing urine on them and spitting on them. The inmate complied with staff's orders and submitted to hand restraints. Staff escorted him out of the cell to be medically assessed. The assaulted staff were also medically assessed. The inmate was returned to his cell, and an incident report was written.

At approximately 2:45 p.m., the Staff Psychologist was conducting a suicide risk assessment with an inmate. During the interview, the inmate spat on the Staff Psychologist three times hitting him in the left eye, left arm, and torso area. Both staff and inmate were medically assessed with no injuries noted. The F.B.I. was notified and an investigation continues.

At 10:45 a.m. a staff member was serving the noon meal in Special Housing and was assaulted by an inmate. Specifically, The officer opened the food slot and handed the inmate his trays. While officer was attempting to close the slot the inmate forcefully and in an aggressive manner, shoved the slot opened. The tray slot struck the staff member in the left hand. The officer then closed the food slot without further incident. The officer then reported the incident to the Special Housing Lieutenant and all notifications were made. Both the officer and the inmate were medically assessed and there were no injuries to either.

At approximately 6:50 a.m., an inmate assaulted staff in the Laundry Department by throwing a pair of institution boots, striking staff in the leg. The inmate was placed in hand restraints, medically assessed, photographed and placed in the Special Housing Unit without further incident.

At approximately 10:15 am, staff were escorting an inmate when he attempted to pull away from staff. Staff then utilized immediate use of force to place him on the ground to gain control of him. When the inmate was placed on the ground, he bit a staff member's forearm area. The inmate would not release the Staff member's arm until additional staff pulled him away. Staff then attempted to apply restraints to the inmate. During the application of restraints, he was still being combative and noncompliant. Once restraints were applied, staff began to move the inmate off the range when he again became combative. Staff placed him on the ground a second time in order to maintain control of his actions. When additional Staff members arrived, he was placed in soft ambulatory restraints due to his disruptive and assaultive behavior.

At approximately 8:54 a.m., an inmate became disruptive in the Special Housing Unit. Specifically, when staff were escorting the inmate to inside recreation he attempted to pull away from escorting staff. The escorting staff immediately placed the inmate against the wall to gain compliance. Once staff regained compliance inmate He was placed back in his assigned cell. Once inside the cell, the inmate turned and spit, striking the staff member in the facial area. Both staff and inmate were medically assessed with no injuries reported.

An inmate approached the officers station and asked if the pill line had been in. The inmate was informed that it had already been in the unit. The inmate was told medical would be called. At this point the inmate reached into the officer’s station and picked up an iron which was sitting on the cabinet and punched the officer’s station window to the right side of the door, shattering it. The officer received minor injuries.

At approximately 1:25 p.m., the Unit Officer, while releasing the inmates for the recreation move, was assaulted by an inmate. Specifically, the inmate patted the Unit Officer on her right rib cage area as he was exiting the unit. The inmate was escorted to the Lieutenant's Office, medically assessed, and placed in the SHU without further incident. The Unit Officer was medically assessed, found to have sustained no apparent injuries.

At approximately 5:35pm the SHU officer was assaulted by an inmate. While the officer was attempting to retrieve the inmate’s food tray from the food slot, the inmate used a broken food tray as weapon to assault the officer. Specifically, the inmate used the broken portion of the tray in a slicing motion causing injury to the right knuckle area of the officer. The officer secured the food slot without further incident and notified the Operations Lieutenant. The staff member was assessed by institutional medical staff and received a small abrasion to the 4th knuckle on his right hand.

At approximately 3:00 p.m., staff was escorting an inmate to his assigned cell. As they arrived at the cell door, the inmate began speaking to staff in an aggressive manner and became increasingly agitated. As he was placed into the cell, he spit on the upper torso of escorting staff. The cell door was closed and the inmate was secured in the cell without further incident. Medical assessments were completed on the staff member and inmate. No injuries resulted from this incident.

At approximately 8:50 a.m., an unit officer entered a cell to conduct a cell search. The staff member ordered the inmate to submit to a visual search; however, he became disruptive by cursing at staff. The inmate removed his shirt and threw it at the staff member striking him in the face. The inmate was escorted to the Lieutenants' Office, medically assessed, and placed in the SHU without further incident.

At approximately 6:40 a.m., the SHU Officer attempted to feed an inmate inside SHU. The inmate assaulted the staff member by shoving his crutch through the food service slot striking the officer in his groin area. The inmate refused to submit to hand restraints to be removed from his cell to be medically assessed. A Calculated Use of Force team was authorized and assembled. Confrontation Avoidance practices were implemented and proved effective. The inmate was restrained, removed from his cell and medically assessed without any issues.

At approximately 1:30 p.m. while escorting an inmate to the upstairs shower area in SHU, the inmate was secured in a shower. Upon removing the hand restraints, the inmate became irate and threw a shampoo bottle out the trap, hitting the SHU officer on the left side of the facial area. The trap was immediately secured. An injury assessment was conducted on the SHU officer, he sustained minor swelling of the left cheek area. An injury assessment was refused by the inmate.

At approximately 12:55 a.m., an inmate became disruptive and assaultive at an outside medical facility. The inmate became violent and assaulted the escorting Lieutenant by spitting on him. The inmate attempted to spit and bite outside medical staff while trying to take the IV tube out of his arm. Staff had to use immediate force to gain control of the inmate to prevent further assaults and disruptive behavior. At approximately 2:55 a.m., the escorting staff arrived back at the institution with the inmate. The inmate was escorted to SHU and placed in ambulatory restraints due to his assaultive and aggressive behavior.

At approximately 12:55 p.m., during the Recreation movement, an inmate ran into the Supervisor of Recreation's left arm with his left shoulder. The inmate was pat searched and escorted to the Lieutenants' Office. The inmate was subsequently placed in SHU without incident.

Staff responded to E Block when an inmate began kicking and hitting a window made of reinforced glass in the cell. The inmate used enough force that he shattered the window, which had chicken wire running through it, and pieces of the window landed on the range floor. Responding staff were able to restrain the inmate, place him in to ambulatory restraints and move him to a cell.

At 5:57 p.m., the Housing Unit Officer called for assistance after observing two inmates engaged in a physical altercation. Specifically, the unit officer observed one inmate stab the other inmate with a prison made weapon. The unit officer also witnessed one inmate strike the other inmate with a plastic chair. During the altercation the unit officer was struck in the back of the head with the plastic chair. Responding staff utilized immediate force in order to gain compliance from both inmates. Both inmates refused a breathalyser test. Both staff and the inmates were medically assessed, staff sustained minor injuries. One inmate received minor first aid for a one inch puncture wound to the upper left torso area. The other inmate sustained no injuries. Both inmates were escorted to SHU without further incident.

At approximately 1:15 p.m., an inmate assaulted SHU staff by squirting an unknown liquid substance through a crack in the food slot. The liquid substance struck the staff member on the pants and shoes. Staff exited the range and the Operation's Lieutenant was notified. The inmate complied with staff's orders and submitted to hand restraints. The assaulted staff and the inmate were both medically assessed and photographed. The inmate was placed back in his assigned cell without further incident.

At 9:09a.m., staff was assaulted while attempting to escort an inmate to the outside recreation cages. The inmate became combative after being told to by another inmate and head butted the staff member who then took the inmate to the ground. Additional staff escorted the inmate back to the block.

**At 9:56a.m., while conducting a range tour on D Block, Staff discovered an inmate hanging from the bed. Staff arrived and entered the cell. After staff cut the inmate down, CPR was initiated. The inmate was taken to the outside hospital where he was pronounced dead.

