USP- MCCREARY- DECEMBER 30, 2010
At approximately 2:35 p.m., during the picking up of the noon meal food trays, an inmate assaulted
the SHU Officer. The food slot was opened to collect food trays, and the inmate threw two food trays onto the range. The first
missed the segregation officer, and the second food tray struck the officer on the left leg. The food slot was then secured
without further incident.
FCC- TERRE HAUTE- DECEMBER 30, 2010
An inmate assaulted another inmate in the SHU Recreation Cage. The inmate victim was sent to
a local hospital for treatment of injuries sustained in the assault.

At 12:16p.m., staff responded to a cell fight involving two inmates. Responding staff were able
to separate the inmates.

FDC- OAKDALE- DECEMBER 30, 2010
A fight occurred between gang members causing an officer to be injured. The officer was sent
to an outside hospital where he was listed to be in stable condition, but had a broken leg. The institution is placed on lock
down status until further notice.
FCI- BENNETTSVILLE- DECEMBER 30, 2010
At 12:20p.m., the institution had a disturbance on the recreation yard.. The institution was
placed on lock down status until further notice.
FCC- COLEMAN- USP 2- DECEMBER 29, 2010
At approximately 9:00p.m., staff were conducting upper body checks when an inmate became disruptive
toward staff. Staff instructed the inmate to place his arms on the wall and submit to a pat search. As staff attempted to
conduct a pat search the inmate turned and hit a staff member in the right hand and arm area. The inmate was placed on the
ground and placed in restraints. The inmate was medically examined and placed in SHU without further incident. The staff member
was medically examined and found to have sustained no injuries.
USP- LEWISBURG- DECEMBER 29, 2010
At 11:24a.m., staff responded to an inmate on inmate assault. An inmate assaulted his cell mate
after the restraints were removed from the first inmate. Staff restrained the assaultive inmate and pulled him out of the
cell. As staff were attempting to retrieve the second inmate, he turned aggressively toward the staff and was combative. Staff
had to take the inmate to the ground to gain control.
USP- LEWISBURG- DECEMBER 28, 2010
At 5:00p.m., a staff member was assaulted while picking up food trays in G Block. An inmate
threw fecal matter through the food slot striking the officer on his shoes.

At 9:31a.m., staff responded to a fight with a weapon(razor blade) in the eastside outside recreation
cages. The inmates were restrained and separated and the razor blade was found in the mouth of the one inmate.
**At
12:53p.m., staff responded to an inmate on inmate assault, which occurred as the Phase 3 inmates were out on the range. One
inmate walked up behind the other inmate in front of the officers station and began striking him in the head and torso. The
inmates were restrained and separated.

At 11:00a.m., as staff were feeding the noon meal on, an inmate attempted to ejaculate on a
female staff when she opened the food slot.

At 1:33p.m., staff responded to a cell fight involving two inmates. Staff were able to separate
the inmates.
**At 4:00p.m., staff responded to an inmate on inmate assault. Staff arrived and the one inmate was choking
the other inmate. Staff were able to restrain and separate the 2 inmates.
FCI- OTISVILLE- DECEMBER 21, 2010
At 10:00p.m., there was a fight in the dorm between two inmates. A calculated use
of force was used on the two inmates fighting. Inmates that were assigned to the dorm started to surround responding staff.
The Lieutenant called for an L8 with less lethal munitions. Approximately 50 inmates that were housed in the dorm became compliant.
USP- POLLOCK- DECEMBER 20, 2010
At 9:00a.m., a staff member was by an inmate in food service. When the staff member opened the
door for the inmate to exit, the inmate hit her in the eye with a freezer lock. The inmate also fought with responding staff.
The staff member suffered a laceration on the eye and was sent to the emergency room. No other staff members were injured.
The institution was placed on lock down status until further notice.

At 8:14a.m., staff responded to a three on one inmate assault in the eastside outside recreation
cages. Responding staff were able to restrain and separate the inmates.

At 6:15p.m., staff responded to a cell fight involving two inmates. Responding staff were able
to separate the inmates.
FCC-COLEMAN- USP 1- DECEMBER 18, 2010
At 8:00p.m., an inmate was beaten to death with a lock in a sock on the recreation yard. This
was a gang related incident. The inmate was trying to recruit other inmates to join his gang and other groups took it as an
insult and beat him to death. The institution is on lock down status until further notice.

At 7:24a.m., staff responded to a cell fight involving two inmates. Responding staff were able
to restrain and separate the inmates.
FCI- MCKEAN- DECEMBER 16, 2010
An officer was blatantly assaulted by an inmate. The officer suffers from a slight concussion,
ringing in his ears , a severe headache, vision impairment, and both eyes blackened.
USP- LEWISBURG- DECEMBER 16, 2010
At 2:18p.m., staff responded to a cell fight involving two inmates. The inmates were separated
by responding staff.
USP- CANAAN- DECEMBER 16, 2010
At 1:23p.m., staff called for assistance in SHU for an inmate on inmate cell fight. Responding
staff were able to separate both inmates without further incident. There were no staff injuries to report.
FCI- HERLONG- DECEMBER 15, 2010
At 5:16p.m., a compound officer was speaking with an inmate during mainline. Then inmate then
assaulted the officer causing a small abrasion to the chin. A call for assistance was announced and the inmate was placed
on the wall by the Compound Officer and the Operations Lieutenant until enough staff arrived. The inmate was escorted to SHU
without further incident.
USP- LEWISBURG- DECEMEBER 15, 2010
At 11:40a.m., staff reported an inmate on inmate assault in library. Staff placed the two inmates
in the library and after removing the restraints from the first inmate, he turned and started hitting and kicking the restrained
inmate. Staff were able to separate the two inmates without further incidents.
FCI- MCKEAN- DECEMBER 15, 2010
At 5:07p.m., an officer was assaulted by an inmate in his housing unit. The officer
was hit and then choked by the inmate. The officer was taken to the hospital. The inmate secured in SHU.
USP- CANAAN- DECEMBER 15, 2010
At 1:41p.m., staff called for assistance in SHU because an inmate had spit in a staff members
face. Responding staff restrained the inmate without further incident.
USP- LEWISBURG- DECEMBER 13, 2010
At 9:58a.m., staff responded to a three on one inmate assault in the west side outside recreation
cages. Responding staff were able to separate the inmates and return them to their cells. The victim was taken to the outside
hospital, treated and released back to the institution.
FCI- FLORENCE- DECEMBER 11, 2010
The institution is locked down. There was a fight on the recreation yard with numerous inmates.
One inmate had about five stab wounds. The inmates were playing flag football at the time of the incident.
USP- LEWISBURG- DECEMBER 11, 2010
At 5:23p.m., staff responded to a cell fight. When staff arrived, the one inmate was laying
on the floor. Staff removed both inmates. One inmate was taken to the outside hospital for treatment as half if his ear was
missing and the other was severely lacerated.
USP- CANAAN- DECEMBER 10, 2010
At 6:01p.m., a fight was announced in food service. Two inmates jumped on one inmate. The one
inmate was be beaten with a broom handle when the handle broke the inmate was then stabbed with the jagged end of the stick.
Staff were able to secure all the inmates involved. One inmate was taken to outside hospital for treatment. There were no
staff injuries to report and all of the were placed in SHU.
FCC- COLEMAN- DECEMBER 10, 2010
At 6:00p.m., while attempting to escort an inmate his cell, the inmate spit on both his escorting
officers. Spit struck officer number one on the left thigh and officer number two was struck in the chest and facial area.
Both staff members and the inmate where seen by health services.
USP- LEWISBURG- DECEMBER 10, 2010
At 6:00p.m., staff responded to an inmate on inmate assault. Staff placed two restrained inmates
back in their cell. After staff removed the restraints from the first inmate, he turned and began hitting and kicking his
still restrained cell mate. Staff were able to restrain the inmate and separate the two without further incidents.

At 2:33p.m., staff responded to a cell fight involving two inmates. Responding staff were able
to separate the two inmates without further incidents.
USP- LEWISBURG- DECEMBER 7, 2010
At 10:00a.m., while performing their two hour checks on a restrained inmate, two
staff were assaulted when the inmate began spitting on staff and the two staff members were struck.
At 11:50a.m., staff
responded to an assault in the east side recreation cages. Responding staff observed an inmate hitting another inmate in the
head and torso. Responding staff separated the two inmates and took them back inside.
USP- CANAAN- DECEMBER 6, 2010
At 9:04a.m., a call for assistance in SHU was
made because an inmate who was locked up refused to cell with another inmate and became insolent and started to buck staff
escorting him to his cell. Staff were able to control the inmate and place him in his cell.
At 2:30p.m., a use of force team was used in SHU on an inmate that broke a sprinkler head in his cell and
refused to cuff up. This was the same inmate that earlier refused to be celled with another inmate. The team was able to extract
the inmate without further incident.

An officer had milk thrown in his face in SHU.

At 2:06p.m., staff responded to a cell fight involving two inmates. The inmates were separated
by responding staff.
FCI- HERLONG- DECEMBER 5, 2010
At 2:04p.m., a unit officer called a fight in the housing unit. Four inmates entered a cell
to attack one inmate for refusing to check in to SHU. One inmate (not the victim) was life flighted to the local hospital
for stab wounds and head trauma. Another inmate (the victim) was taken via ambulance to the local hospital and was treated
and released. One inmate remains at the hospital.
FCC- COLEMAN- USP 2- DECEMBER 4, 2010
At approximately 4:54p.m., an officer called for assistance after observing two inmates fighting
in the common area with weapons. The officer also observed another inmate throwing a chair at inmate on of the inmates. As
responding staff arrived, the inmates threw their homemade weapons on the floor and complied with staff orders to lay on the
floor. The weapons used in this incident were recovered. All three inmates were placed in hand restraints, medically assessed,
and one inmate was placed in the SHU without further incident. Two inmates were sent to the local hospital for further treatment.
The FBI was notified.

Staff responded to a cell fight involving two inmates. Responding staff were able
to separate the two inmates without further incidents.

At 8:23a.m., staff responded to an inmate on inmate assault. Staff placed the inmates back in
their cell. Upon removing the restraints from the first inmate, he turned and began hitting the restrained inmate. Staff were
able to separate the inmates.
USP- CANAAN- DECEMBER 3, 2010
At 7:34a.m., staff called for assistance in the
green corridor, A staff member was assaulted, while the staff member was trying to take an item off of an inmate the inmate
refused the staffs orders and pushed the staff member in his chest with his forearm. The inmate was taken down by responding
staff and placed in SHU.
At 5:37p.m., staff called for assistance in the green corridor during the evening meal. An inmate was assaulting
another inmate. Staff were able to separate and restrain both of the inmates and escort them to SHU without further incidents.
USP- MCCREARY- DECEMBER 2, 2010
At 1:10p.m., a fight was called in a housing unit with weapons. Two inmates assaulted another
inmate. The inmate was taken by ambulance to a outside hospital. The inmate was stabbed 47 times, lacerating his spleen, collapsing
a lung, and one stab wound went all the way through his hand. The Institution was not placed on lock-down for this incident.
USP- LEWISBURG- DECEMBER 2, 2010
At 9:53a.m., an officer called for assistance to the East Side Recreation Cages when he observed
an inmate pull a homemade weapon and attempted to stab the other inmate. The other inmate began punching the first inmate
in the head and torso. Responding staff were able to restrain the two inmates, recover the weapon and returned the inmates
to their cells.
**At 12:07p.m., staff responded to a cell fight involving two inmates. Staff were able to separate
the two and place them in separate cells.
***At 5:46p.m., staff responded to a cell fight involving two inmates. Staff
were able to separate the two and place them in separate cells.
USP- CANAAN- NOVEMBER 30, 2010
At 6:30a.m., a housing unit officer found 11 homemade shanks and a large amount
of homemade intoxicants in the housing unit water fountain. The unit was locked down per the captain and the unit was then
shaken down and mass interviews were completed by day watch staff.
**At 2:00p.m., a staff call for assistance was announced in a housing unit for an inmate fight. One inmate
was beaten by four other inmates. The unit was locked down and the inmate received medical attention in the institution’s
hospital. All of the inmates were placed in SHU.

At 7:33a.m., staff responded to an inmate on inmate fight. Responding staff were able to separate
the two inmates without further incidents.
At 10:48a.m., staff responded to B Block for an inmate on inmate assault.
After Staff removed the restraints from the first inmate, he turned and began hitting the restrained inmate. Responding staff
were able to separate the two inmates.
FCC- COLEMAN USP 2- NOVEMBER 30, 2010
USP-2 is on Lock down status due to groups of inmates lining up on the recreation yard. Staff
members recovered various shanks.
USP- LEWISBURG- NOVEMBER 29, 2010
At 8:35a.m., a staff member was assaulted by an inmate. While escorting the inmate back to his
cell, the staff member secured the inmate in his cell. The staff member began uncuffing the inmate and the inmate pulled back
slamming the staff member into the door and the staff members cuff key was pulled into the cell. Responding staff arrived
and the inmate was restrained and the cuff key recovered.
**At 9:58a.m., staff responded to a two on one inmate assault.
Staff were able to separate the three inmates and take them back to their cells.
***At 12:35p.m., staff responded to
a cell fight involving two inmates. Responding staff cuffed the first inmate and the second inmate (unrestrained) began hitting
the restrained inmate.

At 7:57a.m., while doing cell rotations, staff placed two inmates in a cell. After removing
the restraints from the first inmate, he then began punching and kicking the restrained inmate.
At 8:52a.m., staff
responded to a cell fight involving two inmates. Staff were able to restrain the inmates and place them in different cells.

At 1:29p.m., a fight was announced over the radio in a housing unit for an one
on one inmate fight. There were no staff injuries and both inmates were placed in SHU.
**At 1:55p.m., a fight was announced over the radio for an inmate on inmate fight. Both inmates were separated
and placed in SHU.
***At 4:32p.m., a body alarm was hit in SHU for an inmate on inmate cell fight. The inmates were separated
by responding staff.
****At 10:14p.m., a staff needs assistance call was made in SHU for an inmate on inmate cell fight. The inmates
were separated by responding staff.

At 11:48a.m., staff responded to an inmate on inmate assault. Staff placed both inmates in
the cell. After having his hand restraints removed, the inmate began hitting and kicking the restrained inmate.

At 5:32p.m., staff needs assistance call was made in SHU for an one on one inmate cell fight.
Use of force team had to be used to control the inmates. There were no staff injuries reported.
USP- LEWISBURG- NOVEMBER 24, 2010
At 8:11a.m., staff responded to the outside recreation cages for a two on one inmate assault.
Staff witnessed the two inmates kicking and hitting the one inmate, who was laying on the ground. Staff were able to separate
the inmates and the victim was taken to an outside hospital for treatment.
**At 11:05a.m., a staff member was assaulted
while collecting food trays. The officer reached down to pick up the trays and the inmate struck the officer with a closed
fist through the food slot. The inmate then refused to allow his food slot door to be closed.

At 1:00p.m., an assistance call for a fight in a housing unit was announced. It was two on one
inmate fight. All of the inmates were medically accessed and placed in SHU.
USP- CANAAN- NOVEMBER 23, 2010
At 4:15p.m., an assistance call was made for a cell fight in SHU. Both inmates were separated
and there were no staff injuries to report.
USP- CANAAN- NOVEMBER 22, 2010
At 5:40p.m., a staff assistance was called over the radio in the Lieutenants office. An inmate
was taken to the Lieutenant's office for insolence. While the Lt was talking to the inmate, the inmate spit in the face and
then head butted him. The inmate was restrained and place in SHU.

At 7:53a.m., staff responded to a fight involving two inmates in the outside east recreation
cages. Responding staff were able to restrain the inmates and escort them to their separate cells.
**At 1:39p.m., staff
responded to an inmate on inmate assault with a weapon. The one inmate was slashing the other inmate in the cell with a razor
blade. Responding staff were able to restrain and separate the two inmates without further incidents. Both inmates were medically
assessed and placed in separate cells.

At approximately 5:15 p.m., an inmate refused to submit to a visual search, resulting in an immediate use of force.
There were no staff injuries. The inmate was medically accessed and escorted to SHU.

At approximately 8:10 a.m., several inmates assaulted an inmate on the recreation yard. A yard recall was initiated,
and all inmates were returned to their housing units. There were no staff injuries to report.