***At 12:05p.m., staff was assaulted in Z Block while attempting to collect food trays. The inmate refused to return his trays from the lunch meal. After the Lieutenant arrived, the inmate agreed to return his food trays. When staff opened the food slot, the inmate threw his trays striking both staff members in the legs.

The inmate was placed in the holding cell and medically assessed by the Physician's Assistant without further incident.

At 7:55p.m., staff called for assistance in a housing unit for an inmate on inmate fight with a weapon involved. The inmate attacked the other inmate with a lock in a sock striking the inmate in the head several times the inmate that was assaulted then turned on the attacker striking the inmate with closed fists and knees to the head, both inmates were taken to an outside hospital with one inmate being admitted to the hospital ICU. The other inmate was returned to the institution and placed in SHU. There were no staff injuries.

Subsequently, the inmate was returned to his cell without further incident.

The inmate continued his disruptive behavior by spraying a liquid substance out of the cell, striking staff in the upper torso area. Both the staff and inmate were medically assessed, photographed and the inmate was returned to the cell without further incident.

At approximately 6:10 pm, an Unit Officer was attempting to conduct a random cell search. The unit officer instructed the inmate to exit the cell. The inmate refused staff orders, pulled a blue bag filled with a liquid substance from his locker, and ran towards the unit officer in an aggressive manner. The inmate pushed the unit officer in the upper torso area in an attempt to bypass the officer and dispose of the liquid in the cell's toilet. The inmate was successful in pushing the officer out of his way and spilled the liquid substance on the officer in the process of pushing him. The liquid was spilled on the unit officer’s lower left pant leg. The inmate was escorted to SHU.

At 8:38a.m., staff responded to an two on one inmate assault in the recreation cages. Responding staff were able to restrain and separate the inmates and escort them back in to the housing unit.

**At 9:38a.m., staff responded to C Block for an inmate on inmate assault. Staff were able to restrain and separate the inmates. A five and a half inch homemade metal knife was found in the toilet by staff and the victim had about 6 stab marks on his body.

***At 2:09p.m., staff responded to B Block for an inmate on inmate assault. After staff had placed hand cuffs on the first inmate, the second inmate began hitting the other inmate with closed fists. He then picked up a pencil and began stabbing him. Responding staff were able to restrain the unrestrained inmate and separated the two. Both inmates were examined by medical staff.

****At 10:54p.m., staff responded to a cell fire in J Block. Responding staff used gas to get the inmates to comply and had to take one of the inmates to the floor as he became combative.

At approximately 2:00 p.m., while attending a UDC hearing in SHU, an inmate became angry, stood up and shoved the correctional counselor into the wall with his shoulder and threatened her. Staff then escorted the inmate back to his cell. The staff member and the inmate were medically assessed with no injuries noted.

At approximately 1:40 p.m., the Operations Lieutenant was contacted by the Camp Administrator and reported an inmate had just assaulted the Camp Counselor by running his body into the right shoulder of the Counselor, knocking him backwards. This occurred after a heated discussion between the inmate and the Camp Administrator. The inmate was placed in restraints and escorted to SHU without incident. The Camp Counselor was medically assessed and received no injuries. The inmate was assessed and also received no injuries.

After placing the inmate in a recreation cage, he turned over his left shoulder and spit in the face of the recreation officer. The inmate was then removed from the recreation cage, photographed and escorted back to his assigned cell. The staff member and inmate were medically assessed without injuries.

While SHU staff were walking pass a recreation cage occupied by two inmates an inmate took a container filled with what appears to be urine and feces and squirted it at staff striking them in the facial and upper torso area. Confrontation avoidance was initiated at approximately 10:50 a.m., and proved to be successful. Both inmates submitted to restraints. One inmate was placed in soft ambulatory restraints to deter future assaultive behavior.

The staff and inmate involved received medical assessments and an incident report was written.

In the Lieutenant's Office holding cell, staff was attempting to place hand restraints on an inmate when the inmate became combative and assaulted staff by spitting and kicking at them. The inmate was placed on the ground and hand and leg restraints were applied. The inmate was medically assessed and escorted to SHU. The inmate continued to be disruptive and was placed in ambulatory restraints until a calming effect could be achieved.

At 6:13a.m., staff responded to an inmate on inmate assault. Staff witnessed an inmate striking another inmate with closed fists. Responding staff were able to restrain and separate the inmates.

At 2:30 p.m., staff responded to a duress alarm for an attempted suicide of an inmate. The inmate’s cell mate was kicking on the cell door alerting staff of his cell mate’s. Upon arriving to the cell, the SHU Lieutenant observed the inmate with one end of a bed sheet tied around his neck and the other end of the bed sheet tied to the ladder of the bed. The SHU Lieutenant gave both inmates verbal orders to submit to hand restraints. One inmate complied; however, the other inmate refused. Additional staff arrived. Due to the inmate that was still trying to hang himself and refusing to submit to hand restraints, the SHU Lieutenant entered the cell with staff in an attempt to subdue him. The inmate became very combative by trying to strike the SHU Lieutenant with a close fist. Staff placed the inmate on the ground to gain control. Additional staff responded, and the inmate was placed in hand and leg restraints. Staff removed the inmate from the cell, pat searched, metal detected, and escort him to Health Services. Health Services staff examined the inmate with no injuries being noted. The inmate was placed in soft ambulatory restraints and placed in the Health Services observation cell. Psychology Services staff placed the inmate Francois on Suicide Watch. Due to the inmate’s continued disruptive behavior, the Warden authorized the inmate to remain in soft ambulatory restraints. Two staff member involved in the immediate use of force received minor injuries.

At 9:40a.m., staff was assaulted in the outside recreation area when an inmate broke free from his escort and charged at a staff member who was escorting another inmate. The inmate kicked the staff member in the left arm before responding staff were able to regain control of the assaultive inmate. The staff member was examined and found to have suffered a bruise to his left arm.

While staff were escorting a cuffed inmate from his assigned cell in SHU the inmate lunged backward at one of the escorting officers bumping into the officer and trying to grab at the officers groin area. The inmate was placed on the floor. There were no staff injuries.

**At 3:15p.m., a staff needs assistance in Lt's office call was announce. An inmate was escorted to the Lt's office from the education department, once in the Lt's office the inmate was being strip searched and a homemade weapon was found on the inmates body by one of the officers. The inmate then lunged toward the officer trying to grab the weapon from the officers hand. The inmate was placed on the floor and placed in SHU.

At approximately 1:15 p.m., an inmate assaulted staff while being escorted to the outside recreation cage. The inmate assaulted staff by pulling away before entering the outside recreation area. The inmate was placed on the floor using the minimum amount of force necessary to control him, however, he became aggressive and attempted to kick staff. Responding staff assisted in placing the inmate in leg restraints for escort purposes. He was medically assessed without further incident.
 
**At approximately 10:15 a.m., staff was assaulted by an inmate while conducting weekly rounds of SHU. As staff approached an inmate’s the inmate requested that the food slot be opened in order to return several completed psychology books. Upon opening the food slot and receiving the books, the inmate threw an unidentified liquid substance on the staff member, striking the staff member in the facial area. The staff member called for assistance, and upon arrival, the inmate threw another container of an unidentified liquid from the food slot, striking the staff member in the facial and upper torso area. The staff members were able to secure the food slot without further incident. A Use of Force/Confrontation avoidance Team was assembled for the purpose of placing the inmate in restraints. The cell mate inmate was directed by staff to submit to restraints, and he complied. The other inmate submitted to restraints and was placed into soft ambulatory restraints without further incident. The staff members and the inmate were medically assessed and found to have no injuries.
 
***At approximately 8:20 a.m., while conducting recreation and weekly rounds of the Special Housing Unit, an inmate assaulted several staff near the outside recreation cage. Specifically, Segregation staff were attempting to remove the inmate from the recreation cage after completing his one hour of outside recreation. The inmate refused to submit to restraints. He became very agitated and upset. The inmate walked to the end of the cage and picked up a milk carton. The inmate ran to the front of the cage towards staff and threw a mixture of urine and feces on several staff members, striking them in the facial, upper and lower torso areas. A Use of Force/Confrontation Avoidance Team was assembled. Confrontation Avoidance proved to be successful, and the inmate submitted to restraints. The Use of Force Team removed the inmate from the Outside recreation cage and placed him in soft ambulatory restraints without further incident. Staff members and the inmate were medically assessed and found to have no injuries.
 
****At approximately, 1:00 p.m., the fire alarm was activated in a range. Segregation staff responded to the affected areas and observed fire inside of a cell occupied by two inmates. A fire extinguisher was retrieved, the food slot was lowered, and both inmates were directed to submit to restraints. The inmate refused to submit to restraints and refused to allow the other inmate to submitted to restraints. The fire extinguisher was activated inside the cell through the food slot. Once sufficient staff arrived the cell door was opened and one inmate was escorted from the cell without incident. As staff attempted to escort the other inmate from the cell, he lunged toward staff and began striking at staff. The inmate was removed from the cell and was placed in soft ambulatory restraints.

At approximately 3:00 p.m., the SHU Officer returned an inmate to his cell. Once in the cell, the inmate as ordered to allow staff to remove his restraints which he refused. The officer then attempted to close the food slot and the inmate kicked the food slot open striking the officer on the right wrist. Subsequently, staff and inmate were medically assessed with minor injuries noted to staff. The inmate was returned to his cell without incident.

At approximately 6:45 p.m., immediate use of force was used on an inmate in the Law Library of SHU. Specifically, the Activities Lieutenant was investigating an incident report on an inmate when he stood up and began yelling at the Lieutenant. The inmate refused orders to sit down, physically turned against, and pushed into the Lieutenant. The inmate was then placed on the ground to regain control of him. The inmate was escorted to the close observation cell, photographed, medically assessed on video, and returned to his assigned cell in SHU.

At 1:05 a.m., in Gerard Unit, the Compound Officer observed an inmate in possession of a cell phone. The Compound Officer ordered the inmate to give him the cell phone, which he refused. The inmate grabbed the officer's right arm and pushed him against a locker. The inmate then fled from Gerard unit onto the inner compound. The officer called for assistance and began to pursue the inmate. Responding staff observed the inmate with the cellular phone and observed him throwing the phone into the pond that is located on the inner compound. The inmate refused orders from responding staff to lie down on the ground and submit to hand restraints. The inmate was placed on the ground, restrained and escorted to SHU without further incident. The Compound Officer was medically assessed and sustained abrasions to the left side of his neck, and a contusion on his left knee. The inmate was medically assessed and did not sustain any injuries.

At 7:30a.m., staff discovered two inmates assaulting one inmate in the outside recreation cages by because he threw an unknown substance on them. Inmates were medically assessed and returned to their cells.

At 9:30p.m., a female officer was assaulted by a male inmate in SHU. She opened the food tray slot to give the inmate a towel and the inmate reached out from the slot and sliced her hand with a razor blade that he had removed from a disposable razor. The inmate then attempted to grab her wrist and pull her towards the food tray slot. The officer, despite having received a 1 1/2 inch laceration on her right hand, was able to secure the tray slot without further incident. The officer was medically treated and sent to the local hospital where she received 3 stitches. The inmate remains in SHU and the incident has been referred to the FBI.

At approximately 3:20 p.m., an inmate became disruptive in an unit. Specifically, while staff were passing cleaning supplies, the inmate threw a cup of water, striking the staff member on the left side of the face and shoulder. Medical assessments were completed on both staff and the inmate, with no injuries noted. The inmate was escorted to SHU without further incident.

At approximately 7:55 p.m., while staff were collecting food trays from the evening meal in SHU, an inmate proceeded to throw his Styrofoam food tray from his food slot striking the officer in the chest and arm area. Staff and inmate were medically assessed with no injuries noted. The inmate was returned to his cell without further incident.

At approximately 8:30 a.m., an inmate assaulted a medical staff member while making rounds. Specifically, while the Physician’s Assistant (PA) was performing his weekly rounds of SHU, the inmate threw an unidentified liquid substance on the PA, striking him in the face and upper torso area. The staff member was able to secure the food slot. The inmate submitted to restraints and was moved. The inmate admitted that he was agitated and did commit the prohibited act. The staff member and inmate were medically assessed and found to have no injuries.
 
**At 1:45 pm, SHU staff called for assistance on a range. Staff were escorting an inmate, when he became aggressive by pulling away from staff. He assaulted staff by spitting at them, striking them in the facial area and upper torso. Staff gained control of the inmate by placing him on the ground. Responding staff escorted the inmate to a holding cell where he was medically assessed with no injuries noted.

At approximately 5:25 p.m., the an unit officer was monitoring the metal detector when an inmate walked through to speak with the evening watch Officer. As the inmate walked past the Officer, he struck the officer with his right hand on her right shoulder. The Operations Lieutenant was notified. The staff member did not receive any injuries. The inmate was escorted to SHU.

At approximately 6:10 p.m., the Activities Lieutenant was conducting a pat search of an inmate in the corridor in front of the Food Service shakedown gate. While conducting the pat search, the Activities Lieutenant felt something in the inmate’s pants pocket. As the Activities Lieutenant attempted to retrieve the item from the inmate's pocket, the inmate grabbed the Activities Lieutenant's left wrist and pulled the Activities Lieutenant's hand from his pocket and stepped forward. The Activities Lieutenant immediately restrained and placed the inmate on the ground to gain control. Responding staff assisted in placing the inmate in hand restraints. The inmate was escorted to Health Services where he was examined and treated for minor injuries. The Activities Lieutenant was medically assessed with no injuries noted. The inmate was placed in Administrative Detention without further incident.

At approximately 9:05 a.m., an inmate was being escorted from his assigned cell to SHU recreation yard. The inmate was placed in the SHU recreation yard and staff removed his restraints. Immediately following the restraints being removed, the inmate turned and spit in the face of one of the Escorting Officers. The inmate complied with staff's commands to be restrained and he was removed from the SHU recreation yard and was returned to his assigned cell without incident. The Escorting Officer was examined by Health Services Staff with no apparent injuries at this time.

At approximately 4:35 p.m. an officer issued a food tray to an inmate through the food slot. The inmate retrieved the food tray from the food slot and threw it out the food slot hitting the officer on the upper right leg. The food slot was secured without further incident. The officer was medically assessed with no injuries. The inmate was medically assessed with no injuries and placed on Administrative Detention.

At approximately 6:31 p.m., an inmate was directed to present his ID card during a pat search in the Black corridor. When staff reached out to retrieve the card, the inmate became argumentative and slapped the staff member's hand. The inmate was escorted to the Lieutenants Office where he was photographed, breathalyzed and medically assessed in which he received no apparent injuries. The inmate was placed in SHU without incident. The staff member was medically assessed and found to have no notable injuries.

As SHU staff escorted an inmate to the Recreation Cage, he began resisting staff. Once in the Recreation Cage, the inmate turned his body in an aggressive manner, charging and striking the recreation gate. The inmate proceeded to assault staff by spitting at staff, striking them in the face and head area. Consequently, staff were unable to secure the recreation gate. The inmate then maneuvered his hand restraints to the front. Staff entered the Recreation Cage and placed him on the ground for control. Staff applied leg restraints and reapplied hand restraints. Once secured, staff proceeded to escort the inmate to the Exam Room for a medical assessment. The inmate continued to assault staff by repeatedly spitting on the escorting officer. Staff involved were also medically assessed. An incident report was written.