At 2:53p.m., staff responded to a cell fight involving two inmates. Staff were able to separate
the inmates.
**At 4:31p.m., staff responded to an assault on a staff member. While collecting the food trays, an inmate
threw an unknown liquid on the legs and torso of the officer.
***At 7:05p.m., seven inmates from the Lewisburg Camp
were locked up for engaging in a group demonstration.
USP- CANAAN- NOVEMBER 19, 2010
At 11:36p.m., a call for assistance was announced in SHU for an one on one inmate cell fight.
The inmates were separated by responding staff.
USP- LEWISBURG- NOVEMBER 18, 2010
Staff responded to an attempted assault on an officer with a weapon. While performing a shakedown
on an inmate prior to going to recreation, the officer discovered what turned out to be a 9" metal sharpened weapon. The officer
took the inmate to the shower to do a strip search. The officer secured the inmate in a locked shower and removed the hand
restraints. Upon removal of the restraints, the inmate pulled the weapon from the middle of his back and attempted to stab
the officer. There were no staff injured and the weapon was recovered.

At 1:31p.m., a call for assistance was announced in SHU for an inmate fight in
the outside recreation cage. Staff separated both inmates and there were no staff injuries.
FCI- MCKEAN- NOVEMBER 15, 2010
Around 3:00p.m., over 200 hundred inmates had a stare down on the recreation yard and would
not disperse until recall was announced three times. The institution was placed on lock down status.
USP- TUCSON- NOVEMBER 15, 2010
One inmate strangled and killed his roommate in Special Housing.
USP- LEWISBURG- NOVEMBER 15, 2010
At 7:26a.m.,staff responded to a cell fight involving two inmates. Responding staff were able
to restrain and separate the inmates.
**At 11:12a.m., an inmate assaulted a staff member. While escorting the inmate
to the shower, the inmate slammed the shower door on the forearm of a staff member. Responding staff regained control of the
situation and the staff member was medically assessed.
***At 10:08p.m., staff responded to a cell fight involving two
inmates. Responding staff were able to restrain and separate the two inmates.
FCI- MCKEAN- NOVEMBER 14, 2010
Around 6:30p.m., in Unit AB several inmates got in a fight , staff responded and three officers
were assaulted. One officer suffered with a slight concussion, apparently he was punched or hit with a lock in the sock. The
second officer one was hit with a chair. The third officer injured his hand.
FCI- MCKEAN- NOVEMBER 14, 2010
While responding to an inmate on inmate gang related fight in a housing unit. Four officers
were assaulted and other inmates joined in after the fight started. The first officer was hit over the back with a chair while
securing an inmate on the floor. The second officer was head butted by an inmate he was trying to control and received a broken
nose and was sent to the outside hospital. The third officer was sucker punched in the head by an inmate. The fourth officer
received slight injuries to his hand and knee securing the inmates involved.
FCi- OXFORD- NOVEMBER 13, 2010
A fight occurred with approximately ten inmates involved. The institution was placed on lock
down while an investigation continues.
USP- LEWISBURG- NOVEMBER 13, 2010
At 8:00a.m., staff responded to an inmate on inmate assault with a weapon. The inmates were
placed in the Law Library. After restraints were removed from the first inmate, he turned and began stabbing the other inmate
with a homemade weapon. Staff were able to regain control of the situation.
FCI- OXFORD- NOVEMBER 12, 2010
After completion of the 4:00p.m. count an inmate was struck in the head by another with a lock
in a sock. As another inmate came to the assistance of the inmate being assaulted he was struck and knocked unconscious. Both
inmates were transported to an outside medical facility for examination of their injuries and returned to the institution
and placed in the Special Housing Unit.

At 12:55a.m., staff responded to a cell fight involving two inmates. Responding staff were able
to separate the two inmates.
**At 7:36a.m., staff responded to a fight in the East Side outside Recreation Cages involving
two inmates. Responding staff were able to separate the inmates and escort them back inside.
***At 8:32a.m., staff
called for assistance after being assaulted by an inmate. While escorting the inmate, the inmate spun away from the officer
and began to kick the officer. Responding staff were able to regain control of the inmate.

At approximately 6:30p.m., an inmate was assaulted
by several other inmates on the recreation yard. A yard recall was conducted. All inmates involved were medically assessed
and escorted to SHU. There were no staff injuries to report.

At 9:05a.m., staff responded to a three on one inmate assault in the outside west recreation
cages. Staff were able to restrain the inmates and escort them back to their cells.

At 3:00p.m., a fight was announced in a housing unit between two inmates. Both inmates were

At 11:01a.m., staff responded to a fight in a west side recreation cage involving two inmates.
Responding staff were able to separate the inmates and escort them back to their cells without further incidents.

At 7:08a.m., a fight was announced in a inmate housing unit common area for an inmate on inmate
fight. Both inmates were separated by staff and placed in a full Special Housing Unit. There were no staff injuries to report.
FDC- PHILADELPHIA- NOVEMBER 9, 2010
An inmate kicked two staff members.
USP- TUCSON- NOVEMBER 8, 2010
A fight broke out in one of the units, three inmates were sent to the hospital and the institution
was placed on lock down status until 11-11-10.
FCI- FLORENCE- NOVEMBER 8, 2010
At 9:00 a.m., the Special Housing Unit Staff were escorting inmates to outdoor recreation. An
inmate was placed into a recreation cage and hand restraints were removed. Staff placed another wheelchair confined inmate
in the recreation cage next to the inmate, and began removing the inmates hand restraints. The first inmate reached through
the restraint slot and grabbed the staff member’s buttocks. The inmate was escorted back to his assigned cell with no
further incident. The staff member and the inmate involved were medically assessed with no injuries noted.
FMC- ROCHESTER- NOVEMBER 8, 2010
At approximately 2:30p.m., an inmate assaulted a staff member by striking them on the chin with
a closed fist. The inmate was ordered to return to his cell and he complied. A medical professional assessed the staff member
and the inmate with no noted injuries. The F.B.I. was notified.
FCI- WILLIAMSBURG- NOVEMBER 8, 2010
At approximately 12:40p.m., a large number of inmates engaged in an altercation in a Housing
Unit. The unit officer and security officer ordered the inmates to cease their actions and sounded the alarm. The institution
was placed on lock down status and numerous inmates were secured in the SHU. The inmates were served their meals in their
cells in the units. On November 9, 2010, the institution remained locked down and additional inmates were secured in the Special
Housing Unit. The SHU is operating at maximum capacity without additional staffing. The inmates were later released from their
cells and eventually the institution returned to normal operations.
USP- MCCREARY- NOVEMBER 8, 2010
At 5:30p.m., two staff members were in the process of searching an inmate’s cell. The
inmate pulled out a weapon and stabbed both officers. There officers were airlifted to a hospital and remain in stable conditions.
The institution was placed on lock down status until further notice.
USP- CANAAN- NOVEMBER 8, 2010
At 6:30p.m., a fight was announced on the compound because two inmates assaulted one inmate.
The inmates were separated by staff and placed in a full Special Housing Unit. There were no staff injuries to report.
FCI- PEKIN- NOVEMBER 7, 2010
At approximately 7:15 a.m., as the Operations Lieutenant issued an inmate an alternative sack
meal, the inmate threw the sack meal down the range. The inmate then stuck his arm out preventing staff from securing the
food slot. As the Operations Lieutenant was ordering the inmate to remove his arm from the food slot, the inmate spat striking
the Operations Lieutenant on the left side of the head. The inmate then threw a cup filled with an unknown liquid substance
striking the Operations Lieutenant on the left side of the face, neck and arm, in addition to striking the Special Housing
Unit #2 Officer on the side of the face and neck. The inmate ceased his actions and withdrew his arm into the cell at which
time the food slot was secured. Medical assessments were completed on the inmate and both staff involved. The inmate remains
in the Special Housing Unit pending further investigation.
FCI- WASECA- NOVEMBER 6, 2010
At approximately 1:05 p.m., the D-Unit Officer was monitoring inmate movement when an inmate
entered the unit. The Unit Officer ordered the inmate to leave the unit at which time the inmate began to assault another
inmate by striking her repeatedly in the face with a closed fist. At this time, the D-Unit Officer activated her body alarm
and ordered the inmate to cease her actions. The inmate refused to comply with the officer's orders and continued to strike
the other inmate. As the Unit officer attempted to separate the inmates, the assaulting inmate struck the officer in the forearm
with a closed fist. At this time, staff arrived at D-Unit and the inmate ceased her actions and submitted to hand restraints.
Both inmates were separated, placed in the Special Housing Unit, and medically assessed without further incident.
MCC- NEW YORK- NOVEMBER 6, 2010
At approximately 5:25 pm, unit staff were feeding the evening meal on Unit 2. As staff approached
a cell and handed an inmate her food tray. She threw the tray striking staff in the upper and lower torso area. Staff exited
the cell and secured the door without further incident.
FCI- BENNETTSVILLE- NOVEMBER 6, 2010
At approximately 6:42p.m., a staff called for assistance in a unit. The staff member was assaulted
by an inmate when the staff member observed the inmate attempting to pouring out intoxicates. Staff arrived and restrained
the inmate, and escorted him to SHU. The staff member was taken to medical and screened, where minor injuries were noted.
FCI- OTISVILLE- NOVEMBER 5, 2010
There was an internal disturbance for gang power between gang members of the same group. The
institution was placed on lock down status and returned to normal operations on November 8, 2010. There were no staff injuries
to report.
FDC- PHILADELPHIA- NOVEMBER 5, 2010
At 6:00a.m., an inmate assaulted a staff member with a pen. The inmate stabbed the officer in
the ear.

At 09:30a.m., as an Officer was placing an inmate in the SHU
Recreation cage, the inmate pushed the slider towards the officer, striking him. The inmate then spit on the Officer twice,
striking his face and arm.

At 11:10a.m., a fight was announced in a inmate housing unit for an inmate on inmate
fight. Both inmates were separated by staff and placed in a full Special Housing Unit. There were no staff injuries to report.

were separated by staff and placed in a full Special Housing Unit. There were no staff injuries to report.
FMC- LEXINGTON- NOVEMBER 4, 2010
At approximately 5:58 p.m., an inmate assaulted a Housing Unit Officer. Specifically, the Officer
responded to loud yelling in the lobby area of the Unit. The inmate was yelling at other inmates in the area. The Unit Officer
tried to calm the inmate. The inmate grabbed the Officer by the right arm and pulled the Officer towards him. The Officer
pulled away from the inmate and called for assistance.
FCI- HERLONG- NOVEMBER 4, 2010
At 12:10p.m., an officer called for assistance. Two inmates were fighting in the unit. The inmates
were separated, medically assessed, and escorted to SHU without further incident. Two staff members were injured while responding,
due to slipping on a freshly mopped floor.

At 5:58p.m., an officer called control for assistance via radio transmission. After further
investigation an inmate entered the officers station and grabbed the officer’s arm. The officer and staff responding
were not injured.

At 7:44a.m., staff assistance was announced in SHU for an inmate on inmate fight
in a recreation cage. There were no staff injuries to report.
*At 8:36a.m., a call for assistance was announced in SHU because an inmate became disruptive with staff.
**At 3:19p.m., a call for assistance was announced in SHU because an inmate became disruptive and aggressive
towards staff. A chemical agent was used on the inmate to calm him down and to gain control.
MDC- BROOKLYN- NOVEMBER 3, 2010
At approximately 9:15 p.m.,an inmate scraped a staff member's right hand after pulling the handcuffs
through the slot of the law library. The staff member was medically assessed and photographed with minor injuries. The inmate
was also medically assessed, photographed and placed back into his cell in the Special Housing Unit without further incident.

At 1:00p.m., a call for assistance was announced in the green corridor for an inmate on inmate
fight. Both inmates were separated by staff and placed in a full Special Housing Unit. There were no staff injuries to report.

At 8:48a.m., staff responded to an inmate on inmate assault. After both inmates were cuffed,
the one inmate started kicking and head butting the other inmate. The inmates were separated by staff.

At 11:00a.m., A-unit called for compound to step to A-unit to pick up two inmates for a possible
assault. At 11:32a.m., a fight was called over the radio by Compound Officers, involving multiple inmates with weapons. As
staff were controlling these combatants, a staff needs assistance was called over the radio at 11:35 AM for another fight.
Control was ordered to recall staff to the institution due to the amount of inmates involved. A staff needs assistance was
called at 11:53a.m., due to an attempted assault on staff while attempting to secure his unit. Inmate combatants were medically
assessed, and secured in the Special Housing Unit and R&D. Twenty –One inmates were placed in Special Housing for
fighting. Four staff members received injuries and one staff member was transported to an outside hospital for injuries received.

At 8:34a.m., staff responded to a cell fight involving two inmates. Staff were able to separate
the two inmates without further incidents.
FCI- ESTILL- OCTOBER 30, 2010
At 2:03p.m., staff called for assistance on the recreation yard. Approximately 16 gang related
inmates were fighting with weapons. All 16 inmates were place in SHU and the institution was placed on lock down status. Mass
interviews were conducted and the institution returned to normal schedule on November 2, 2010.
USP- LEE- OCTOBER 29, 2010
At 7:55 p.m., the Recreation Supervisor was assaulted by a inmate throwing a racquet ball at
his back. The inmate was escorted to the Lieutenant’s office without incident and was given a BA test with the first
reading of .176 and a second reading of .222.
USP- LEWISBURG- OCTOBER 29, 2010
At 1:14p.m., staff responded to an assault on staff. While attempting to uncuff an inmate in
his cell, the inmate pulled a staff member through the food slot ripping his keys and chain off the officers belt. Staff were
able to restrain the inmates and the keys were recovered.
USP- LEE- OCTOBER 28, 2010
At 10:45 a.m., staff in the Special Housing Unit called for assistance reporting
an inmate fight. Staff observed two inmates striking each other with closed fists to the head and upper body. Both inmates
were medically assessed with minor injuries. One staff member received injuries requiring him to be treated at outside
hospital.

At 2:09p.m., staff responded to a cell fight involving two inmates. Staff were able to separate
the two inmates without further incidents.

While one inmate was at the door talking to an officer, the other inmate began slashing him with a razor blade. The inmates
were separated and the victim was treated at the institution.

announced in a housing unit for an inmate fight. Two inmates assaulted one inmate and the one inmate was taken to outside
local hospital for the injuries he sustained from the assault. There were no staff injuries to report.

At 7:53a.m., staff responded to an assistance call for a two on one inmate assault. The inmates
were using closed fists and also were kicking the inmate victim. Inmates were separated and treated at the institution.

when the officer secured him in the cell, the inmate slipped his cuffs.

An inmate in the SHU threw a bar of soap, striking
an Officer in the stomach.
USP- LEWISBURG- OCTOBER 21, 2010
At 8:13a.m., staff responded to a two inmate on one inmate assault in the west outside recreation
cages. Responding staff ordered the inmates to lay on the ground. When they told the first inmate to come to the sallyport,
the inmate stood up and stomped the victim in the head. The inmate got hit with a 3 round burst from the pepperball gun and
laid down. Staff ordered the 2nd inmate to the sallyport and he proceeded to kick the victim twice in the head. After that
all inmates were restrained after gas and pepperballs and stinger grenades were used. A staff member was accidentally hit
in the side of the face with a pepperball round.
At 10:42a.m., staff responded to a fight in the east side outside
recreation cages for a fight involving two inmates. Staff were able to separate the two inmates and escort them back to their
cells.

At 6:38p.m., a yard recall due occurred with a large inmate groupings on the recreation yard
and inmates digging up weapons. All inmates were locked in their cells and an emergency count was conducted. The institution
remains on lock down status until further notice.
FCC- COLEMAN- USP 2- OCTOBER 20, 2010
The USP-2 is on lock down status due to a fight in the housing unit One inmate hit another inmate
with a lock in a sock. Two other inmates jumped in and beat up the one inmate. A staff member was injured during the fight.
He was taken to local hospital.

At 8:25a.m., staff responded to a cell fight between two inmates. Staff were able to separate
the two inmates without further incidents.

At approximately 12:24 p.m., the Special Housing Unit officer was walking past a holding cell.
At this time, the inmate spat and hit the officer in the right, rear, lower leg. The inmate was removed from the holding cell,
and placed in his assigned cell, without further incident. An investigation continues.

At 6:09a.m., staff responded to a cell fight involving two inmates. Responding staff were able
to separate the inmates.

At 3:04p.m., a cell fight was announced in Special Housing. After sufficient staff were present,
both inmates submitted to hand restraints and were removed from the cell. There were no staff injuries to report.
USP- HAZELTON- OCTOBER 18, 2010
At 01:30p.m., two SHU Officers were escorting an inmate from
SHU Recreation back to his cell.The inmate refused to return to his cell and became combative. Immediate use of force was
utilized to restrain the inmate and he was taken to the floor. The inmate kicked one Officer in the lower leg twice, and kicked
the other Officer in the ribs.