At approximately 10:03 a.m., the Unit Officer called for assistance due to a fight in a cell. Specifically, staff observed two inmates striking each other with closed fists about the head and upper torso area. Staff ordered both inmates to cease their actions and submit to restraints, to which they refused. The Unit Officer attempted to separate the inmates; however, one inmate struck the staff member in the face with his elbow. Additional staff separated the inmates, placed them in restraints, and escorted them from the unit. Both inmates were medically assessed and placed in SHU without further incident. Due to the a staff member being struck in the face, he was medically assessed and sent to an outside hospital for further treatment.

At 8:19a.m., staff responded to a cell fight involving 2 inmates. Responding staff were able to restrain and separate the inmates. One of the inmates involved was taken via ambulance for treatment at the local hospital and returned to the institution.

At approximately 1:50 p.m., the Camp officer observed an inmate acting suspiciously. The inmate appeared to have something he was trying to pass off to another inmate. He approached the inmate to further investigate the item in his hand, but the inmate refused to show him the item. The officer requested the item again and the inmate continued to refuse. The inmate then pushed the officer back and quickly tried to leave the area. As he attempted to stop the inmate, he was pushed again by the inmate two more times before being able to leave the area with the unidentified contraband. The officer continued to order the inmate to stop which met with negative results. The inmate was able to dispose of the contraband before complying with the officer's order. A medical assessment was conducted by the on duty Registered Nurse resulting with no staff or inmate injuries.

At approximately 12:05 p.m., staff were attempting to pat search an inmate. The inmate ran back into his cell, staff ordered him to stop, he did not comply. Staff ordered the inmate out the cell, he grabbed one of the Officers and shoved the Officer's head into the wall and then flushed a cellular phone down the toilet. An immediate use of force was conducted to gain control of the inmate. The inmate was placed in restraints, escorted to the Lieutenant's Office for a medical assessment, photographed, and placed in SHU without further incident. Staff were medically assessed in the institution.

At 5:00p.m., a fight was announced in a housing unit with three inmates on one in progress with weapons involved. Responding staff were able to separate all inmates involved without further incident. Three inmates were placed in SHU and one inmate was taken to an outside hospital for treatment. There were no staff injuries reported.

At approximately 6:55 a.m. an Officer was conducting the morning feeding when an inmate became upset because she did not receive cereal with her morning meal (cereal was not on the menu for the day). At this time, she placed her arm in the food slot refusing to move it. The Unit Officer continued feeding, upon picking up the trays from the inmate's cell next door to the inmate’s cell, the inmate threw what appeared to be urine out the food slot onto the Unit Officer hitting the Officer on the left side and pant leg. A medical assessment was conducted on the Unit Officer with no injuries noted and the inmate refused to be seen by the PA on duty.

At 9:10a.m., while escorting an inmate in from outside recreation, the inmate kicked another inmate and then kicked the staff member escorting the other inmate in the shoulder. Responding staff were able to gain control of the assaultive inmate and applied leg irons to complete the escort.

At approximately 12:16 p.m., SHU staff were in the process of escorting two inmates to holding cells when both inmates became combative by pulling away from escorting staff. Both inmates were placed on the ground to gain control and leg restraints were applied. As one inmate was being escorted off the range he attempted to head butt escorting staff. The inmate was again placed on the ground to gain control, and then placed in an ambulatory chair and moved to a holding cell. After being placed in the holding cells both inmates continued to threaten staff and display imminent signs of violence. Both inmates were then removed from the holding cell, pat searched, metal detected, medically assessed and subsequently placed in ambulatory restraints until such time as they regain control of their emotions and their displays of imminent signs of violence subside.

At approximately 10:55 a.m., an inmate became agitated in Health Services and struck a staff member in the left shoulder with an open hand. The inmate was escorted out of Health Services without incident and placed in SHU. The staff member was medically assessed and found to have no injuries.

At about 9:40a.m., a team was assembled to remove an uncooperative inmate from a cell and escort him to Health Services for a medical evaluation. During the escort, the inmate threw himself from the wheelchair in an effort to assault staff. Staff regained control and continued the exam. The inmate twisted his body while staff attempted to apply the blood pressure cuff. Again staff regained control. While attempting to draw blood from the inmate, he tried to grab the needle to have a weapon. Again staff had to physically control the inmate.

As staff attempted to remove the hand restraints, the inmate assaulted staff by pulling away, taking the staff member's left hand and striking it onto the food slot, causing the cuff key to brake into two pieces. Staff recovered the hand restraints and broken cuff key. Both staff and inmate were medically assessed and treated for their injuries.

Staff and inmate was medically assessed with no injuries noted. Subsequently, the inmate was placed in SHU without further incident.

At approximately 8:29a.m., an officer activated his body alarm after an inmate became verbally aggressive, and moved aggressively towards the officer. The officer secured the inmate inside his assigned cell. Upon arrival of additional staff, the A-Upper Unit officer opened the cell door without supervisor authority. Staff entered the cell and ordered the inmate to submit to hand restraints. The inmate refused to submit to hand restraints. He took a fighting stance and struck the officer with a closed fist, to right side of his face. Subsequently, staff used immediate force to secure the inmate on the lower bunk and placed hand restraints on him. The inmate was medically assessed, photographed, and placed in SHU. An incident report was written. The Officer was medically assessed. He sustained a contusion to his right cheek and right middle and index finger. The officer was advised to see his primary doctor.

At approximately 5:34 p.m., an officer notified the Activities Lieutenant that an inmate was ready to move from the shower. The inmate was placed in the shower in order to have a light replaced in a cell , the cell in which she is assigned. The inmate complied with having restraints placed on her while she was in the shower. While escorting the inmate to a cell, she became agitated and began kicking and scratching escorting staff. After returning to her assigned cell, the inmate refused to be medically assessed and she refused to allow staff to remove the handcuffs. At 6:47 p.m., the inmate allowed staff to remove the handcuffs without further incident.

At approximately 9:00 a.m., an inmate assaulted staff in SHU by spitting on them. The inmate was in a recreation cage and called the officer over to discuss an issue. As the officer was talking to the inmate, he spit on the officer twice. The spit landed on his right hand, trousers and jacket. The inmate was returned to his cell, without further incident. The officer was medically assessed and photographs taken.

At approximately 6:00 p.m., an inmate came through the serving line during the evening meal and got a tray. The inmate proceeded to the dining area, then returned to the serving line and told the Cook Supervisor he wanted a different tray. The inmate was told the trays are all the same, and he would not be given a different tray. The inmate than picked up a bag of potato chips from the tray and threw them at the Food Service staff striking him on the chin and chest. Both the staff member and inmate were medically assessed and the inmate was escorted to SHU without further incident.

Both staff and inmate were medically assessed with no injuries noted. The inmate was placed in SHU without any further incident.

At approximately 8:44 p.m., the Unit Officer activated his body alarm after observing several inmates fighting each other with various weapons within the unit. A partial recall of staff was initiated to assist in the emergency situation. Responding staff were able to separate several inmates actively fighting and containing them to opposite sides of the unit. All inmates were eventually secured in a cell, and upper body searches were conducted of the entire unit. Several inmate’s were transported to a local hospital for additional medical treatment. The other inmate’s were treated for minor injuries and placed in Administrative Detention. Four staff members reported minor injuries during the incident. The institution was placed on lockdown subsequent to this incident.

At approximately 8:05 a.m., an inmate approached the Cook Supervisor in the Food Service Dining Hall by the Cold Bar Cart and asked him for cereal. The Cook Supervisor explained to the inmate that it would be a moment until he could get him some. The inmate stated, "Fuck You." At that time, the Cook Supervisor ordered the inmate to give him his ID card, however, the inmate stated, "Fuck You" and walked away. The Cook Supervisor followed the inmate and again ordered him to give him his ID card. The inmate threw his ID card at him striking him in the facial area. The Cook Supervisor bent down and picked up the inmate ID card at that point the inmate threw his food tray striking him above the lip. The Activities Lieutenant was present in the dining hall and escorted the inmate out of the dining hall and applied hand restraints without incident. The Cook Supervisor was medically assessed and treated for minor cuts to his mouth and inner lip. The inmate was medically assessed and escorted to SHU. No injuries were noted. Disciplinary action is pending.

At 10:00 a.m., a nurse notified the Operations Lieutenant, that while giving inmates on the unit medication through the food slot, an inmate grabbed her hand and scratched it with her fingernails. The nurse was able to pull away her hand. She was medically assessed, an investigation continues.

The inmate pushed the food slot open hitting the officer on the left hand. The inmate became verbally abusive toward the officer requesting more food. The officer secured the food slot without further incident. The officer was treated for minor injuries to the left hand. The inmate received no injuries.

At approximately 12:20 p.m., an inmate refused to return his food trays after the feeding of the noon meal. The Activities Lieutenant ordered the inmate to return the food trays, at which time he refused and threw an unknown liquid substance through the crack of the door striking the Lieutenant in the upper chest area. The inmate subsequently submitted to hand restraints, was removed from the cell and moved to another cell without further incident. The Lieutenant and the inmate were medically assessed and found to have sustained no injuries.

At approximately 7:55 p.m., the Unit Officer observed an inmate out of bounds. The officer ordered him to return to his assigned unit. On the way out the inmate touched the hip of the officer with his hand. The officer considered this touch to be accidental and ordered the inmate to his assigned housing unit. Minutes later the inmate returned to the Unit and entered the sally port again. The Officer ordered him to leave again and the inmate assaulted the Unit Officer by grabbing her buttock. Responding staff escorted the inmate to the Lieutenant's Office, where he was visually searched and medically assessed. The inmate was placed in SHU. The officer was medically assessed and provided counseling by Psychology staff.

At approximately 8:45 a.m., an inmate became disruptive in an unit by holding the hand restraint slot open to the inside recreation area on a range. As staff were attempting to gain compliance, the inmate assaulted the staff member by throwing urine in a small plastic bag, striking the staff member in the upper body. The Warden was contacted and authorized confrontation avoidance and use of force team to be assembled. Oleoresin Capsicum was also authorized based on the inmate utilizing urine as a weapon and his extensive history of assault on staff. Confrontation avoidance was attempted and proved ineffective. The inmate was given a final order to submit to restraints, which he refused. Three-two second burst of oleoresin capsicum was deployed into the inside recreation area. Between each burst, fifteen seconds was given and the inmate was given an order to submit to restraints, which he refused. The use of force team entered the inside recreation area and secured the inmate. The inmate was escorted to SHU, where he was decontaminated, searched, medically assessed, and placed in soft ambulatory restraints without further incident.

At 4:15 p.m., an inmate was given a direct order to provide a breathalyzer test. At this time the inmate became agitated and disruptive by spitting in the Unit Officers face. Staff immediately placed the inmate on the ground to regain control. The inmate was restrained, medically assessed and taken to SHU without incident.

At approximately 9:40 a.m., an inmate became disruptive in SHU. Specifically, the inmate was placed in a holding cell from the compound for being intoxicated. The inmate began spitting at staff members and threatening to harm them. A use of force team was authorized by the Warden due to the inmate assaultive behavior. Confrontation avoidance was initiated at approximately 10:15 a.m. and was ineffective. The use of force team entered the cell, removed the inmate and placed him on the floor. He was placed in hand restraints and escorted to a larger holding area. He was visually searched, medically assessed and photographed without further incident.

At approximately 11:50 PM, Camp Staff observed an inmate running out of the wood line behind the Camp Housing Unit. Staff ordered the inmate to stop and the inmate continued to run. Upon Staff apprehending the inmate and while attempting to apply hand restraints, the inmate broke free and started running towards the institution road. Two Staff commenced a chase of the inmate, subdued him, placed him on the ground and applied hand restraints. The inmate was placed in SHU. The inmate was medically assessed by Health Services and found to have no injuries. No staff received any apparent injuries.

At approximately 8:20 a.m., while SHU staff was placing an inmate back into his assigned cell his cellmate assaulted staff by spitting on staff's face and neck area. The inmate then continued to verbally threaten the officer. Staff secured the cell. The Operations Lieutenant as well as SHU staff responded and ordered the inmate to submit to hand restraints, to which he complied. Both staff and inmate were medically assessed.

At approximately 4:35 p.m., a SHU staff member was placing a food tray in the food slot of a cell, when an inmate became irate and began cursing the staff member concerning his food tray. The inmate then spit through the food slot, striking the staff member in the groin and left thigh area. The inmate continued to curse at staff and threatened the staff member. The staff member was assessed by the health services department and did not report or sustain any visible injuries. The inmate refused to be assessed by medical staff.

At approximately 12:30 p.m., an inmate was being escorted from a cell in SHU, when he turned and spat in the face of the SHU Officer. Staff placed the inmate on the wall to gain control and escorted him to the holding cell. Staff and the inmate were medically assessed with no injuries noted. Subsequently, the inmate was returned to his assigned cell without further incident.

At approximately 3:08 p.m., an inmate assaulted staff by throwing sour milk and fecal matter on the staff member's face, chest, legs, back of the head and back area. The inmate also held his food slot, refusing to allow staff to secure it. A Calculated Use of Force Team was authorized and assembled. Confrontational avoidance proved effective, the inmate submitted to hand restraints. He was moved to an observation room, where he was searched and placed into hard ambulatory restraints. The assaulted staff and the inmate were both medically assessed.

At 10:37 a.m., the Operations Lieutenant was escorting an inmate to the holding cell in the Captain's Complex, when he became combative and struck the Operations Lieutenant in the left eye. The inmate was placed on the floor to gain control. Responding staff applied hand and leg restraints. The inmate was escorted to the Health Services where he was examined and treated for minor injuries. The inmate was later placed in Administrative Detention pending further investigation. The Operations Lieutenant was noted to have sustained redness and blurred vision to his left eye.

At approximately 11:50 a.m., an Unit Officer was conducting a pat search on an inmate, when he grabbed the Unit Officer's hand preventing him from finding what was concealed in his pockets. The BB Unit Officer placed the inmate on the wall. Responding staff arrived and assisted in searching the inmate. The inmate was placed in hand restraints and escorted to the Lieutenant's Office. Health Services staff examined the inmate noting no injuries. The Unit Officer was medically assessed with no injuries noted. The inmate was placed in Administrative Detention without further incident.

At approximately 11:25 am, the Operations Lieutenant was notified by Food Service Staff that an inmate was acting very erratically while mainline operations was being conducted. While escorting the inmate to the Lieutenant's Office, he became very belligerent and aggressive towards staff, which resulted in him being placed on the ground by staff to regain control. Responding staff arrived and applied hand restraints on the inmate and escorted him to Health Services for a medical assessment. The inmate was later placed in SHU without further incident.