At 01:25p.m., an inmate in the SHU Recreation cage
spit in the face of an Officer.
**At 1:31p.m., as an inmate was being escorted
from the SHU Recreation area to his cell, he spit on an officer striking him
in the head. After this inmate was returned to his cell he attempted to throw feces on a third officer but missed, and instead
contaminated 35 food trays that were on a nearby cart.

At 6:56p.m., a fight was announced in a housing unit. It was an inmate on inmate fight. Both
inmates were separated without further incidents. There were no staff injuries to report.

At 11:45p.m., staff was called to a cell for a disruptive inmate. Staff discovered that the
inmate had his cell mate tied to the bed and was threatening to harm or kill him if he wasn't removed. Staff were able to
separate the inmates without any further incident.

At 6:38a.m., officers responded to a cell fight involving two inmates. Staff were able to separate
the two inmates without further incident.
At 10:12a.m., officers responded to an assault with a weapon. One inmate
stabbed another inmate with a plastic shank in the east side recreation cages. The inmate was taken to an outside hospital.

At 6:00p.m., a fight in front of building was announced by control. Responding staff had to
physically separate and restrain both inmates. There were no staff injuries to report.
FCI- WILLIAMSBURG- OCTOBER 14, 2010
At approximately 7:30p.m.,, about 30 inmates were involved in an altercation. Several inmates
were sent to SHU and the institution was placed on lock down. On October 15, 2010 the institution remained on lock down. Mass
interviews were conducted and the population ate breakfast in their cells. At lunch time, the inmates were fed in the dining
hall, one unit at a time. Visitation was cancelled for the day. On October 16, 2010, the inmates began to group again and
they were locked in their units, with feeding taking place, one unit at a time. Visitation is again cancelled.

At approximately 7:15 a.m., an inmate assaulted a staff member by throwing feces all over him.

and moved by staff.

FCI- BENNETTSVILLE- OCTOBER 13, 2010
At approximately 12:30 p.m., two staff members were injured in Special Housing while taking
a combated inmate down that was non compliant. One staff injuried his right hand while the other staff member’s knee
was injured.
USP- LEWISBURG- OCTOBER 13, 2010
At 8:05a.m., staff responded to an inmate on inmate assault. Staff uncuffed the first inmate
who then began hitting the cuffed inmate. The cuffed inmate fell to the floor and his assaulter began kicking him in the head.
Staff were able to separate the two inmates without further incidents.
**At 1:39p.m., staff responded to a cell fight
involving two inmates. Responding staff separated the two inmates.
FCI- OXFORD- OCTOBER 12, 2010
A female teacher was "shouldered" by an inmate as he left the education department. The inmate
received an incident report and was placed in SHU. It is unknown if there were any injuries to the staff member.

At 5:18p.m., staff responded to a cell fight involving two inmates. Staff was able to separate
the inmates and place them in different cells.
FCI- HERLONG- OCTOBER 8, 2010
At 5:09p.m., a unit officer announced a fight between two inmates. The Operations
Lieutenant ordered the unit locked down due to extenuating circumstances. After a review of the cameras, the unit was allowed
to come back up. The two inmates who were fighting were medically assessed, and escorted to SHU.
USP- LEWISBURG- OCTOBER 8, 2010
At 9:40a.m., a staff member was stabbed twice, once in the hand and once in the side with a
sharpened homemade knife. The staff member was taken to the Local Hospital for treatment.
**At 5:44p.m., staff responded to a cell fight involving two inmates. The inmates were separated
and placed in different cells.

At 2:47p.m., a cell fight was called in Special Housing. SHU staff gave both inmates several
orders to stop fighting and to submit to hand restraints and they both refused. After a sufficient amount of staff were present,
the cell door was opened and staff entered the cell and physically separated and restrained both inmates. One staff member
received injuries during this incident.

the inmates and place them in different cells.

trays when an inmate threw a cup of unknown liquids on him.
FCI- HERLONG- OCTOBER 7, 2010
At 5:37p.m., a housing unit officer announced over the radio that there were two inmates fighting
in front of the unit. The two inmates complied with orders to stop fighting. The inmates were separated, medically assessed
and escorted to Special Housing.
***At 8:50p.m., a fight was announced in the Special Housing Unit. Two inmates were
fighting in a cell. The inmates complied with staff orders to stop fighting and were separated without further incident.
USP- FLORENCE- OCTOBER 6, 2010
At approximately 12:30p.m., assistance was called for a staff assault in general
population. A staff member approached an inmate to confiscate some unauthorized material. The inmate produced an ice pick
type weapon with approximately a one inch point. He stabbed the officer in the right shoulder blade and the left shoulder
blade area causing a half centimeter wound on one shoulder and a one center meter wound on the other shoulder. The officer
was stabbed a third time in the right upper arm causing a one inch deep puncture wound. The second officer that was injured
misplaced his footing running to the incident and pulled a hamstring and possibly has a fractured elbow. Both officers are
at home now and recovering.

At 7:17p.m., assistance was called for an assault on staff in R&D. As the staff member was
attempting to remove the restraints in R&D, the inmate attempted to pull the staff member through the wicket. The staff
member received minor cuts and injured his finger.

At 11:00a.m., staff responded to a cell fight involving two inmates. Staff were able to separate
the inmates without further incidents.

At 8:33p.m., staff responded to a cell fight. Responding staff were able to separate the inmates
without further incidents.
FCI- MEMPHIS- OCTOBER 1, 2010
At 10:14 a.m., staff requested assistance after observing an inmate striking the Special Housing
Unit Officer in the face with his closed fist. Immediate Use of Force was initiated and the inmate was restrained by responding
staff. The inmate was escorted to the Health Services Unit and evaluated noting minor injuries. The Officer was medically
assessed noting minor injuries and was referred to his personal physician for further evaluation. The inmate was escorted
to SHU pending further investigation.
USP- TERRE HAUTE- OCTOBER 1, 2010
An inmate ran from staff during a shakedown outside the housing unit. The inmate knocked down
another staff member. There were no injuries noted to the staff member.

was able to separate the inmates and remove them.

At 8:57p.m., a fight was announced in a housing unit. It was an inmate on inmate fight. One
inmate was taken to outside hospital for head trauma. There were no staff injuries to report.
USP- LEWISBURG- SEPTEMBER 30, 2010
At approximately 8:50a.m., an inmate assaulted a staff member by throwing an unknown liquid
substance through the side of the cell door, striking the staff member in the front of the body.
USP- POLLOCK- SEPTEMBER 30, 2010
At approximately 3:32 p.m., an officer activated her body alarm. While conducting a search of
the cell assigned to an inmate, the officer found what appeared to be home made intoxicants and instructed the inmate to exit
the cell. As the Officer opened the bottom locker and found a large bag of homemade intoxicants, the inmate then pushed the
Officer in an attempt to get the intoxicants from her. The officer was able to place the inmate in hand restraints and wait
until additional staff arrived. Staff responded, secured the unit, and the inmate was escorted out of the unit. The inmate
was medically assessed with no injuries noted. The staff member was medically assessed with the following injuries noted,
twisted right ankle and soreness to the right side. The staff member went to local hospital for precautionary treatment. The
inmate was placed in SHU.
FCI- DANBURY- SEPTEMBER 30, 2010
At approximately 12:00 p.m., staff requested assistance upon observing two inmates striking
each other with closed fists in the center of the compound. The inmates were separated, medically assessed and placed in Administrative
Detention without incident. One inmate sustained minor multiple abrasions to the face and right elbow. The other inmate did
not have any notable injuries.
FCI- FT. DIX- SEPTEMBER 30, 2010
At 12:05 p.m.,an inmate was observed in his assigned room in the Housing Unit, talking on a
cellular phone. The Unit Officer ordered the inmate numerous times to hand over the phone, but the inmate refused and charged
toward the staff member, striking her on the upper right side of the body. The inmate fled the scene and was located by responding
staff. The inmate was medically assessed and escorted to the Special Housing Unit without any further incident. The staff
member was medically assessed and was treated.
CCM- PHILADELPHIA- SEPTEMBER 30, 2010
At 8:00a.m., staff reported being assaulted by an inmate during a cell sanitation inspection.
Specifically, when the staff member emptied the inmates bowl into the trash which contained unauthorized food items, the inmate
grabbed his right forearm and snatched the bowl out of his hand. Upon notification to the Lieutenants Office, the inmate was
placed in hand restraints and escorted to the Special Housing Unit without further incident. Both staff and inmate were medically
assessed.
FCI- BECKLEY- SEPTEMBER 30, 2010
At approximately 8:30 a.m., an officer informed the Operations Lieutenant that two inmates had
been in a fight. The officer observed the two inmates engage in a physical altercation, on the floor of the cell. The officer
ordered them to separate, to which they complied. The inmates were escorted to the Lieutenants' Office, interviewed, medically
assessed, and placed in the Special Housing Unit without further incident.
FCI- TALLADEGA- SEPTEMBER 30, 2010
At 09:20a.m., an officer was escorting an inmate back to his cell.
The inmate began to try to pull away from the officer and slip his ambulatory restraints. At that time the inmate slipped
his left hand free from two sets of restraints and began to fight with staff. An officer then attempted to regain control
of the inmate when he pushed the officers into the wall making one officer hit his head and shoulder. The inmate was taken
to the floor. The inmate was restrained and taken to medical for evaluation. The officers has minor injuries.
FCI- OXFORD- SPETMEBER 30, 2010
The institution was placed on lock down status due to gang related fighting that occurred in
three (3) separate units simultaneously. The inmates were separated upon arrival of responding staff. Those involved were
assessed by medical staff for minor injuries and placed in SHU. There were no staff injured.
USP- BIG SANDY- SEPTEMBER 30, 2010
At approximately 8:25 a.m., an inmate assaulted a staff member by throwing an unknown substance
through the food slot striking the staff member on the upper back and legs. The staff exited the range without further incident.
The inmate was removed from the cell, medically assessed and placed back into a cell.
FCI- RAY BROOK- SEPTEMBER 29, 2010
At approximately 9:00a.m., an inmate assaulted a staff member during a pat search. Responding
staff placed the inmate on the floor to gain control of the inmate and prevent further assaultive behavior. The inmate was
escorted to the Health Services Unit for an injury assessment with no injuries noted. The inmate was then placed in the Special
Housing Unit without further incident. All staff were medically assessed, and photographed with one staff member receiving
a superficial abrasion to the back of his right hand.
FCI- BASTROP- SEPTEMBER 29, 2010
At approximately 10:33a.m., an inmate approached the Food Service Corridor and stated he needed
to go to work. The Main Corridor Officer ordered the inmate to return to his assigned unit. The inmate became belligerent
and threatened the officer. The inmate assumed an aggressive posture with clinched fists and began to advance toward the officer
in an aggressive manner as he continued to verbally threaten the officer. The inmate struck the officer in the facial area.
Staff activated their body alarm and assisted the Main Corridor Officer in gaining control of the inmate. Health Services
staff examined the inmate with no injuries noted. The inmate was placed in Administrative Segregation without further incident.
FCC- TERRE HAUTE- SEPTEMBER 29, 2010
At approximately 1:32 p.m., the Warden authorized a use of force team to perform
a involuntary feeding on an inmate. This is due to his deteriorating health related to his declared hunger strike. The inmate
refused to voluntarily consume the liquid nutritional supplement. At this time medical staff attempted several times to insert
a tube into the inmate nasal passage in order to involuntarily feed him the supplement. Due to the inmates lack of cooperation
during the procedure, medical staff were unable to properly insert the feeding tube into the nasal passage. Following several
attempts, the inmate agreed to consume the supplement and he voluntarily consumed the supplement. He was provided 474cc of
a nutritional drink (Ensure) and 50cc of water. The inmate sustained no injuries during the procedure.
USP- LEWISBURG- SEPTEMBER 29, 2010
At 10:29a.m., staff responded to a fight in the east side recreation cages involving two inmates.
The two inmates were separated and returned to their cells.
**AT 2:49p.m., staff responded to an assault. A staff member
uncuffed one inmate and the inmate began hitting and kicking the other inmate in the cell. The inmates were separated by staff
without further incidents.
ADX- FLORENCE- SEPTEMBER 28, 2010
At approximately 7:45 a.m., the Operations Lieutenant was notified of a possible assault in
the recreation area. The SIS office was contacted and reviewed the video footage, which revealed at approximately 7:39 a.m.,
an inmate passed an envelope to another inmate. The inmate then threw a brown liquid substance at the other inmate striking
him in the face and upper torso. The inmate then refilled his cup with the assistance of another inmate and threw this brown
liquid substance at the inmate again. The inmates were medically assessed with no injuries noted and were placed in the Special
Housing Unit pending further investigation.
USP- CANAAN- SEPTEMBER 28, 2010
At approximately 11:00a.m., SHU staff ordered an inmate to submit to restraints. He refused
and then broke the fire suppression sprinkler flooding his cell. He then escalated the situation by covering his window and
barricading his cell; and threatened to inflict serious injury upon himself and any staff who attempt to remove him from his
cell. The inmate was then inundated with two (2) two second bursts from the X-10 OC Dispenser. This proved ineffective and
a transition to the pepper ball launcher was conducted, and a total of 28 rounds were introduced into the cell. The inmate
was then removed from the cell, decontaminated, medically assessed, and placed in four point restraints until he regains control
of his emotions and his imminent displays of violence subside. An investigation continues.
USP- LEWISBURG- SEPTEMBER 28, 2010
At 7:46a.m., officers responded to a cell fight involving two inmates. Responding staff were
able to separate the two inmates without further incidents.

two inmates. Responding staff separated the two inmates and moved them.
USP- LEWISBURG- SEPTEMBER 27, 2010
At 9:16a.m., staff responded to a cell fight involving two inmates. The inmates were separated
without further incident.
USP- BIG SANDY- SEPTEMBER 26, 2010
Two staff were assaulted while assigned to the Special Housing Unit. The inmates where restrained
and took the officers down with their legs. Both staff members were kicked numerous times. The inmates were restrained with
no further incidents.
**A fight occurred on the recreation yard. Two inmates were assaulting one inmate. The tower
officer had to utilize a dispersion round to stop the assault. Staff responded and restrained the inmates with no further
incidents.
USP- HAZELTON- SEPTEMBER 25, 2010
At 02:50p.m., while conducting cell rotations in the SHU, an inmate kicked one of the SHU officers
in the left knee because he was upset about contraband that the officer had taken from him.
FCI- HERLONG- SEPTEMBER 24, 2010
At 7:18a.m., a fight was announced on the compound. Two inmates were fighting.
Responding staff separated the inmates, who were then medically assessed and escorted to SHU.
FDC- SEATAC- SEPTEMBER 24, 2010
While
an inmate was being transported from his cell by SHU staff, he spit on the assisting officer. The inmate then resisted being
moved and had to be placed on the ground. During this event, the inmate kneed and struck an officer in the lip causing a contusion.
The inmate also elbowed another officer causing a contusion. The inmate was carried to strip cage and dressed out despite his resistance, then transported to R&D.
USP- LEWISBURG- SEPTEMBER 24, 2010
At 10:51a.m., assistance was needed for a cell fight involving two inmates. Staff
separated the inmates without further incident.
At 1:19p.m., while escorting an inmate back to his cell, the inmate
kicked the escorting officer. Responding staff were able to gain control of the inmate without further incidents.
At 1:33p.m, officers responded to a fight in the outside recreation cages involving two inmates. The inmates
were separated and returned to their cells.
FCI- GILMER- SEPTEMBER 23, 2010
At 7:30p.m., approximately 100 gang member inmates jumped approximately 20 gang
rival inmates with rocks, horseshoes, fists, and boots in the recreation yard. One officer was hit in the face with a rock,
but he continued working. DCT was activated. Five inmates are in the hospital. One inmate was beaten severely with a horseshoe
and isn't critical condition. The institution is on lock down status.