At approximately 6:45 p.m., the SHU staff notified the Operations Lieutenant that an inmate become disruptive and assaulted staff. Specifically, the staff member approached the cell to place another inmate in the cell. Staff ordered the inmate to submit to restraints and accept an appropriate cell mate; to which he refused. The inmate threw an unknown liquid substance through the food slot, striking the staff member on the left arm. The Warden was contacted and authorized a calculated use of force team and the application of ambulatory restraints until a pattern of compliance has been reached and the desired calming effect has been met. The use of force team was assembled and confrontation avoidance proved to be ineffective. The team removed the inmate from the cell and escorted him to a shower due to him being covered in feces. The inmate was then escorted to holding a cell where he was placed in ambulatory restraints, and medically assessed without further incident.

At approximately 11:25 a.m., an unit officer activated his body alarm after an inmate pushed him with his arm when he ordered the inmate step out of his cell to conduct a search. The inmate became aggressive towards the staff member and proceeded to push him. The inmate was escorted to the Health Services Unit for medical assessment and staff was also provided with an medical assessment by the on duty Mid level Practitioner that reported no injuries to staff or inmate. Subsequently the inmate was place in Administrative Detention.

At approximately 7:40 p.m., the Operations Lieutenant received a telephone call from a SHU Officer, and informed him an Immediate Use of Force was conducted on an inmate. The officer stated after placing the inmate in a, he refused to let the officer remove the hand restraints. The cell was ordered opened and the inmate charged at one of the officers. The inmate was then restrained and placed on to the wall. At that time, he kicked the officer on his legs. The inmate was then placed on the floor, and the hand restraints were removed. The inmate was medically assessed by Health Service staff and placed in a secured cell without further incident. All responding staff were examined and did not receive any injuries.

At approximately 11:37 a.m., an inmate entered the Laundry Department and became verbally abusive towards staff. The inmate escalated the situation by striking the laundry foreman in the chest with his chest, and then attempted to strike the staff member with a closed fist. Staff placed him on the ground to gain control, and applied restraints. The inmate continued to display signs of imminent violence by spitting at and threatening responding staff. Leg restraints were applied and he was escorted to the SHU by utilizing the restraint chair. The inmate was medically assessed, and placed in a cell without further incident. The FBI was notified and an investigation continues.

At 7:13a.m., staff called for assistance outside the green corridor, an inmate could not clear the metal detector when staff went to pat search the inmate, the inmate took of running onto the compound. Responding staff were able to restrain the inmate and after the inmate was restrained a homemade shank was discovered on his body. The inmate was placed in SHU and there were no staff injuries reported.

**At 11:37a.m., staff called for assistance in the laundry room. An inmate assaulted a staff member. The inmate was cursing at the laundry foreman and then lunged his body toward the staff member chest bumping the staff and then started to wrestle with him. Responding staff were able to restrain the inmate and the inmate continued to curse at staff and threatened to get even with the staff member when released from SHU. While staff were holding the inmate he began to spit at staff. The inmate was taken down to the floor until full restraints were applied and escorted to SHU.

***At 2:51p.m., a call for assistance was announced in SHU for the same inmate involved in the earlier incident. While in SHU the inmate refused staff orders to give up a razor in his cell. The inmate then complied and cuffed up when taken out of his cell he started to buck on staff. Staff were able control the inmate and after shaking the cell down the inmate was placed back in without further incident. There were no staff injuries reported.

At approximately 3:10pm, an inmate refused numerous orders to leave the Lieutenant's Office, staff attempted to escort the inmate out of the office. The inmate suddenly lunged at the Activities Lieutenant, grabbed him with both hands and pushed him on to the Operations Lieutenant's desk. As a result, an immediate use of force was used on the inmate. When staff placed the inmate on the floor, he struck the Activities Lieutenant once on the left side of the jaw with a closed fist. As staff were placing hand restraints on the inmate he bit the Activities Lieutenant on the inner right forearm. Both the Activities Lieutenant and the inmate were medically assessed and photographed. The inmate was placed in SHU without further incident.

At 9:39a.m., while staff where pat searching inmates in the green corridor during the morning meal an inmate had a large amount of items in his possession. As staff were conducting the pat search the inmate became upset that they were taking everything and the inmate spun around and punched the staff member in the face. The inmate was taken down by staff and escorted to SHU. The staff member was medically assessed and no further treatment needed the officer is ok.

The inmate was medically assessed and placed in a cell without further incident. No staff injuries occurred.

Upon exiting the cell the inmate pulled away from staff and attempted to kick the escorting officer. Staff called for assistance and placed the inmate on the floor to regain control. The inmate was medically assessed and found to have sustained no injuries. There were no staff injuries were reported.

At 1:03p.m., a unit officer activated his body alarm due to a staff assault on him and another officer. The two officers were in the unit office testing intoxicants found in the inmates cell. The inmate came into the office and assaulted the two officers with closed fist. The officers gained control of the inmate and placed him in restraints. As additional staff arrived they locked down the unit and the inmate was escorted to Special Housing. At 1:18p.m., a recall was announced and all inmates were ordered to return to their housing assignments. At 1:20p.m., a staff needs assistance call was announced in the SHU Corridor, the inmate involved in the earlier assault became assaultive towards staff and was placed on the floor. A total of seven staff were injured during the assault, two with the initial assault and five responding to the two different incidents. USP Lee was placed on lock down status due to the assaults.

At approximately 9:40 p.m., Special Housing Unit staff were issuing an inmate bed linens. As staff handed the bed linens to the inmate he placed his arm in the cell door food slot, refusing to allow staff to close the food slot. The inmate refused staff's orders to remove his arm and assaulted staff by throwing fecal matter through the food slot opening, striking three staff members in the face, chest, and legs. As responding staff arrived, the inmate voluntarily submitted to hand restraints. The inmate was medically assessed and returned to his assigned cell.

At approximately 8:15 a.m., the Control Room Officer announced staff needing assistance on the recreation yard. An inmate became disruptive and refused to submit to a pat search by grabbing an officer’s hand, while being searched. The staff member attempted to subdue the inmate by placing him on the ground. The inmate struggled free from the staff member, punched the staff member in the head and ran to the recreation yard. Responding staff ordered the inmate to submit to restraints and he would not comply with staff orders. Staff placed the inmate on the ground. The inmate continued to struggle with staff, after being placed on the ground, where he continued kicking at staff and resisting staff orders to submit to restraints. Once he was placed in restraints the inmate was escorted to SHU where he was medically assessed.

At approximately 6:17 p.m., staff called for assistance due to an inmate becoming combative with staff. Specifically, while in the dining hall, the inmate was in possession of an unknown liquid substance and refused all orders from staff to relinquish the substance. The inmate began to dump the liquid out when staff utilized force and placed him on the ground. The inmate struck staff in the face prior to him being placed in restraints. The inmate was medically examined and placed in the Special Housing Unit without further incident.

At 12:32p.m., staff call for assistance in SHU. An inmate was refusing programs he did not want to be housed in a cell with another inmate then he started to buck on staff. Responding staff were able to gain control of the inmate and place him in the cell. There were no staff injuries.

**At 12:40p.m., staff call for assistance in SHU because another inmate was refusing programs he did not want to be housed in a cell with another inmate and started to buck on staff. Responding staff were able gain control and place the inmate in his cell. Both inmates had come off of the bus and wanted single cells. There were no staff injuries.

***At 1:00p.m., USP Canaan was placed on lock down status until further notice. Inmates came to mainline with signed petitions for the Warden demanding not to have the washers and dryers taken out of the housing units, through intelligence gathering the inmates through out the institution were getting ready to demonstrate by having a WORK STRIKE and FOOD STRIKE if there demands were not meet, by not having the washers and dryers taken out of the units. The Warden then decided to lock down and all washers and dryers were taken out of all housing units (the washers and dryers were originally set to be taken out of the units by the end of February for the water conservation act and all inmates received this information over a month ago through town hall meetings.) Mass interviews were completed the next day.