At 6:30p.m., a fight was announced in the green corridor. It was an one on one inmate fight.
Both inmates were separated without further incidents. There were no staff injuries to report.
USP- LEWISBURG- SEPTEMBER 22, 2010
At 4:55a.m., officers responded to a cell fight involving two inmates. Staff separated the two
inmates and moved them to different cells.
At 1:37p.m., officers responded to a cell fight involving two inmates. Responding
staff were able to separate the inmates without further incidents.
USP- CANAAN- SEPTEMEBR 22, 2010
At 8:23p.m., a call for assistance was announced in SHU for a cell fight. It was an one on one
fight. Both inmates were separated when enough staff arrived. There were no staff injuries to report.
USP- LEE- SEPTEMBER 22, 2010
At 2:39a.m., staff members were removing two inmates from a holding cell in SHU. One inmate
spit on staff. Both inmates were already in restrains due to an earlier incident. The inmates were separated. It was determined
that they had been in a physical altercation with each other. The staff member was medically assessed with no further injuries.
FCI- BIG SPRING- SEPTEMBER 21, 2010
At approximately 3:00p.m., a unit officer was dealing with an irate inmate, in front of the
unit. The officer attempted to calm the inmate, but was unsuccessful. The inmate became more aggressive and chest bumped the
officer. The inmate was secured and escorted to special housing. The officer was assessed by medical, and was able to resume
his post.

At 6:54a.m., a fight was announced in a housing unit. It was a one on one inmate
fight. One inmate was taken to outside hospital for treatment. There were no staff injuries to report.
USP- LEWISBURG- SEPTEMBER 21, 2010
At 7:51a.m., staff responded to a fight involving six inmates. Responding staff
were able to separate and restrain the inmates.
At 8:31a.m., an officer activated his body alarm for a fight in the east side recreation cages. Six inmates
were involved. Staff were able to separate and restrain the inmates and take them back to their cells.
USP- LEE- SEPTEMBER 21, 2010
At 5:33p.m., a staff member was conducting a pat search of an inmate the inmate became assaultive
toward the staff member. Staff members discovered, what later tested positive for heroin, on the inmate. After staff members
removed the substance from the inmate he turned, lunged, and tried to take the substance
back from staff. The inmate was placed in restraints and taken to SHU. There were no staff injuries to report.
USP- HAZELTON- SEPTEMEBR 21, 2010
At 05:15p.m., an inmate in SHU was demanding new sheets, and when he did not receive them fast
enough he broke a sprinkler head then threw urine on the SHU Lieutenant.
FCI- BENNETTSVILLE- SEPTEMBER 20, 2010
At approximately 3:30p.m., a staff member in the special housing unit was assisting
SHU staff with escorting the inmates from recreation back to their cells. When staff opened the recreation cage door the inmate
assaulted the staff member by kicking him in the leg. The inmate was escorted back to his cell after staff regained control
of the inmate. The staff member was not injured.
USP- MCCREARY- SPETEMBER 20, 2010
At 11:10a.m., the institution was placed on lock-down status. The lock down was pro-active due
to intelligence that the inmate population was going to refuse to comply with a Inmate Restricted Movement Schedule that Management
was in the process of creating for proposal, and implementation.
FCI- OAKDALE- SEPTEMBER 19, 2010
At approximately 12:33 p.m., Special Housing Unit staff observed
an inmate fighting with two other inmates inside one of the recreation cells. Responding staff ordered
the inmates to stop fighting and they complied. The inmates were placed in restraints, medically assessed,
and placed back into their assigned cells without further incident.
FCI- WILLIAMSBURG- SEPTEMBER 19, 2010
A cell fight occurred in a general housing unit. The two inmates were medically assessed and
placed in SHU. One of the inmates is believed to have MRSA, which may have been a reason for the fight. There are also what
are believed to be other cases of MRSA in the institution. There were no staff were injured.
FCI- VITCORVILLE- SPETEMBER 19, 2010
At approximately 6:10 p.m., as staff attempted to conduct a pat
search on an inmate utilizing a hand held metal detector. The inmate attempted to evade staff by running into D-Lower unit,
and was unable to enter the unit because the entrance door was secured. The inmate then became aggressive, at which time he
was placed on the ground and placed in hand restraints. During a search of the inmate, staff discovered a
6" steel rod sharpened to a point concealed with the inmate’s shoe. The inmate was escorted to the Special
Housing Unit without further incident.
MDC- BROOKLYNN- SEPTEMBER 18, 2010
At approximately 8:56 p.m., an inmate was assaulted by three other
inmates. Due to the severity of their injuries, two inmates were transported to an outside hospital for further medical treatment.
USP- LEWISBURG- SEPTEMBER 17, 2010
At 7:42a.m., officers responded to a fight involving two inmates. Staff were able
to gain control and the inmates were placed in SHU.
*At 9:07a.m., an officer was escorting an inmate to recreation and the inmate became disruptive and aggressive.
Responding staff were able to control the inmate and take him back to his cell.

cell fight involving two inmates.
FCI- TERRE HAUTE- SEPTEMBER 16, 2010
Approximately 75 to 80 inmates were involved in a fight on the recreation yard.
The institution was placed on lock down status until further notice.
USP- CANAAN- SEPTEMBER 16, 2010
At 6:13a.m. a fight was announced on the compound. It was an inmate on inmate fight with weapons
involved. One inmate received a laceration to the face area. There were no staff injuries to report.
USP- LEWISBURG- SEPTEMBER 16, 2010
At 5:21p.m., staff responded to a cell fight in SMU involving two inmates. Staff separated the
two inmates and placed them in different cells.
FCI- FORREST CITY- SEPTEMBER 16, 2010
At approximately 12:35 p.m., eighteen gang related inmates refused
staff orders to submit to restraints and exit the Special Housing Unit’s recreation area. Confrontation
avoidance procedures were initiated, and was deemed unsuccessful. Due to their continued display of noncompliant behavior,
and a final order to submit to restraints was given. The inmates again refused to comply with staff. As a result, a 2-second
burst of oleoresin capsicum was dispensed into recreation cage. A second order to submit to restraints was given and all inmates
complied
USP- HAZELTON- SEPTEMBER 16, 2010
At 07:15p.m., an officer in SHU was spit on by an inmate as he was collecting food trays.
USP- LEE- SPETEMBER 15, 2010
At 7:00a.m., an officer announced a fight in a unit. The staff member observed an inmate with
blood on his head and face. The victim was secured in the laundry room. After reviewing the video surveillance the two assailants
were identified and placed in SHU. There were no staff injuries to report.
USP- LEE- SEPTEMBER 15, 2010
At approximately 7:00 a.m., staff requested assistance after observing
an inmate bleeding from an apparent head injury. The inmate was removed from the unit, medically treated, and placed in the
Special Housing Unit pending further investigation. Upon review of recorded video surveillance, staff observed an inmate strike
this inmate on the head with a lock affixed to a belt. Staff also observed an inmate assault the other inmate with his fists.
Both the assailants were placed in restraints, medically assessed, and escorted to the Special Housing Unit.
FCC- TERRE HAUTE- SEPTEMBER 15, 2010
At 12:15 a.m., an inmate was found dead in his cell in the SHU. He had apparently been beaten
to death by his cellmate.
USP- LEWISBURG- SEPTEMBER 15, 2010
At 1:04p.m., staff responded to the eastside outside recreation cages for a fight between two
inmates. Staff were able to separate the two inmates and return them to their cells.
FCI- BENNETTSVILLE- SEPTEMBER 14, 2010
At approximately 9:45a.m., an officer was called to the laundry department, via radio. When
officers arrived the laundry foreman said he was assaulted. The inmate threw his laundry bag at the foreman and hit him in
the stomach area through the window of the laundry cage. The inmate was escorted to SHU with no further incident. The staff
member was not injured.
USP- BIG SANDY- SEPTEMBER 14, 2010
At 1:00p.m., there was an assault on a staff member in an unit. While the officer was unlocking
the top range an inmate rushed the door as it was unlocked. The inmate struck the officer in the face with a closed fist.
The other officers in the unit restrained the inmate which was covered in feces.
USP- LEWISBURG- SEPTEMBER 13, 2010
At 9:15a.m., two inmates were locked up for an assault that occurred in general
population.
**At 12:05p.m., staff called for assistance for a two on one fight in the recreation cages. Responding staff
were able to separate the three inmates and return them to their cells.
USP- VICTORVILLE- SPETEMBER 13, 2010
At approximately 6:15 a.m., staff requested assistance after observing
two inmates assaulting another inmate with prison- made weapons. Responding staff ordered the inmates to cease their attack;
however, they refused. As two of the inmates attempted to flee the area, staff ordered the inmates to the ground and they
complied. The inmates were separated, restrained, and medically treated for minor injuries. During a search of the area, staff
recovered the two homemade weapons believed to have been used during the assault. All inmates were placed in the Special Housing
Unit without further incident.
FCI- FORREST CITY- SEPTEMBER 12, 2010
At approximately 1:45 p.m., Special Housing Unit staff requested
assistance after observing two inmates assaulting another inmate. Responding staff ordered the inmates to cease their attack
and they complied. The inmates were placed in restraints, medically assessed, and placed back into their assigned cells without
further incident.
FCI- CUMBERLAND- SEPTEMBER 11, 2010
At approximately 12:20 p.m., recreation staff requested assistance
after observing groupings and increased tension amongst numerous inmates and several gang rival inmates on the recreation
yard. When responding staff approached the group of inmates, they quickly dispersed.
FCI- BENNETTSVILLE- SEPTEMBER 10, 2010
Staff were escorting inmates from the SHU recreation cages back to their cells.
A staff member was in the process of removing the hand restraints. The staff member had one part of the restraint uncuffed
and the inmate took the restraints from the officer who still had control of the restraints and attempted to pull the restraints
into his cell, while the officer was attempting to uncuff the inmate, and this caused injury to the SHU officer's left hand.
FCC- COLEMAN- SPETEMBER 9, 2010
An inmate barricaded himself in his cell. He smeared feces on his window and staff could not
see in his cell. A request for chemical munitions was placed and denied . The team went to enter the barricaded inmate's cell,
and once the door was opened, the inmate complied.
USP- HAZELTON- SPETEMBER 8, 2010
At approximately 05:38a.m., an officer noticed
approximately 7-9 inmates grouped in the multi purpose room praying. The officer opened the door, and order them to stop praying
in the multi purpose room. Immediately an inmate started tell the officer that he was disrespectful, and placed his hands
on the officer’s chest pushing him out of the door. The inmate then charged at the officer, so the officer placed his
hands on the inmate’s chest to gain a safe distance. The inmate then began striking the officer with his fists approximately
7 to 10 times in the face, neck, and left shoulder area. The officer then pushed his body alarm. Responding staff arrived
and placed the inmate on the ground and placed him in hand restraints.
USP- LEWISBURG- SEPTEMBER 8, 2010
At 8:00a.m., an inmate on inmate assault occurred. As staff removed the hand restraints
from the first inmate, he turned and began assaulting the cuffed inmate.
**At 4:14p.m., officers responded to a call for assistance for a cell fight involving two inmates.
USP- LEWSIBURG- SEPTEMBER 7, 2010
At 10:30a.m., an officer called for assistance for a cell fight involving two inmates.
**At 10:42a.m., an officer called for assistance for a cell fight involving two
inmates.
**At 11:24a.m., a staff member was assaulted by an inmate. While shaking down the
inmate who was going to recreation, the inmate struck the officer and had to be taken to the floor.
**At 7:20p.m., assistance was called for a cell fight involving two inmates.
FCI- HERLONG- SPETEMBER 6, 2010
At approximately 1:00p.m., a major disturbance occurred on the recreation yard. The fights spilled
over on to the compound. Inmates inside the housing units tried to join in, but were locked in, so they started fighting in
the units. The inmates were fighting with closed fists, as well as homemade weapons. There were approximately 200 inmates
involved. The command calling system was activated, and it took responding staff until 3:30 p.m. to secure the compound. Chemical
agents and less lethal munitions were deployed to gain control of the inmates. 20 inmates were taken to outside hospitals
for treatment from various injuries as a result of this disturbance. Out of the 20 inmates only 2 were admitted to the hospital,
the others were treated and released. This disturbance was a result of the fight that happened yesterday, in the RDAP unit
over control of the TV's. Only a minor elbow injury to a staff member occurred. The institution was placed on lock down status
until further notice.
FCI- HERLONG- SEPTEMBER 5, 2010
At 12:46p.m., a fight was announced in recreation. Two inmates were fighting. They
were escorted to SHU without further incident.
***At 8:50p.m., an unit officer announced a fight in the RDAP unit.
Two inmates were fighting over the TV. Both inmates were medically assessed and escorted to SHU without further incident.
The unit will be kept locked down until mass interviews can be conducted, due to high tension between the inmates.
FCI- TALLADEGA- SEPTEMBER 5, 2010
An officer observed an inmate striking another inmate with closed fist. The assaulted inmate
was not hitting back. The staff member ordered the inmate to stop but the inmate did not comply and began striking the inmate.
The inmate also threw something down the toilet which later was recovered to be a metal rod. Once enough staff arrives the
inmate was placed in cuffs and placed in SHU. The other inmate was taken to health service. There were no staff injuries to
report.
USP- LEE- SEPTEMBER 5, 2010
At 4:18p.m., a cell fight was announced in a general housing unit. Responding staff ordered
both inmates to stop fighting and to submit to hand restraints. Both inmates complied and were removed from the cell. After
a medical assessments were complete one inmate was sent to an outside hospital for more treatment. At 10:40p.m., the inmate
was sent to a trauma center from the local hospital. The inmate remains in the trauma center due to his injuries. There were
no staff injuries to report.
FCI- HERLONG- SEPTEMBER 4, 2010
At 8:40a.m., an inmate ran out of the recreation yard and ran across the compound. The Operations
Lieutenant followed the inmate. The inmate had markings consistent with a fight. The inmate stated he had been assaulted by
two other inmates. The assault victim was medically assessed then taken to SHU.
**At 8:22p.m., a fight was announced
in food service. Three inmates were fighting. Responding staff separated the inmates and they were medically assessed, then
escorted to SHU.

At 9:55a.m., an officer called for assistance for a cell fight involving two inmates.
*At 10:40a.m, officers called for assistance for a fight involving two inmates
in the recreation cages.

in the recreation cages.
USP- LEE- SEPTEMBER 1, 2010
At 6:11a.m., control announced that staff needs assistance
in a general housing unit. Two inmates were assaulting another inmate. Responding staff had to physically separate and restrain
all three inmates. There were no staff injuries to report.
USP- HAZELTON- AUGUST 31, 2010
At 12:45p.m., several assistance calls were made for multiple inmates fighting
on the recreation yard and the corridor. Weapons were involved in the fights. There were no staff injuries to report.
The institution was placed on lock down status until further notice.
MCC- NEW YORK- AUGUST 30, 2010
A staff member was assaulted while uncuffing an inmate inside of the recreation cage in SHU.
The inmate pulled the officers arm through the slot which caused the officer hand to be injured. The officer was taken to
the hospital. The inmate was placed in a cell.

Multiple inmates were involved in a fight in USP housing unit.

USP- LEWISBURG- AUGUST 26, 2010
At 7:25a.m., a cell fight involving two inmates occurred. Responding staff were able to separate the 2 inmates. There were
no staff injuries to report.
At 8:01a.m., assistance called was made for in the recreation cages for an assault. Staff
had to use less than lethal munitions to gain control of the situations and to separate the inmates. There were no staff injuries
to report.
FCI- HERLONG- AUGUST 26, 2010
At approximately 7:55p.m., an officer announce a fight on the recreation yard.
Two inmates attacked one inmate. The fight ended before staff arrived. Upper body checks were initiated, and three inmates
were found to have markings consistent with a fight. The inmates were medically assessed and taken to SHU.
USP- LEWSIBURG- AUGSUT 25, 2010
At 5:43 p.m., assistance was called for a cell fight involving two inmates. Responding staff
were able to separate the 2 without the use of less than lethal. There were not staff injuries to report.
FCI- BENNETTSVILLE- AUGUST 25, 2010
At approximately 11:30a.m., a female staff member was assaulted by an inmate while
standing in the mainline inside the food service. The male inmate put his hand on her leg. The inmate was escorted to SHU
without further incident.
FCI- TALLADEGA- AUGUST 24, 2010
An officer got choked out by a SMU inmate. There were two inmates in the cell with
eight staff members prepared to go conduct a force cell move. The inmate that choked the staff member out was lying on his
bunk and all of a sudden attacked the officer. The officer was sent to the Emergency Room.
FCC- TERRE HAUTE- AUGUST 24, 2010
A group of gang rival inmates became combative in the USP SHU recreation cages. Inmates damaged
the recreation cages. DCT was called in and calculated use of force was initiated. All disruptive inmates were placed in ambulatory
restraints and order was restored.
USP- TUCSON- AUGUST 23, 2010
An inmate punched the lieutenant.
USP- CANAAN- AUGUST 23, 2010
At 8:20p.m., a medical emergency was announced in the laundry area. An inmate was bleeding from
the neck area. Staff responded and locked down the laundry area. The inmate had been stabbed in the neck area numerous times
by another inmate. The inmate was transported to an outside hospital were he was later pronounced dead from his injuries.
Institution was placed on lock down the FBI was notified . There were no staff injuries to report.
FCI- BENNETTSVILLE- AUGUST 22, 2010
At approximately 12:15p.m., the control center announced a body alarm in a housing

At 12:22p.m., a staff member was assaulted by an inmate. The inmate spit on a staff member.
Staff responded and the inmate was restrained.
USP- VICTORVILLE- AUGUST 19, 2010
At approximately 7:10 p.m., an officer called for assistance when
he observed three inmates assaulting staff with their fists. Staff ordered the inmates to cease theirs actions; however, they
refused. During the assault, one inmate produced a homemade weapon and attempted to use it on staff. Responding staff were
able to disarm the inmate and gained control of the inmates. All three inmates were placed in restraints, medically assessed,
and placed in the Special Housing Unit without further incident. No serious staff or inmate injuries were reported.

At 1:34p.m., staff called for assistance for a two on one assault in the recreation
cages. Responding staff were able to restrain and separate the three inmates and return them to their cells. There were no

cage. Responding staff were able to separate the inmates and return them to their cells with no further incidents. There were

FCC- COLEMAN USP 2- AUGUST 16, 2010
An officer was escorting an inmate through the scan machine to the strip search
area when the inmate became aggressive. The inmate struck the officer one in the face and scratched him in the face. The officer
is okay just a little sore.
FCI- OXFORD- AUGUST 16, 2010
At 3:30p.m., three gang members assaulted one rival gang member. All three of the inmates were
placed in SHU. There were no staff injuries to report.
MCC- NEW YORK- AUGUST 16, 2010
At 8:45a.m., a body alarm was announced in the R&D area. A female inmate attacked
a staff member. The inmate’s shoulder bumped the officer so the officer pushed the inmate to the wall to gain control
. The inmate was able to scratch the officer in the eye and face. The inmate continued to fight with responding staff, a staff
member received injuries to her arm while fight with the inmate. The inmate was placed on the floor and placed in handcuffs.

A fight occurred with 25-35 inmates involved in a housing unit. This fight was
a gang related fight. There were no staff injuries reported.

At 12:36p.m., staff called for assistance for a cell fight. Staff members were able to separate
the two inmates and place them into different cells.
USP- POLLOCK- AUGUST 13, 2010
A staff member was assaulted in his office by an inmate. Other inmates helped restrain the assaulting
inmates until enough staff arrived to gain control of the inmate. The staff member was assaulted pretty bad but was not hospitalized.

At 8:20a.m., officers called for assistance for a fight involving two inmates in
the recreation cages. Staff separated the inmates and escorted them back to their block.

At 4:28p.m., a cell fight was announced in SHU. It was an inmate on inmate fight.
Both inmates were separated without further incidents. There were no staff injuries to report.

At 8:41a.m., an assistance call was made for a fight in the recreation cages. Four
inmates were in the cage although it seems only two inmates were involved. Staff was able to separate the inmates and all
four were escorted back into their unit. There were no staff injuries to report.
**At 5:34p.m., staff responded to a call for assistance for a cell fight. Responding
staff were able to restrain and separate the two inmates. There were no staff injuries to report.

A fight occurred in the dinning hall and inmate was stabbed numerous times. Approximately
one minute later another assault occurred in a housing unit. One inmate was transported to the local hospital. The institution
was locked down for one day.

A body alarm was activated due to a fight in progress on the recreation yard. Responding staff isolated
the incident and the inmates suspected of being involved were removed from the area. There were approximately 75 inmates involved.

MCC- SAN DIEGO- AUGUST 9, 2010
An officer was assaulted by an inmate . The officer hit his body alarm and continued to defend
himself until enough staff arrived to control the inmate. The officer was taken to the hospital for stitches.

An inmate assault occurred in a housing unit. There were two inmates on one inmate. The inmate
was beat with a lock in a sock. The inmate was transported to the local hospital.
FCI- TALLADEGA- AUGUST 5, 2010
TALLADEGA — An inmate at Talladega’s
Federal Correctional Institute was pronounced dead early Thursday morning after being involved in an altercation with another
inmate Wednesday.
Bobby Wayne Cowley, 32, “was pronounced dead
at a local hospital” at 3:40 a.m., according to Charles Sansom, executive assistant to the warden at FCI.
“Cowley was serving a 60-month sentence for possession of a firearm in furtherance
of drug trafficking,” he said. “The next of kin have been notified
(and) the death is currently under investigation.”
Cowley was sentenced to prison by the federal court for the
Northern District of Texas. According to the online federal inmate locator,
he was due to be released April
9, 2012.
Sansom declined to comment on Cowley’s cause of death or any details of the altercation leading
up to it.
FCI- TALLADEGA- AUGUST 4, 2010
At approximately 11:00 am, while working in SMU an officer asked another officer
to do a round on a range. The officer arrived at a cell and a inmate was yelling that his cell mate needed medical attention.
The officer looked into the cell and observed the inmate on the floor with blood surrounding him. The inmate was then taken
down for medical assistance.
FCI- GREENVILLE- AUGUST 4, 2010
At 7:00p.m., approximately 40 inmates were involved in a fight in the recreation yard. There
were no staff injuries to report. At least one inmate suffered from puncture wounds to the abdomen. The institution is locked-down
and SIS is reviewing the video. The word is we will be conducting mass interviews and shakedowns.

At 10:50 a.m. a call for staff assistance was announced in SHU.
An inmate attempted to assault a staff member by spitting on him at which time the staff member took control of the inmate
and placed him on the floor till more staff arrived on the scene. There were no staff injuries to report.

on inmate fight in a recreational cage. Both of the inmates were separated without further incident. There were no staff injuries

At approximately 4:10p.m., the Lieutenant announced a cell fight in R&D, after just arriving
at the institution from a bus run. Two inmates of different gang affiliation were fighting. The inmates were separated, medically
assessed, and escorted to SHU.
USP- ATWATER- JULY 31, 2010
At 7:11p.m., there was a use of force team assembled in the special housing unit, due to two
inmates hitting a officer with the food trays and holding the trap. The officer was medically assessed and only had minor
bruising to his hand and abdomen. There were nine use of force team members due to there were three inmates in the cell. There
were no other staff injured. The inmates were placed in precautionary restraints due to the inmates refusing to submit to
hand restraints.

A large fight broke out between gang rival inmates. More than a hundred inmates
were involved and some used weapons. Lethal munitions were used in order to get the inmates under control. All inmates were
placed under control and put back into their cells without any further incidents. There were no staff injuries to report.

The institution has had three gang related confrontations involving over 200 inmates each time.
The first incident was at 6:30p.m. on July 29, 2010. The second confrontation was at 9:00a.m. on July 30, 3010. Followed by

At 11:30a.m., a fight was announced in an unit with weapons involved. Three inmates were airlifted
to the trauma center. The institution was placed on lock-down status.

At 7:54a.m., staff called for assistance for a two on one assault outside recreation cages.
There were no weapons involved. Responding staff removed the two assailants. The victim was taken to an outside hospital.

At approximately 6:50a.m., an inmate in SHU broke the fire sprinkler head in his cell.
**At
approximately 2:00p.m., SHU announced a fight in a cell. One inmate attacked his cell mate striking him about the face and
body with closed fists. When the inmate was ordered to cuff up the attacking inmate complied. The inmate victim had to be
taken via ambulance to the hospital due to broken ribs and a punctured lung.

At 9:14a.m., a call for assistance was announced for a cell fight. Staff separated the two inmates
and placed them in separate cells. One of the inmates refused staffs orders to relinquish to the restraints. There were no
injuries to staff.
USP- CANAAN- JULY 24, 2010
At 8:40p.m., a fight was announced in a housing unit. An inmate threw a hot substance into the
face of another inmate causing burns. He then continued to punch and kick the inmate who was down on the ground. Two more
inmates became involved in the fight and one inmate had a shank and stabbed the inmate that was burned multiple times in shoulder
area. All of the inmates were separated and the housing unit was placed on lock down. There were no staff injuries to report.

At 12:00p.m., an assistance call was made for a cell fight. It was a cell fight involving two
inmates. Gas was used to gain compliance of the inmates. There were no staff injuries to report.
USP- TUCSON- JULY 22, 2010
At approximately 9:00p.m., another officer was assaulted by an inmate. The inmate attempted
to strangle the officer. The institution did not lock down. The officer is back at work.
USP- LEWISBURG- JULY 21, 2010
At 6:30p.m, a use of force team was assembled for a calculated use of force for 2 inmates who
were barricaded in their cell. Chemical agents were utilized and staff was able to go in and retrieve the inmates. There were
no staff injuries to report.
FCC- OAKDALE- JULY 20, 2010
An inmate in a shower requested to speak with the Lieutenant. The inmate then struck the Lieutenant
in the left front pocket with a shank. The Lieutenant had a note pad in the pocket so the shank didn’t puncture the
skin. The inmate was placed in SMU.
FCI- HERLONG- JULY 20, 2010
At approximately 9:50p.m., a unit officer was locking down his unit when he came across an inmate
whose head was covered in blood and was acting incoherent. This inmate also had other markings consistent with fight. The
officer called the Operations Lieutenant, and the inmate was escorted to the Lieutenants office. Upon review of the video,
it showed the inmate was assaulted by two inmates. The assaulted inmate then staggered into his cell. Another inmate then
went in after him and came out approximately 2 minuets later. The inmate was medically assessed then taken by ambulance to
an outside hospital due to extensive head trauma.

At 8:04a.m., staff responded to a block inside the recreation area for a fight involving three
inmates. Staff members were able to separate the three inmates and remove them from the area without further incident.
**At 8:15a.m., staff responded to the recreation yard for a fight involving two inmates. Officers
had to use gas, a pepper ball gun, and a flash bang to get the inmates to stop fighting and submit to hand restraints. Staff
then separated the two inmates. There were no staff injuries to report.

Responding staff applied hand restraints and separated the inmates without further incident. There were no staff injuries

and both inmates were separated. There were no staff injuries to report.
**At 3:51p.m., another fight was announced in a SHU recreation cage. It was also

An inmate on inmate assault with weapons occurred in a housing unit. One inmate was sent to
a local hospital for treatment of the stab wounds.
USP- TUCSON- JULY 16, 2010
At approximately 6:15a.m., staff responded to a body alarm in the housing unit. The female officer
in the unit was attacked by an inmate in a unit offericer's office and he attempted to strangle her. She was separated
from her body alarm. She screamed and other inmates in the unit responded to help her and got the inmate who was attacking
her away from her. The officer is back at work.

At 4:55p.m., a fight was announced in a housing unit. It was an inmate on inmate
fight and both inmates were placed in SHU. There were no staff injuries to report.

involving three inmates. Two inmates attacked one inmate. When staff responded, the inmates ceased fighting and allowed staff
to restrain them. The three inmates were removed from the cage, medically assessed and returned to their cells.
**At 9:00a.m., staff responded to an assistance call for a cell fight involving two inmates. Staff applied
hand restraints and the inmates were separated.

USP- ALLENWOOD- JULY 13, 2010
An employee at was blindsided by an inmate as he was walking through a doorway and punched in
the face. The employee received minor injuries. The inmates was then put in restraints and placed in SHU. USP-Allenwood had
just returned to normal operations from a two week institutional lock-down the day prior after several staff had been
assaulted in a pre-meditated incident.
MDCC- NEW YORK- JULY 13, 2010
As an inmate was being removed from the strip cage, he became argumentative with the escorting
officer and refused to be escorted. The inmate tried to walk past the officer without being escorted and when the officer
tried to grab onto the inmate, the inmate elbowed the officer in the rib area. Two other officers then gained control of the
inmate and escorted him to his cell without further incident.

At about 5:17p.m., assistance was called for a cell fight with two inmates fighting.
Responding staff ordered the inmates to cease fighting and to cuff up. Both inmates submitted, were medically
assessed and placed in separate cells. There were no staff injuries to report.

At approximately 9:00a.m., an inmate assaulted a staff member by making inappropriate
comment toward the staff member. As staff was telling the inmate to leave the area, the inmate slapped the staff on the hands
and left. The inmate was sent to SHU.
**At 10:30a.m., an officer was conducting rounds in the housing unit when he saw
an inmate talking on a cell phone. The inmate tried to flush the phone down the toilet but the officer grabbed the phone and
wrestled with the inmate. The officer got the room secure when another inmate came and hit the officer in the ribs. The officer
then fell to the floor causing him to sprain his ankle.

At approximately 8:25a.m., the control center announced a body alarm in a housing unit. The unit officer had ordered an inmate out of a cell for a pat search. While attempting to pat search
the inmate another inmate ran over and got between the officer and the first inmate. The second inmate then started assaulting
the unit officer by pushing him, at which time the first inmate ran away. The first responding staff member saw the first
inmate running by him and gave chase into a cell. The first inmate then began assaulting the responding staff member. Enough
staff arrived and were able to place both inmates on the ground and in restraints. The inmates were then escorted out of
the unit, medically assessed and placed in SHU.

At 6:08p.m., a fight was announced in a housing unit. Responding staff separated
the inmates and escorted them to SHU. There were no staff injuries.
FCI- LORETTO- JULY 6, 2010
At approximately 12:04p.m., during the afternoon mean an inmate became disruptive.
The inmate refused orders of the staff to be handcuffed and transported to SHU. When staff attempted to physically secure
the inmate he became assaultive. After staff obtained control of the inmate, he was placed in restraints and had to be physically
carried to SHU by six staff. Some of the staff received abrasions.

At 2:24p.m., control announced a fight in front of a building. One inmate was chasing
another with a weapon. Responding staff had to separate and restrain both inmates. Staff recovered the weapon. One inmate
was taken to an outside hospital due to his injuries. There were no staff injuries to report.
**At 3:09 p.m., while making rounds in a general housing unit, staff observed what appeared to have been a
physical altercation in a cell. Staff secured the cell with three inmates inside. The unit was secured and upper body searches
conducted and a victim was identified. Four inmates involved were placed in SHU. There were no staff injuries to report.
***At 8:22p.m., a fight was called outside the dining hall. Responding staff physically separated and restrained
the inmates. There were no staff injuries.

Staff discovered an inmate on the recreation yard with head injuries consistent
to that of being in a physical altercation. The inmate was sent to an outside hospital due to the extent of his injuries.

FCI- BIG SPRINGS- JULY 2, 2010
At approximately 8:30 a.m., a compound officer found blood on the fence from the razor wire
to the ground in the recreation yard. A total yard recall was called and emergency count was conducted. All inmates were accounted
for, and no injuries were found. At 9:00 a.m., the local police were called, and a test was conducted on the blood. It tested
positive for human blood. The blood was tested 6 more times, and in every case was found to be human. The FCI is on lock down
until further notice, due to an escape attempt.

At 2:06p.m., a fight was announced in front of a building. Seven inmates were fighting. Responding
staff had to physically separate all involved inmates. After this incident three sharpened weapons were taken from these inmates.
There were no staff injuries to report.
FCC- TERRE HAUTE- JULY 1, 2010
An inmate was placed in the SHU for assaulting a staff member during the conduct of a pat search.
**An inmate was pronounced dead at local hospital. The inmate had been in the hospital since
his involvement in a physical altercation with another inmate in the FCI SHU on 6/29/10. The inmate was strangled during the
altercation.

At 10:33a.m., a fight was called in special housing. One inmate was assaulting
another in a cell. Once sufficient staff were present the door was opened and staff had to separate the inmates. One staff
member was injured during the incident requiring treatment at outside hospital.
**At 7:19p.m., a fight was called in a housing unit courtyard between two inmates with weapons involved. Staff
separated the inmates and escorted them to SHU. There were no staff injuries to report.
**At 9:03p.m., a fight was called in a housing unit courtyard between two inmates. Staff separated the inmates
and escorted them to SHU. There were no staff injuries to report.

At 7:25 p.m., a gang related disturbance occurred on the recreation yard. Eleven
staff members reported minor injuries. Seven inmates were sent to local hospitals for treatment. One inmate suffers from life
threating injuries. The institution was placed on lock down status.

At 6:28a.m., an unit officer announced a fight in the housing unit. Three inmates assaulted
one inmate. Once enough staff were present the unit was locked down and upper body searches were conducted . One staff member
received injuries requiring outside medical treatment.