At 6:40 a.m., while escorting an inmate to SHU, he became assaultive and turned toward staff, grabbing the escorting officer's shirt and tie. Escorting staff placed the inmate against the wall to gain control. The inmate was then placed in SHU and into a holding cell. The inmate remained combative and spit on staff while in the holding cell. Additionally, while escorting the inmate to be medically assessed, the inmate again spit on staff. The inmate was medically assessed and placed in a cell without further incident.

At approximately 11:18 am, while a SHU officer was removing the hand restraints of an inmate the inmate pulled away while staff was still trying to remove the handcuff key inserted into the handcuff. This action from the inmate caused the officer to hurt her hand against the cell door food slot. The inmate was removed from the cell and escorted to the Health Services Unit for Medical assessment. The inmate was examined and reported to have no injuries. The staff member was also examined, and sustained minor laceration to the left hand and minor swelling of the elbow.

At approximately 1:35 p.m., two inmates were ordered to exit their cell while the DCU Officer conducted a cell search. One inmate became verbally abusive and aggressive, stating that he was going to kill the DCU Officer as soon as he got a chance. While the inmates were being allowed to return to their cell one inmate spit in the Officer's face and then struck him in the head and facial area with a closed fist. The other inmate attempted to assist his cell mate. A call for assistance was announced and responding staff entered the area. Both inmates were placed on the floor and hand restraints were applied. Subsequently, both inmates were medically examined and treated by Health Services Staff for minor injuries. There was one staff injury reported and an investigation continues.

At approximately 7:55 a.m.,an unit Officer was conducting random pat searches of inmates entering the unit. One inmate was pat searched and had several items in his pants pocket. The officer instructed him to remove the items, the inmate refused to take the items out of his pocket, as the officer attempted to remove the items, the inmate slapped the officer's hand away. The unit officer notified the compound officer to come and escort the inmate out of the unit. The inmate was photographed, medically assessed with no injuries noted, and placed in the SHU. In addition, the officer was photographed and medically assessed with no injuries noted.

At approximately 10:15 p.m., an unit Officer was conducting a routine pat search on an inmate. The officer felt what was believed to be a cellular phone in the left front pocket of the inmate. The inmate turned toward the officer and pushed him in the chest area. The inmate then ran from the officer. The officer radioed for assistance, and responding staff assisted with the search of the inmate. The inmate was found standing in front of the officer station. He was medically assessed, photographed and escorted to SHU for assaulting another person.

At 6:45 a.m., an inmate became aggressive and assaultive towards staff as the nurse was distributing medication in SMU. The inmate placed his arm in the food slot and refused to let staff close the food slot after he was given his medication. The inmate was given several orders to remove his hand but refused. As the officer moved past the cell and the inmate reached through the slot and grabbed the officer's right front pocket of his pants. The officer immediately swung down with his hand striking the arm of the inmate in order to break his grip. The unit officer was examined by the Staff Physician and treated for a small laceration to his right hand. The inmate was removed from his cell and examined by the Duty Nurse. The inmate was found to have no visible injuries and returned to his cell without further incident.

At 7:40p.m., a staff member was injured responding to a call for assistance in a housing unit. There was an inmate assault in progress, six inmates on one. Responding staff were able to separate all inmates, while doing so one officer was injured while wrestling with an inmate cutting his elbow open. All inmates were placed in SHU and the officer was sent to outside hospital for stitches.

At approximately 5:20 P.M., the SHU Officer reported to the P.M. SHU Lt. that she had been assaulted by an inmate. The Officer stated while picking up razors on a range, she ordered the inmate to return his razors. The inmate refused. The inmate was again ordered to return the razors. The inmate became agitated and threw the razors at the Officer. One razor struck the Officer in the upper part of her right leg. Medical staff was notified. The Officer was medically assessed and was found to have no injuries. The inmate was removed from his cell and medically assessed with no injuries found. The inmate was returned to his assigned cell without further incident.

At approximately 8:50 a.m., while conducting recreation moves in the Special Housing Unit, an inmate became disruptive while staff were removing the restraints and spun around and assaulted a staff member by striking him in the facial area through the cell bars. A Use of Force and Confrontation Avoidance team was approved and assembled. Confrontation proved successful and the inmate submitted to hand restraints. The inmate was removed from the cell, searched, medically assessed, and placed in soft ambulatory restraints without further incident.

An inmate was instructed to submit to a pat search and he walked to the officer and struck the officer in the chest with his right shoulder. The inmate was placed in hand restraints and escorted from the area. The inmate and staff member were medically assessed and the inmate was placed in SHU without incident.

At 4:00 p.m., a visual search was conducted on an inmate by Receiving & Discharge staff at the Federal Detention Center upon the inmate's return from court. During the search of the inmate, he became insolent and threw his shirt at the officer, striking him in the face and shoulder. The inmate was placed against the wall and placed in restraints without further incident. Both the staff member and the inmate were examined by the on Duty PA with no injuries noted. The inmate was placed in SHU.

At approximately 7:45 a.m., an inmate became disruptive by covering his cell door window and attempting to assault staff by throwing a clear liquid thru the food slot. The Warden was notified of the situation and authorized the assembling of Confrontational Avoidance and a Use Of Force team to extract the inmate and place him in ambulatory restraints. He submitted to hand restraints and was removed from his cell. He was then escorted to the holding area where he was metal detected, visually searched, and given clean boxers. The inmate became combative by spitting on and attempting to strike the members of the use of force team. He was immediately escorted to a cell and placed in soft four point restraints. The inmate continued his assaultive behavior by spitting on staff during the application of soft four point restraints. He was medically assessed with no injuries noted.

At approximately 1:50p.m., an officer reported that while conducting shakedowns, he noted several inmates by an inmate on the floor. The unit officer ordered the inmates to move away from the inmate, who got up off the floor and pushed the officer and spitted twice on the officer's face. The inmate was secured until staff escorted him to the Health Services Area for medical assessment. The inmate was placed in Administrative Detention without further incident. The officer was examined and did not sustained any injuries.

At approximately 5:33p.m., an inmate was ordered to submit to hand restraints and move to another cell. The inmate refused staff orders. The Warden authorized a Use of Force Team and confrontational avoidance procedures were then utilized and proved to be effective. The inmate was then escorted to a cell without incident.
 
**At approximately 6:25p.m., an immediate use of force was conducted on an inmate in SHU. The inmate was utilizing a razor blade to cut his lower forearm area. The Use of Force team was utilized to enter the cell and placed the inmate in hand restraints. The inmate was escorted to a multipurpose room, where he was searched and placed in ambulatory restraints without incident.

At approximately 5:05 p.m., while staff were attempting medically treatment on an inmate, he became disruptive by using profanity, and assaulting staff. Specifically, he hit staff with his closed left fist, striking the staff member on the left shoulder and left cheek. The inmate received a medical assessment and was placed in administrative detention without further incident.

At 3:19 p.m., the Warden authorized an inmate to be placed in ambulatory restraints after the inmate assaulted staff in SHU by throwing a liquid substance striking the staff in the face and upper torso. Confrontation Avoidance techniques proved effective and the inmate was placed in restraints, medically assessed and placed back into a cell without further incident.

At 7:29 p.m., staff observed two inmates fighting in the gymnasium and requested assistance. Responding staff ordered the inmates to stop fighting to which they did not initially comply. As Recreation staff attempted to separate the inmates, one inmate assaulted staff by shoving and striking one staff member in the chest area multiple times, and shoving another staff member once in the chest area. After several orders were given to stop fighting, both inmates complied. Both inmates were escorted from the area, placed in hand restraints and escorted to Health Services where they were medically assessed. Both staff members were medically assessed with no injuries noted. An initial investigation revealed the inmates were fighting over a basketball game. Both inmates were placed in the SHU pending further investigation.