At 6:19p.m., two inmates attacked one on the recreation yard. The victim was laying on the ground
while two assailants were kicking him in the head/face area. The assailants had to be placed on the ground by responding staff
and then were restrained. There were no staff injuries to report.

At 7:10p.m., a fight was announced in a housing unit. Three inmates were fighting with each
other. They were separated and secured in cells. After responding staff arrived they were restrained and taken to SHU. There
were no staff injuries to report.

At approximately 7:20a.m., an officer announced a fight in a SHU cell. One inmate was restrained,
the other was not. The unrestrained inmate was hitting the restrained inmate with closed fists about the head and body. When
sufficient staff arrived for immediate use of force the restrained inmate was unconscious on the floor of the cell. The cell
door was opened and the attacking inmate was placed on the floor and restrained. The inmates were separated and medically
assessed.

At approximately 6:55p.m., officer’s were escorting two inmates to the institution’s
library. Once the inmates were placed inside the library law and the door was shut, one inmate slipped off his handcuffs and
started assaulting the other inmate.
USP- POLLOCK- JUNE 24, 2010
At approximately 12:43 p.m., an unit officer observed an inmate
covered with blood in the common area of the housing unit. The Unit Officer immediately called for assistance, and responding
staff began securing inmates in their cells. Medical staff arrived and escorted the inmate to the Heath Services Department
for medical assessment. The inmate was medically assessed and sustained blunt force trauma to the head. Upper body searches
were conducted on all inmates in the housing unit and three inmates were escorted to the Lieutenants Office as possible assailants
in the incident. The inmate was transported to the local hospital due to the extent of his injuries. The inmate was pronounced
deceased at 3:00 p.m. The FBI was notified and an investigation continues.

An inmate was placed in the SHU after staff observed him with facial injuries consistent with
having been in a fight. He was later transported to the local hospital for treatment of his injuries.

An officer was assaulted by an inmate while picking up food trays. The inmate threw a cup of
urine on the officer as the officer approached his cell.
USP- LEWISBURG- JUNE 21, 2010
At approximately 10:25 a.m., an officer called for assistance
after observing an inmate striking another inmate, with his feet and closed fists.
Upon the arrival of responding staff, the inmate was observed choking inmate the other inmate. Responding
staff separated the inmates and medical assessments were conducted. The assaulted inmate sustained multiple head trauma and
bruising on his forehead, temple, and neck. The inmate was transported to the local hospital for further treatment.

An inmate told an officer that he had been assaulted by another inmate. After reviewing video
it was determined that the inmate had been in an altercation with another inmate. After being medically assessed the inmate
was taken to an outside hospital for more treatment. There were no staff injuries to report.
USP- ALLENWOOD- JUNE 21, 2010
Three staff members were assaulted while trying to contain inmate on inmate gang related violence.
All three staff members sustained injuries. The institution was placed on lock down status until further notice.

At 5:14p.m., recreation staff announced a fight on the recreation yard. There were approximately
30 rival gang members engaged in a fight with weapons. Staff orders to stop fighting went unheeded. Chemical munitions were
deployed, and approximately 10 minuets later the inmates laid down to be restrained. The inmates were separated, medically
assessed and placed in SHU.

At 6:56a.m., control announced a body alarm activation in SHU for a fight in one of the recreation
cages. The inmates complied with staff orders to cease fighting and cuffed up. The inmates were separated and medically assessed.

At approximately 6:35 a.m., staff observed an inmate being assaulted by two
inmates with closed fists and an eight inch homemade knife. The incident was secured by responding staff
and all inmates involved were escorted to Health Services for medical assessments. One inmate sustained
a puncture wound to his left leg. The other inmates sustained no injuries. All inmates were placed in
Administrative Detention.

At 3:24p.m., a fight was called in front of a building. Responding staff had to physically separate
and restrain both inmates. There were no staff injuries to report.
FCI- HERLONG- JUNE 19, 2010
At approximately 3:30p.m., a staff member noticed that one of his workers had markings about
his face consistent with a fight. The inmate was sent to the Lieutenants office. Upon questioning the inmate said that he
was hit by an inmate of a different race because of personal issues, but would not disclose who the attacker was. The inmate
was medically assessed and placed in SHU.

At 6:17p.m., a fight was announced in the gym. One inmate was assaulting another. Responding
staff had to physically separate and restrain the inmates. There were no staff injuries to report.
FDC- DUBLIN- JUNE 16, 2010
At 6:15a.m., a fight broke out in an unit involving six gang affiliated inmates. The unit was
locked down and no staff were injured.
FCI- HERLONG- JUNE 16, 2010
At approximately 8:15a.m., an unit officer was conducting the AM census count and came across
two inmates in a cell who both had facial contusions and other markings consistent with a fight. Both inmates were sent to
the Lieutenants office and subsequently placed in SHU.

At 7:30a.m., an inmate assaulted a food service foreman. The inmate punched the officer twice
in the face.

At 11:15a.m., a fight was announced on the compound for two inmates fighting. Inmates complied
with verbal commands to stop fighting. One inmate was escorted to SHU, the other was unable to be located.
USP- BEAUMONT- JUNE 15, 2010
At approximately 7:40a.m., a disturbance occurred involving several different gang
affiliated inmates. Fighting occurred near food service, commissary, one housing unit and the Rrecreation yard. Approximately
100 inmates were involved. There were no staff were injured. The institution is on locked down status until further notice.
USP- LEE- JUNE 14, 2010
At 3:41p.m., a staff needs assistance call came from a housing unit. Staff witnessed an inmate
exit a cell with blood on his clothes and enter another cell. Responding staff secured both the victim and the assailant without
incident. There were no staff injuries to report.
FCI- HERLONG- JUNE 13, 2010
At approximately 2:30a.m., an officer in SHU radioed control about two inmates
that were intoxicated and fighting in a cell. Multiple orders were given to the inmates to cuff up, but the inmates didn’t
comply. The Operations Lieutenant deployed non-lethal munitions at both inmates with negative results. Two use of force teams
were assembled, and confrontation avoidance was initiated with negative results. Chemical munitions were again used at 7:45a.m.
with negative results. The inmates were given a final order which was refused. At 7:50a.m., the two use of force teams made
entry into the cell, restrained and separated the inmates, who were then decontaminated and placed in holding cells for medical
evaluations. There were no staff injuries to report.
USP- ATWATER- JUNE 12, 2010
At 4:32p.m., there was a fight announced in a housing unit with two inmates on
one inmate. This was a fight between two different gangs.
**At 4:34p.m., there was a fight between two inmates with weapons involved on the compound. The institution
was locked down for an emergency count and the following day it was let back up to normal operations.

Two inmates were involved in a fight in the housing unit. Both inmates were sent to SHU. There
were no staff injuries to report.
USP- CANAAN- JUNE 11, 2010
At 9:00a.m., a fight in SHU was announced. It was an inmate on inmate cell fight. The inmates
were separated. There were no staff injuries to report.
USP- BEAUMONT- JUNE 10, 2010
At approximately 1:15 p.m., staff observed an inmate with visible
injuries. Staff also observed a large amount of blood and a homemade weapon on the floor outside of an
unit. The unit was immediately secured, and the inmate was escorted to Health Services were he was medically
assessed. Due to the severity of his injuries, the inmate was transported to an outside hospital for further medical treatment.
A review of the recorded video revealed the inmate was assaulted by a group of inmates with weapons. All
of the inmates were sent to SHU.
USP- BIG SANDY- JUNE 10, 2010
At approximately 12:41 p.m., staff requested assistance after
observing two inmates assaulting one inmate. Responding staff separated the inmates and restrained the inmates without further
incident. The assailants were medically assessed and escorted to SHU. The other Inmate was escorted to Health Services where
he was treated for a laceration to his right ear and multiple contusions and abrasions to his head and face.

At 8:42a.m., a fight was announced in the education department. It was an inmate on inmate fight.
There were no staff injuries to report.
USP- MCCREARY- JUNE 10, 2010
At approximately 3:28 pm, staff observed an inmate assaulting
another inmate with a prison-made weapon on the recreation yard. In an effort to prevent serious injury, the tower officer
activated the emergency response system which brought an end to the attack. One inmate was placed in restraints and sent to
SHU. The other inmate was sent to an outside hospital for the severity of his injuries.
USP- ATWATER- JUNE 8, 2010
At approximately 7:13 p.m., staff requested assistance after observing
several inmates fighting on the recreation yard. One inmate was armed with a lock affixed to a belt. As a result, the Tower
Officer activated the emergency announcement system which was deemed ineffective. As a result, one aerial dispersion round
was deployed which brought an end to the fighting. The inmates were separated by staff, placed in restraints, and escorted
to SHU. No serious injuries were reported.
FCI- TALLADEGA- JUNE 8, 2010
At approximately 1:20p.m., a call for assistance was called in a recreation
area. There was a fight between four inmates containing weapons. One inmate was sent to a hospital for puncture wounds. All
other inmates were treated by medical staff and released.
FCC- TERRE HAUTE- JUNE 6, 2010
Two inmates were involved in a fight in a housing unit. One of the inmates was taken to the
local hospital for treatment of his injuries.
USP- LEE- JUNE 4, 2010
At 3:52p.m., SHU staff called for assistance after two inmates started fighting
in their cell. One inmate had stabbed the other. The victim was on the top bunk while the assailant stood in the door and
would not show his hands to staff. Chemical munitions had to be used. After the third time munitions were used the assailant
then laid on the floor and staff entered the cell and restrained both inmates. There were no staff injuries to report.
**At 10:28p.m., a call for assistance came from a housing unit. Three staff members pulled an inmate form
his assigned cell after receiving a drop note that the inmate had a weapon. During the pat search of the inmate the inmate
became disruptive when staff discovered the weapon. The inmate tried to maintain possession of the weapon and in doing so
injured two staff members. Staff were able to take the weapon before the inmate could assault them. The inmate was restrained
and taken to SHU. The two staff members received injuries that required treatment at an outside hospital.
FLS- JESUP- JUNE 4, 2010
An inmate on inmate assault incident occurred.. An inmate assaulted another inmate
with a lock in a sock. The inmate was transported the local hospital sustaining head injuries.
USP- CANAAN- JUNE 4, 2010
At 6:05p.m., an inmate could not clear a metal detector and an officer attempted to pat search
the inmate. The inmate took off running on to the compound. The inmate then threw a shank on the roof of the facility. The
inmate was then restrained and escorted to SHU. There were no staff injuries to report.
USP- LEE- JUNE 3, 2010
At 8:28p.m., a fight was announced by control in a housing unit. Staff witnessed
one inmate throw a broom at two other inmates. The two retaliated with weapons and assaulted the first inmate. One of the
inmates held down the first inmate while the other stabbed him in the head and neck area several times. After responding staff
were on the scene the inmates separated and ran back to their assigned cells. While securing the unit staff found the weapon
used in a trash can. All involved inmates were taken to SHU. There were no staff injuries to report.
USP- MCCREARY- JUNE 3, 2010
At approximately 7:20a.m., an unit officer observed an inmate bleeding profusely from several wounds on
his head. The inmates were secured in their cells while upper body visual searches were conducted. Two inmates were identified
as the assailants. Both inmates were examined by Medical staff and placed in SHU. The assaulted inmate was transported to
an outside hospital for treatment.