At 11:59a.m., a call for assistance announced in the green corridor for inmates fighting. The fight started on the compound and continued into the green corridor. This was three inmates on one inmate. There were no staff injuries. Two inmates were taken to an outside hospital for treatment. One inmate had a broken eye socket and the other inmate sustained a broken hand. The third inmate was taken to SHU.
 
**At 12:05p.m., a body alarm was announced in a housing unit for an inmate fight with weapons involve. One inmate was hit with a lock in a sock and the other inmate was stabbed, both inmates were transported to outside hospital for treatment

At 4:12a.m., a housing unit officer announced over the radio that two inmates were fighting in a cell. The door was ordered open by the Operations Lieutenant for immediate use of force. The inmates were physically separated and cuffed up. Upon escorting one of the inmates to the door, he assaulted the housing unit officer by kicking him in the leg. The inmate was placed on the ground to gain control of the situation. The inmates were then escorted to SHU and medically assessed.

At approximately 12:20 p.m., staff were escorting an inmate from the recreation cage to the metal detector cage. As staff attempted to close the door, the inmate became disruptive by spitting in a staff member's face. The inmate was immediately taken to the floor to gain control of his aggressive behavior. When staff attempted to remove him from the cage, he continued to be disruptive by struggling to break away from staff. Staff again placed the inmate on the floor to gain control of the situation. The SMU Lieutenant talked with the inmate briefly explaining he was going to be placed into ambulatory restraints and into a cell after being medically assessed. The inmate cooperated with staff with no further incident other than being verbally disruptive. He was medically assessed with no injuries requiring medical treatment. The inmate Law was placed in a cell in ambulatory restraints.

***At 12:41p.m., staff responded to R&D for an inmate on staff assault. While receiving his property, the inmate became irate when told by staff he could not have a magazine. The inmate kicked at staff and had to be taken to the ground. The staff member received several minor injuries.

At approximately 12:30p.m., a Use-of-Force team member was assaulted by a SMU inmate. As the inmate managed to get over the shield to get the officer’s throat with a homemade shank. The officer sustained laceration, scratches to the neck, and some facial bruising.

Both inmates were issued a direct order to seize their action and both inmates refused to comply. Staff used immediate use of force to subdue both inmates. Both inmates were medically assessed with minor injuries and the staff involved in the immediate use of force were medically assessed. There were no injuries to report.

The inmate was placed in hand restraints and escorted to a holding cell.

As an inmate was exiting the housing unit during a institution move, the unit officer noticed he appeared to have been involved in a fight. The unit officer called for assistance where they found an unconscious inmate in the unit. A review of the video surveillance system revealed that two inmates were attacking one inmate during the move and when the inmate stopped moving the two attacking inmates stopped their actions. The unconscious inmate was transported to the outside hospital where he stayed for 4 days. There were no staff injuries reported.

At 9:10p.m., an unit officer noticed an inmate that had been severely beaten in front of the shower area of the unit. The Operations Lieutenant was notified, and staff came to assist the unit officer. Upper body checks were conducted, and one other inmate was found to have markings on his hands and blood on his shirt consistent with a fight. The inmate that was assaulted was escorted via ambulance to the outside hospital for head trauma, and the attacker was escorted to SHU. There were no staff were injured.

There were minor staff injuries reported.

The inmates refused all orders and continued to fight. The tower officer then fired one round from the tower and then the inmates complied with staffs orders to stop fighting. **There was a one on one inmate fight in the unit. Staff gave both inmates direct orders to stop fighting and the inmates refused to listen to staff orders. Responding staff had to use force to restrain the inmates. There were minor staff injuries reported.

At 2:58p.m., a body alarm was announced in a housing unit for an inmate fight in progress involving multiple inmates. Responding staff were able to separate all the inmates involved and get the housing unit locked down. Eight inmates were involved in the incident. While the inmates were being cuffed, the housing unit officer was assaulted by one of the inmates. When the inmate spit blood in the officers face. The officer was taken to an outside hospital for treatment. The 8 inmates were taken to medical and then taken to SHU. One inmate was taken to an outside hospital for treatment. The fight was gang related. The housing unit was locked down and remains locked down at this time. One staff member was taken to the outside hospital for treatment.

At approximately 12:27 pm, the 5-B unit officer called for assistance due to a fight in the unit. Staff observed two inmates striking each other in the upper torso. The inmates refused all staff orders to stop fighting, at which time staff utilized immediate use of force to place the inmates in hand restraints. Both inmates were restrained, removed from the unit, medically assessed, and placed in the SHU.

At approximately 12:35p.m., recreation staff requested assistance on the yard due to several hundred inmates facing off. The alarm was sounded and staff responded to quell the situation. The inmates were searched, videoed and released to their housing units. The institution was placed on lock down status. On January 10th, 2011, the inmates were released to the common areas of the housing units. Shortly thereafter a modified feeding schedule was implemented, with the inmates being returned to their units upon completion. The institution will operate in this fashion until a decision is made to return to normal operations.

At 8.55a.m., staff responded to a fight in the west outside recreation cages involving two inmates. Responding staff were able to restrain the inmates and escort them back to their cells

**At 4:45p.m., an inmate who had been escorted via ambulance to a local hospital for medical treatment head butted the facial area of a nurse at the local hospital as the nurse attempted to put a tube into the inmate.

At approximately 3:05 p.m., an inmate was placed in hand restraints in order to allow his cell mate back inside the cell. The inmate became agitated due to a property issue. He forcefully walked outside of the cell, and refused to step back inside the cell. Segregation staff attempted to escort the inmate back into the cell. The inmate began to resist staff and he was placed on the floor by segregation staff using minimum force necessary until the inmate gained control of his disruptive behavior.

At 8:04a.m., while staff was escorting an inmate from the shower back to his cell, the inmate became combative and attempted to pull away from the staff and then began spitting on them. Staff took the inmate to the ground to gain control. After that, staff escorted the inmate to the holding cell.

At approximately 10:28 am, the Unit officer called for assistance due to a fight in the unit. Two inmates were observed striking each other in the upper torso. One inmate refused staff's orders to submit to hand restraints, at which time staff utilized immediate use of force to place him on the ground and apply hand restraints. Both inmates were removed from the unit, medically assessed and placed in SHU.

At approximately 2:00p.m., there was a fight on the compound between a group of inmates. Approximately 5 minutes later there was a fight in the housing unit. Two inmates were taken to the hospital and there were no staff injuries reported. These were apparently two isolated incidents that involved the same group of inmates as the first fight.

**At 5:29p.m., staff responded to an inmate on inmate assault. As the one inmate was handing the food trays to staff, the other inmate came up behind him and began hitting the first inmate in the head. Responding staff were able to restrain and separate both inmates.

At approximately 3:00p.m., an officer was stabbed with a homemade spear in the groin area while escorting a medical staff member around to give out medication to the Special Management Unit inmates. The Officer opened the food slot and at the same time he was stabbed by an inmate. Then
the officer was taken to the local hospital and was released at approximately 7:00p.m., under the doctors care.

At approximately 5:55 pm, the Lieutenant was making rounds in a Housing Unit when he walked into the laundry room and attempted to conduct a pat search on an inmate. During the search, the inmate produced a weapon in his right hand and attempted to discard the weapon by throwing it into the common area of the unit. The Lieutenant then exited the laundry room to retrieve the weapon. While attempting to stop the inmate from exiting the laundry room, the inmate pushed the Lieutenant. An immediate use of force was conducted due to the inmate’s assaultive behavior. The inmate was placed on the ground and restraints were applied. The inmate was escorted to SHU.