At 10:28p.m., a call for assistance came from an unit. Three staff members pulled an inmate
form his assigned cell after receiving a drop note that the inmate had a weapon. During the pat search of the inmate, the
inmate became disruptive when staff discovered the weapon. The inmate tried to maintain possession of the weapon and in doing
so injured two staff members. Staff were able to take the weapon before the inmate could assault them. The inmate was restrained
and taken to SHU. Two staff members received injuries that required treatment at an outside hospital.
USP- MCCREARY- JUNE 2, 2010
At approximately 10:05a.m., staff requested assistance when an inmate jumped up from the chair during a
medical screening in Health Services and attempted to charge toward the examining physician in an aggressive manner. The escorting
officer immediately placed the inmate on the floor to gain control of her. Upon arrival of additional staff, the inmate was
placed in ambulatory restraints and escorted to SHU where she was medically assessed without further incident.
**At approximately 10:55a.m., staff discovered an inmate with his SHU cell window covered.
He was also using his mattress to barricade himself in his cell. Confrontation
was initiated with negative results. The inmate was given a final order to come to the door and submit to hand restraints
with negative results. Two 2-second bursts of OC chemical agent was dispersed from the MK-21 with negative results. The Use
of Force Team entered the cell and placed the inmate in hand and leg restraints. He was decontaminated, medically assessed,
and placed in ambulatory restraints.
**At approximately 11:15a.m., staff observed an inmate smearing feces on the
walls of his assigned cell. The inmate refused staff's orders to submit to hand restraints. A Use of Force Team was assembled
and confrontation avoidance proved ineffective. The inmate was given a last order to submit to hand restraints, to which he
complied. After being placed in an observation cell, the inmate manipulated his hand restraints, moving them from the back
to the front. The inmate was then placed in progressive ambulatory restraints. Medical assessments were conducted and an incident
report was written.
**At approximately 12:00p.m., an officer observed two inmates exit a cell and fall to the floor fighting
and punching each other. Staff ordered the inmates to stop fighting and they complied. Responding staff separated the inmates,
applied restraints, and escorted them to SHU with no further incident.
**At approximately 2:45 p.m., while conducting rounds in SHU, staff requested
assistance upon observing an inmate attempting to asphyxiate herself by tying strips of clothing around her
neck. The inmate refused staff orders to cease her actions. Responding staff immediately entered the cell, removed the strips
of clothing from the inmate’s neck and placed her in ambulatory restraints. The inmate was escorted to Health Services
and medically assessed. The inmate sustained a contusion to the left side of the neck. The inmate was subsequently placed
on suicide watch.
**At approximately 3:59p.m., an inmate was escorted from R&D for placement
in his assigned cell, upon which time he was placed in the first floor shower as a temporary holding area. The inmate then
stated he would not go into a cell with another inmate and verbalized threats of violence towards any inmate he was placed
in a cell with. The Warden was notified and authorization was received to place inmate in ambulatory restraints due to his
aggressive and assaultive behavior. At 5:01p.m., confrontational avoidance procedures were initiated with positive results.
At 5:23p.m., the inmate was placed into ambulatory restraints. A medical assessment was performed on the inmate with no injuries
noted.
**At approximately 9:10p.m., the Operation Lieutenant was notified that an altercation
between two inmates had occurred in front of a unit. A preliminary investigation revealed that one inmate assaulted the other
inmate by ripping a large portion of her hair from her scalp, pulling her head into the bench, and striking her in the face
with a closed fist. Both inmates were escorted to health services for a medical assessment and placed in temporary holding
pending an investigation.
USP- LEWISBURG- JUNE 1, 2010
At approximately 7:49 a.m., staff observed an inmate assaulting
another inmate. The assault occurred in a general housing cell. Both inmates were placed in hand restraints
and placed in SHU. Both inmates were transported to health services. One inmate had to be sent to an outside hospital for
further medical treatment. On June 15, 2010 the inmate was pronounced dead at the outside hospital due to his injuries.
USP- MCCREARY- JUNE 1, 2010
At
approximately 8:30 a.m., SHU staff observed several inmates striking each other with closed fists in the recreation cage.
Responding staff ordered the inmates to stop and submit to restraints. The inmates complied and were removed from the recreation
cage without further incident. All three inmates were medically assessed and returned to their assigned cells. An investigation
continues.
**At approximately
12:40 p.m., staff in SHU observed several inmates assaulting another inmate. Specifically, staff observed one inmate strike
another inmate with a closed fist to the facial area, which caused the inmate to fall to the ground. Two inmates then began
to strike and kick the inmate with closed fists and their feet. Staff also observed another inmate stomp the inmate in the
face/head area. All inmates submitted to hand restraints and were removed from the cage. The other inmate was unable to stand
or submit to hand restraints, so staff entered the cage and he was restrained and placed in a wheelchair. He was escorted
to the Health Services Unit, where he was treated for large contusions on the right side of his face and head, multiple small
abrasions on his scalp and knees, and a laceration on the left side of his upper lip. The inmate was escorted back to SHU
without incident. The other inmates were treated by the on-duty Registered Nurse and sustained no injuries. One staff member
sustained minor injuries as a result of the incident.
**At 1:04 p.m., an inmate refused
to submit to the application of hand restraints to be removed from the Special
Management cell and be placed in SHU. The Warden was notified and authorized the use of calculated force to remove the inmate
from the cell. At 1:09 p.m., after confrontation avoidance proved ineffective, the Lieutenant introduced the Use of Force
Team to the cell. Upon staff entering the cell, the inmate jumped on the back of the a team member and struck him in the lower
right jaw. The team was able to gain control of the inmate and place him in restraints. The inmate was escorted via van to
the SHU and medically assessed. The inmate received contusions of the face, neck and scalp requiring minor first aid. The
restraints were removed from the inmate and the team departed the cell without further incident. The staff member was medically
assessed and received a small laceration to his lower lip requiring no medical attention.
**An
inmate was observed in his cell with a homemade noose placed around his
neck. The inmate was ordered to submit to restraints and moved to an area where he could remain under constant staff supervision.
Psychology was notified and conducted an assessment. The determination was made that the inmate had no suicidal ideation but
was attempting to achieve secondary gain. The inmate was subsequently placed in soft ambulatory restraints without incident.
**At approximately
1:50 p.m., an inmate approached the Unit Officer and informed him that he could not be in the unit anymore. The Unit Officer
called for the Compound Officer to escort the inmate to the Lieutenant's Office. The inmate told the Lieutenant he had just
been released from SHU to general population, and upon arriving to the Unit, he was disciplined by a gang. The inmate was
medically assessed and placed in SHU without further incident.
**At approximately
3:15 p.m., an inmate, approached an Unit Officer and reported that he had been assaulted. The inmate was secured and escorted
to the Lieutenant's Office. The inmate was medically assessed, treated, and placed in SHU without further incident.
**At approximately
7:15 p.m., Recreation staff called for assistance on the recreation yard after observing several inmates fighting by the basketball
court. The Tower Officer activated the amplified verbal warning system and made an announcement on the megaphone at which
time, the inmates disregarded. The Tower Officer fired two less-lethal
rounds, at which time the inmates complied with staff orders. The inmates were then placed in hand restraints
and escorted to SHU without further incidents. Medical assessments were conducted on the inmates sustaining minor injuries.
One staff member sustained minor injuries as a result of this incident.
**At 7:55
p.m., a SHU Officer activated his body alarm when he observed an inmate,
cutting himself on the arm with a sprinkler head he had broken. Responding staff ordered the inmate to submit to hand restraints,
to which he complied. Staff escorted the inmate for a medical assessment, psychological evaluation, interview, and photographs.
He was secured in SHU without further incident.
**At approximately 7:58 p.m., an Officer announced
a fight on the recreation yard. Two inmates involved in a physical altercation. Responding staff escorted both inmates to
the Lieutenants' Office. The inmates were medically assessed and placed in SHU without any further incident.
**At approximately 11:30 a.m., while staff were delivering
food trays to inmates in SHU. Staff observed both inmates striking each other with closed fists in the chest and upper body
areas. Staff immediately gave the inmates direct orders to stop fighting, in which they complied and submitted to hand restraints.
Both inmates were medically assessed and placed in SHU.
**At approximately 11:37 p.m., an unit officer observed an inmate cutting
himself on the right arm. The officer called for assistance and responding
staff reported to the cell. Upon staff observation that the inmate was
physically harming himself, immediate Use of Force was used to control the inmate. The inmate was medically
assessed and place on suicide watch without further incident.
USP- MCCREARY- MAY 31, 2010
At
2:29 P.M., the Unit Officer responded to a duress alarm. While investigating the duress alarm, the unit Officer observed two
inmates striking each other with their fist to the upper torso, head, and facial area. The unit Officer notified control of
the fight and location via radio, then gave both inmates a direct order to cease their actions. Responding staff placed the
inmates in hand restraints and escorted them to Health Services where they were treated for minor injuries.
**At approximately
7:05 p.m, a call for assistance was announced in Health Services. Specifically,
staff observed an inmate take his left fist and hit another inmate on the left temple. Staff ordered the inmate to cease his
combative actions, and he complied. The inmates were submitted to hand restraints and were separated. The aforementioned inmates
were placed in the Administrative Detention pending investigation.
***At approximately 10:18 p.m., the Control Center announced
a fight in an Unit, after the Unit Officer activated his body alarm. Specifically,
the Unit Officer heard noise coming from a cell. He responded to the cell and observed an inmate strike another inmate, in
the face with a fist. The inmate retaliated striking the other inmate in the back of the neck with his fist. The Officer gave
several orders for the inmates to stop fighting, to which the inmates complied. Responding staff separated the inmates, placed
them in hand restraints and escorted them to SHU. The inmates weremedically assessed, photographed, interviewed, and secured
in SHU without further incident.
****At
approximately 10:20 p.m., an Unit Officer activated their body alarm in response to two inmates fighting in the TV room. Two
inmates were observed exchanging punches with closed fists. The Officer ordered both inmates to separate but with negative
results. Upon the arrival of additional staff, both inmates were separated. Both inmates were medically assessed and escorted
to SHU without further incident.
USP- MCCREARY- MAY 30, 2010
At
approximately 11:17 am, two inmatesbegan striking another inmate with closed fists to the head and upper torso, after he was placed in the SHU recreation cage. The inmates did not comply with staff's orders to stop and get on the ground. Once enough responding staff were present the recreation door was opened and all three inmates were separated, placed on the ground, and restrained. The inmates were medically assessed and placed back in their cells without further incident.
**At 9:03 p.m., the
Unit Officer announced a fight in the unit after observing several inmates fighting with other inmates. Responding staff observed
the inmates involved in the fight striking one another in the head and torso areas with sharpened metal prison made weapons
and combination locks attached to institution issued nylon belts. The inmates did not comply to the direct orders given by
staff to cease their actions, and the Operations Lieutenant authorized the deployment of less-lethal munitions in the Unit.
A total of 22 inmates were placed in SHU. One inmate was transported to an outside hospital for treatment due to the severity
of his injuries.
FCC- COLEMAN USP 1- MAY 30, 2010
At approximately 3:23p.m., a call for assistance was made by an officer who observed staff attempting
to secure an inmate during the recall move for the north side. Specifically the officer was monitoring the recall move for
the north side when an inmate attempted to enter the recreation yard, an officer stopped inmate The officer instructed him
to submit to a pat search, once the officer began the pat search of the inmate, he felt something in his left pant pocket,
he asked the what was in his pocket, the inmate replied, my glasses". The inmate
then turned and attempted to flee from the officer. The officer with the assistance of another officer placed the inmate on
the ground and applied restraints. Another officer assisted the Compound officers and applied leg irons to the inmate. A search
of the inmate’s pocket revealed a 6 inches long black hard plastic homemade weapon, sharpened on one end and wrapped
on the other end with black shoe string for a lanyard. The inmate was escorted to health services for a medical assessment
and escorted to the special housing unit without further incident. An investigation continues.
USP- ATWATER- MAY 30, 2010
At approximately 8:30p.m., a fight was announced in a housing unit between multiple inmates
with weapons. This was a gang related incident between approximately 5 different gangs and approximately 18 inmates were involved.
One inmate was taken to the outside hospital due to having multiple puncture wounds. There were no staff injuries and the
institution was placed on lock down until after the holiday to be re-evaluated once the administration gets off of their holiday
break.
FCI- BENNETTSVILLE- MAY 30, 2010
An officer was assaulted by an inmate. The inmate punched the officer in the neck. The inmate
was restrained and placed in SHU. The officer was not seriously injured.
USP- MCCREARY- MAY 29, 2010
At
approximately 1:55 p.m., five inmates escorted aninmate to the Compound Officer. The inmates told the inmate to "check in".
The inmate refused and pulled out what appeared to be a homemade weapon while stepping towards the inmates. Staff called forassistance,
and the inmate ran from the scene. While staff were following the inmate, he turned towards staff with weapon in hand and
began to run. Staff gave the inmate several orders to cease his actions, to which he complied and laid on the ground. Hand
restraints were applied. The inmate was removed from the area. He was medically assessed and placed in SHU without further
incident. Staff conducted a search of the direct area of the yard, the weapon was not recovered.
FMC- ROCHESTER- MAY 29, 2010
An inmate in SHU assaulted staff on two occasions by throwing a liquid substance on them. The
inmate was placed on a sack lunch for 5 days for his continued disruptive behavior which continued throughout the shift.
FCC- TERRE HAUTE- MAY 28, 2010
Eight inmates assaulted another inmate in a housing unit at the USP. During the response by
staff, one of the inmates bit a case manager on the finger. The inmate who was assaulted was taken to the local hospital for
treatment of his injuries.
USP- MCCREARY- MAY 28, 2010
At
approximately 6:54 p.m., the Center Hall Officercalled for assistance as an inmate ran past Center Hall towards the Rear Corridor.
The Rear Corridor Officer approached the inmate and observed a combination lock attached to a belt in the inmates right hand.
The Rear Corridor Officer ordered the inmate to drop the weapon. The inmate refused staff orders and was placed on the ground
by responding staff. Hand restraints were applied and the inmate was subsequently escorted to Health Services where an injury
assessment was completed. The inmate was noted to have received no injuries during the incident. The inmate was escorted from
Health Services to SHU where he awaits the disciplinary process.
At approximately
7:20 p.m., staff monitoring the Visiting Room surveillance cameras, observed a female visitor place an item into a potato
chip bag and give the bag to the inmate who swallowed the contents of the bag. The inmate was removed from the Visiting Room
and placed in Dry Cell status. Local authorities were notified and the
female visitor consented to a search of her vehicle. During the subsequent
search of the vehicle, an orange colored balloon containing marijuana
was located and an unmarked container containing numerous Xanax tablets
were found. The female visitor was arrested and taken into custody by the local police department. On May 29, 2010, the inmate
provided a stool sample which contained one orange colored balloon containing four
Xanax tablets. The FBI was notified and an investigation continues.
USP- MCCREARY- MAY 27, 2010
At approximately 2:20p.m., staff observed an inmateacting as if he was intoxicated. Staff called
for compound officers to escort him to the Lieutenants' Office. As staff escorted the inmate he became argumentative and began
to pull away from staff. The inmate attempted to aggressively turn on staff. Staff placed the inmate on the ground to gain
control of his disruptive behavior. Responding staff applied leg restraints, placed the inmate in a wheelchair, and escorted
him to the Lieutenants' Office. The inmate was breathalyzed, medically assessed, and admitted into SHU without further incident.
The FBI was notified and an investigation continues.
**At approximately 2:30p.m., staff was conducting linen exchange in SHU. As staff approached and opened the food slot
to exchange linen with an inmate the inmate became argumentative with staff. The inmate then placed his arm in the food slot
and refused staff orders to remove it. When staff attempted to secure the food slot, the inmate struck the
staff member in the forearm with an unknown object that he had in his right hand. Staff immediately gained control of the
inmate=s arm and placed it back in
the cell. The food slot was secured without further incident. Both staff and inmate were medically assessed. The F.B.I. was
notified and the investigation continues.

At 2:40p.m., an inmate was placed in SHU for assault on a staff member.
**At 5:53p.m., an inmate was sent to SHU for assault on a staff member and engaging
in a sexual act.
USP- CANAAN- MAY 24, 2010
At 6:49a.m., a fight was announced in the Special Housing Unit recreation cage. It was an inmate
on inmate fight and both inmates were separated. There were no staff injuries to report.
**At approximately 6:45p.m., a cell fight was announced in the Special Housing Unit. It was
an inmate on inmate fight. Both inmates were restrained and one inmate received treatment for a laceration. There was no staff
injuries to report.
USP- CANAAN- MAY 23, 2010
At 6:57p.m., a fight was announced in a housing unit. It was an inmate on inmate cell fight

Officer's in the special housing unit were running recreation. They were escorting
an inmate back into his cell when the inmate jerked away from the officer as the officer attempted to uncuff him thru the
tray slot, therefore breaking the cuff key off inside the handcuffs. Gas had to be used on this inmate to get him to comply
with orders from staff.

At 11:32a.m., the control center announced that there was a body alarm activation for two inmates
fighting. The inmates were separated, medically assessed, and placed in the Special Housing Unit with no further incident.
FCI- EL RENO- MAY 18, 2010
A staff member was attempting to restrain a disruptive inmate. The inmate became combative and
resisted against being restrained. The inmate turned around on the staff member catching the staff members hand in the cuffs
and began running away. A call for assistance was initiated and responding staff were able to subdue the inmate after a short
chase. The inmate was placed on the ground and restrained. The inmate was moved to the SHU. The staff member was treated at
a local hospital and suffered injuries to his hand requiring it to be placed in a cast.

At approximately 10:00a.m., there was a fight announced with weapons involved.
Two inmates attacked one inmate. The fight was gang related. Due to where the fight was taking place the center yard tower
was unable to observe or intervene. The inmates were separated and medically accessed. There were no staff injuries reported.

At 11:30a.m., a fight was announced in the Special Housing Unit inside the recreation cage.
It was an inmate on inmate fight and one inmate was taken to an outside hospital for injuries he sustained from fight. There
were no staff injuries to report.
FCI- HERLONG- MAY 16, 2010
At approximately 9:20p.m., an inmate asked an officer if he could speak to the Operations Lieutenant.
Upon arriving at the Lieutenants Office, the inmate was told to remove his hat because it was pulled low over his eyes. The
inmates face showed to have severe bruising and lacerations consistent with a fight. The inmate was unable to be medically
assessed due to the time. The inmate was sent out to a local hospital for evaluation and treatment. Upon the inmates return to
the institution he was placed in the Special Housing Unit.

At 10:30a.m., an inmate lunged at the Lieutenant. The Lieutenant called for assistance and the
immediate use of force was used on the inmate. The inmate was placed in hand restrains without any further incident. The inmate
did not receive any injuries. There were no staff injuries reported.
USP- CANAAN- MAY 13, 2010
At 1:30p.m., a fight was announced in a housing unit. Two inmates attacked one inmate. One inmate
was taken to an outside hospital for treatment. All of the inmates involved was placed in the special housing unit. There
was no staff injuries to report.

At approximately 3:00p.m., a fight broke out in the recreation yard among gang
rival inmates. Shortly after this happened an unit fight also occurred. There were several inmates involved. There were no
staff injuries and interviews were conducted.

At 1:29p.m., control announced that assistance was needed in SHU. An inmate attempted suicide
by trying to hang himself. A staff member suffered a sever laceration to his finger by trying to cut the inmate down.

At 8:27p.m., an inmate stuck his arm out the food slot in a special housing unit
and refused to put his arm back inside. The inmate then assaulted staff by spitting a mouthful of saliva striking a staff
member in the face and upper body. The inmate then grabbed a book from his cell and threw it at another officer nearly striking
him in the head. All staff were medically assessed by health services.

inmate. The inmate was given time to calm down and was sent back to his unit.

A fight broke out between inmates causing four staff members to get injured. Two
of the staff members were treated at a local hospital.

At 6:40p.m., an officer advised the Operations Lieutenant that an inmate had been
assaulted in indoor recreation. After a review of the video it showed that 8 inmates assaulted 1 inmate, with weapons. The
assault victim was medically assessed and escorted to the Special Housing Unit.
FCI- OAKDALE- MAY 4, 2010
There were five correctional officers assaulted in the SMU Unit. One correctional officer received
injuries to his hand. The other four officers were assaulted with bodily fluids.

At 7:20p.m., control announced a fight in a housing unit. One inmate assaulted two other inmates
with a weapon. The inmates had to be physically separated by responding Staff. One inmate was taken to an outside hospital
due to the extent of his injuries. There were no staff injuries to report

At approximately 2:00p.m., recreation staff notified the Operations Lieutenant and control that
there was a large grouping of inmates from two different gangs in a standoff. The inmates told the Lieutenant that there was
a disrespect issue going on.

There was an inmate on inmate fight inside the recreation area. One inmate was
attacked by other inmates and received a possible broken jaw among other injuries. There were weapons involved in this fight.

At approximately 9:00a.m., a fight broke out in the recreation yard. There were
approximately 8-10 inmates involved with weapons. The tower officer fired off non lethal munitions. One inmate was cut several
times. There was no staff injured. An investigation is ongoing.

At 8:10a.m., an unit officer activated his body alarm and advised there was a fight
in his unit. Three inmates attacked one inmate. The inmates were separated, medically assessed, and placed in SHU.

12:30p.m., an inmate refused a pat search in recreation. The inmate then attempted to place something in his mouth. A recreation
officer activated body his body alarm and placed the inmate on the fence.
***At 3:00p.m., an inmate assaulted another
inmate in front of a housing unit, during open movement. This assault occurred in front of the Operations Lieutenant, and
the Unit Officer, the inmates were separated, medically assessed, and placed in SHU.

There was an inmate on inmate fight. One inmate was stabbed several times. The
inmate was found during the 4:00p.m. count.
USP- HAZELTON- APRIL 27, 2010
At 6:04p.m., an inmate became assaultive toward two officers. One of the officers
got a split lip and scratches on his neck and wrist.
**At 6:20p.m., four inmates attacked another inmate. CPR was
administered to bring the inmate back to life. He is at an outside hospital in critical condition.
FMC- LEXINGTON- APRIL 27, 2010
At 12:05p.m., a body alarm was announced in a housing unit by the unit officer.
Responding Staff arrived to find 5 inmates who entered the housing unit and were chasing another inmate who they already assaulted
on a different unit. There were two inmates who received injuries to the head and received stitches from medical staff. A
total of 9 inmates from 3 separate gangs were placed in SHU. There were no staff injuries to report. The institution was placed
on lock down status.
FCI- EL RENO- APRIL 27, 2010
At approximately 6:25a.m., an unidentified inmate entered into a housing unit.
He was dressed with gloves and a mask. He proceeded to assault another inmate with a pipe. The assault victim had to be medically
treated. The unit was placed on lock down until interviews and upper body searches could be conducted.
**At approximately 2:20p.m., a fight was called in the facilities area of the institution.
Three inmates were involved. All three were medically assessed and treated on site, subsequently placed in SHU. The institution
was not locked down.

At 9:46p.m., a body alarm was announced in a housing unit by the unit officer.
The officer found an inmate lying on a cell floor in a pool of blood. Responding staff arrived and medical staff began working
on the inmate. The inmate had been stabbed several times and was transported to a local hospital by an ambulance. The inmate
was pronounced dead later in the morning. The institution was placed on lock down status. The FBI was called and an investigation
continues. There were no staff injuries to report.

At approximately 6:00p.m., there was a body alarm activation in a housing unit. A unit officer
was pat searching an inmate coming into his unit. The unit officer was inspecting the inmates radio and the inmate lunged
for the item, knocking the radio out of the officer's hands. The inmate was placed on the ground with the assistance of another
unit officer. The inmate had hidden a 2.5 inch razor blade from a utility type knife, inside his radio holder. The inmate
was medically assessed and escorted to the Special Housing Unit without further incident.

*At 12:26p.m., An inmate was placed in SHU for assaulting a staff member.
**At 2:30p.m., a visitor was arrested and removed from the institutional grounds for attempting
to bring in 3 balloons of Black Tar Heroin (identified by SIS, and the arrest was per FBI).

fight broke out in an unit. A staff member injured his knee while responding.
USP- ATWATER- APRIL 23, 2010
At approximately 9:00p.m., while the unit officer was locking down the unit for
the 9:30pm count he heard a shuffle below him. He noticed several inmates acting strangely and called for staff to assist
him in locking down. Through the video subservience system it was identified that 2 inmates were in a heated argument and
were pushing each other. One inmate went back to his cell and grabbed what appeared to be a weapon. When responding staff
arrived the inmate got rid of the weapon. Both inmates went to SHU with no staff injuries.
FCC- COLEMAN USP 2- APRIL 24, 2010
USP-2 is on lock down due to an inmate being stabbed 15 times. The inmate that
was stabbed is on life support. The stabbing was drug related.

At 12:55p.m., a call for assistance was announced in SHU. There was an inmate on inmate cell
fight in progress. Responding staff separated both inmates and both inmates medically assessed and placed in SHU. There were
no staff injuries to report.
At 9:35p.m., a call for assistance was announced in a housing unit. An inmate on inmate cell
fight was in progress. Responding staff separated both inmates and both inmates medically assessed placed in SHU. There were
no staff injuries to report.

At 9:06a.m., an unit officer announced a fight in the unit with weapons involved.
Responding staff had to physically separate the inmates. One inmate was transported to outside hospital by ambulance and then
transferred by ambulance to the trauma center . Three staff members received injuries requiring treatment at local hospital
with one of them being transported by ambulance.
At 9:08a.m., a fight was announced on the recreation yard. Four inmates were involved in a fight. Three inmates
jumped on one. Two towers fired less lethal munitions with negative results. Responding staff physically separated the inmates.
There was no staff injuries to report.

*At 9:05a.m., there was an one on one inmate fight in R&D. The inmates stopped fighting
when sufficient staff were at the door. There was no staff injuries to report.
**At 9:31a.m., an unit officer announced a fight with weapons involved. Staff members
physically separated the inmates and located additional weapons in their pockets. One staff member received injuries.
***At 12:24p.m., the tower officer announced s fight on the recreation yard. Less lethal munitions
were fired from 3 different towers with non compliance from the 2 inmates involved. Responding staff separated the inmates.
There was two staff injuries reported. One had to be transported to an outside hospital for injuries.
****At 8:53p.m., an unit officer announced fight in the unit. Five inmates were
assaulting another inmate with weapons. Staff had to physically separate the inmates. One inmate was transported to the local
hospital by a Govt. vehicle and then transferred to a trauma center by ambulance. There was three staff members injured in
the incident with two of them being transported to outside hospital for treatment. The inmate that was transported to the
hospital was pronounced dead on April 29, 2010.
FCI- ALLENWOOD- APRIL 20, 2010
A fight occurred in the dining hall in response to a fight two days before. The institution
was placed on lock down until 4-27-10.
USP- BIG SANDY- APRIL 20, 2010
Two inmates assaulted three officers. One officer received a dislocated finger. Another officer
had swelling of the wrist. The third officer was hit in the nose. All inmates were placed in SHU.

At 8:14p.m., a fight was announced on the recreation yard. Several warning shots fired by the
tower 7 officer. Twenty inmates were secured in SHU and R&D pending SIS investigation for fighting. The yard was cleared
and the inmates were locked in their cells for the night.

At 9:31a.m., the control center announced a fight with weapons in a housing unit. Responding
staff had to physically separate both inmates. After the inmates were restrained they were searched and both were in possession
of weapons. After being medically assessed they were placed in SHU. One staff member injured their back while separating the
inmates.
FCI- EL RENO- APRIL 19, 2010
At approximately 4:50p.m., an officer stopped an inmate who was failing to identify himself
to the officer. The inmate began combative when he turned and swung an arm at the officer. The inmate then took a combative
stance and jumped as if he were going to strike the officer. A call for assistance was initiated. The inmate then assaulted
the staff member and was taken to the ground. This was the same officer who was assaulted the day before. The officer sustained
minor injuries, again. The inmate was identified and placed in SHU. The unit was placed on lock down until interviews could
be conducted the following day.

A fight occurred in the food service area. The fight was gang related and one inmate
had to be emergency airlifted to an outside hospital. He was diagnosed with a cracked skull. The institution was placed on
lock down status and an investigation continues.

At 7:05p.m., two inmates were sent to SHU for assaulting a staff member. and one
inmate to SHU for inciting a riot while intoxicated.

At 1:31p.m., a call for assistance was announced in a housing unit. An inmate on
inmate fight was in progress. Responding staff separated both inmates and both inmate were medically assessed and placed in
SHU. There were no staff injuries to report.
FCI- ALLENWOOD- APRIL 18, 2010
There was a fight in the housing unit. It was gang related. Eight inmates were
placed in SHU and one inmate was taken to the hospital.
FCI- Beaumont Low - April 18, 2010
At approximately 12:15p.m., an inmate was assaulted on the recreation yard. The inmate was medically
assessed and escorted to SHU.
FCI Beaumont Medium - April 18, 2010
At approximately 2:00p.m., a staff member was assaulted by an inmate when he attempted to pat
the inmate down. The inmate was restrained and escorted to SHU. The officer was not injured.
USP- MCCREARY- APRIL 18, 2010
At 11:20a.m., there was a call for staff assistance in an unit. Staff completed
a shakedown and an inmate struck a Lieutenant and pushed an officer. A second inmate attempted to throw hot soup on the staff,
with only a small amount hitting the staff. Three inmates were placed in SHU.
FCI- EL RENO- APRIL 18, 2010
At approximately 4:50 p.m., an officer was making rounds through a housing unit.
He came upon an inmate in a cell acting suspiciously. When he attempted to search the inmate, the inmate became combative
and attempted to flee the cell. The inmate assaulted the officer and managed to get out of the cell and throw something to
another inmate. The officer initiated a call for assistance. Both inmates were identified and restrained by responding staff.
Marijuana was found in the area where the assault occurred. The staff member received minor injuries and continued to work
through his shift. The unit was placed on lock down until interviews could be conducted the following day.
FCC- COLEMAN- USP 1- APRIL 17, 2010
A square off occurred on the recreation yard. Several inmates started grouping together and
when staff arrived the inmates broke up. There was 12 shanks recovered on the recreation yard.
USP- LEE- APRIL 16, 2010
At 12:00p.m., the control center announced a fight on the recreation yard. Two inmates were
assaulting another on the softball field. The assailants refused staff orders to stop and had to be physically removed from
the inmate being attacked. All three inmates were placed in SHU after this incident. There were no staff injuries to report.
USP- TERRE HAUTE- APRIL 16, 2010
Several gang related fights with weapons broke out in four different housing units. Two inmates
were taken to the local hospital for treatment of stab wounds and lacerations. Several inmates were placed in SHU. There was
no staff injuries to report. The institution was placed on lock down status. One inmate was pronounced dead after a few days
in the hospital.

At approximately 3:30p.m., an inmate in the Modified Special Housing unit attempted
to kill himself by cutting his left and right wrist with a razor blade. The inmate had to be taken out to the hospital for
treatment.
FCC- FORREST CITY- MEDIUM- APRIL 13, 2010
During lock down an inmate jumped on an officer for shaking his cell down. The inmate beat the
officer with a lock on a belt in the head until other staff members could respond. The officer was rushed to the ER with broken
bones and trauma to his face.

At 9:30p.m., a fight was announced in a housing unit. Two inmates jumped on one inmate. One
inmate was taken to an outside hospital for the injuries he sustained from the assault. There was no staff injuries to report.
FCI- HERLONG- APRIL 13, 2010
At approximately 9:00p.m., an inmate came into an unit officers office with a bloody face and
other marks consistent with a fight saying he needed to "check in." Upper body checks were initiated and another inmate was
found to have markings consistent with a fight as well. Upon review of video a third inmate was identified as an attacker.
Three inmates were escorted to the Special Housing Unit without further incident. This was gang related due to the victim
not wanting to hang out with a particular gang anymore.

At 9:19p.m., control announced that staff needs assistance in SHU. Two inmates were fighting
in their assigned cell. Both inmates refused several orders from staff to stop fighting. The door was opened and the inmates
had to be physically separated, restrained, and moved from the cell by responding staff. There was no staff injuries to report.

At approximately 9:00a.m., an inmate in a holding cell tried to grab a staff member
thru the bars but failed. He then assaulted staff by spitting in the staff members face.

At 5:55p.m., several inmates attacked one inmate. The incident was gang related. The inmate
that was attacked was taken by ambulance to the local hospital then airlifted to UT medical center. The institution was placed
on lock down status.

At 6:40a.m., while feeding breakfast in SHU an inmate attempted to assault staff. After the
food slot was opened, the staff member turned to get the inmate a tray off the cart the inmate reach out the slot trying to
grab the staff member. The staff member placed the inmate s arm back in the cell
and secured the slot to prevent the inmate from assaulting him.
FCI-BECKLEY- APRIL 11, 2010
An Officer was assaulted while in the visiting room shakedown area by an inmate assigned to
SHU. The Officer was taken to the Local Hospital and was treated for serious injuries (broken bone/eye laceration) to his
facial area The institution did not lock down outside of normal operations.

At 6:07p.m., a fight was announced in the gym. It was an inmate on inmate fight. Both inmates
were separated and placed in SHU. There was no staff injuries to report.
FCI- HERLONG- APRIL 9, 2010
At approximately 9:20p.m., while searching a cell, an officer pricked his finger on a tattoo
needle that was hidden under the inmates desk. The officer went to the hospital for treatment after the incident. The inmate
was placed in the Special Housing Unit.

Five inmates were involved in a fight. All of the inmates were placed in SHU.

At 8:18a.m., staff called for assistance in the East Corridor. Staff witnessed 4 inmates assaulting
one inmate outside of the commissary. Two inmates were immediately identified as assailants and after reviewing video the
other two were identified. All inmates involved were medically assessed and placed in SHU. One Staff member injured his back
while trying to restrain one of the inmates.

an inmate handcuff’s the inmate pulled away causing abrasions to the officers right arm.

assessed and placed in SHU.

At 2:18p.m., a fight was announced in Special Housing Unit. There was an inmate
on inmate cell fight. There was no staff injuries to report.

At 6:30 p.m., a fight was announced on the recreation yard. An inmate was stabbed in the neck.
The inmates was sent out to a local hospital then transferred to U.T. medical center. The institution was placed on lock-down
status.
FCI- LA TUNA- APRIL 5, 2010
After hearing rumors of a food strike, staff started conducting interviews with inmates. This
escalated into an inmate curing at staff. Staff members attempted to detain the inmate when another inmate got involved and
a physical confrontation began between staff and two inmates. The institution was placed on lock down status. The staff members
were not seriously hurt.

At approximately 4:30p.m., a fight was announced between two groups of inmates who were in two
different gangs. The institution was placed on lock down. A total of four inmates went to the level 4 trauma for their injuries.

At 9:55a.m., control announced a fight in a housing unit. An officer ordered the inmates involved
to stop fighting and they separated. One inmate was secured in the cell while the other was escorted out of the unit. After
the unit was secure the second inmate was retrained and taken out of the unit. Both inmates were medically assessed and placed
in SHU. There was no staff injuries to report.

During the E/W shift an Officer was assault by an inmate in a housing unit. The Officer was
taken to the hospital and received serious injuries (broken bones) to his facial area. The institution did not
lock down outside of normal operations

Unit. He assaulted staff by bitting the officer in the left arm twice. The staff member was taken to an outside hospital for

**At 4:30p.m., there was a fight between
approximately 30 inmates with weapons involved. The institution went on lock down and one inmate went to the local hospital
for his injuries. There was no staff injuries reported. The next day the institution went back to normal since it was a isolated
incident.

At 6:30p.m., a fight on the recreation yard was called via radio. Several inmates assaulted an
inmate. The inmate was medically assessed and placed in SHU. There were no staff injuries reported.
MCFP- SPRINGFIELD- MARCH 23, 2010
At approximately 7:50a.m., the unit officer asked an inmate to clean his room and told him that
he needed to start taking showers. The inmate squared off on the officer and then assaulted him, hitting him in the face with
his fist causing a scratch on the officer's nose. A body alarm was activated and staff responded. The inmate was taken to
lockup and was not injured.

At 1:02 p. m., a call for assistance was announced in a housing unit for a inmate on inmate
fight. The officer was able to separate the inmates until staff arrived. Both inmates were medically assessed and sent to
SHU. There was no staff injuries to report.
**At 1:05 p.m., a call for assitance was announced in a housing unit for a fight in progress.
This was five inmates on one. Staff arrived secured the area locked the unit down and and escorted the five inmates to SHU.
The other inmate was escorted to medical and sent out to a local hospital. There was no staff injuries to report.
***At 8:12 p.m,. an immediate use of force was used in Special Housing Unit during the move
because an inmate assaulted a staff member. One staff member broke his ankle and leg when the inmate bucked and landed on
the officers leg. The staff member was sent to outside hospital for treatment.

At 12:48p.m., an inmate was placed in SHU for threatening a staff member.

At 8:16p.m., an unit officer was conducting a random search of an inmate in the
corridor when the inmate turned and attempted to punch the officer in the face several times. Responding staff restrained
the inmate. The officer received a 1.5 inch cut on his forearm during the incident.
FCI- DUBLIN- MARCH 21, 2010
An inmate attempted to overdose on medication and when the inmate was told to cuff up the inmate
began to fight officers. The officers activated their body alarm for assistance till the inmate was placed in restrains and
taken out to the local hospital. One officer went to the local hospital for a hand injury.

An inmate punched and officer in the face for no reason. The officer was sent to an outside
hospital where he was treated and released for his injuries. The inmate was moved to another unit.

A staff member walked into an inmate’s cell as he turned to leave the inmate assaulted
him with a chair. The staff member received minor injuries. The institution is on temporary lock down.

An officer was assaulted by an inmate while locking down. The staff member was
taken to a local hospital and was treated and released for facial contusions.

At 7:45p.m., an inmate assaulted a staff member. The staff received minor injuries. The inmate
was places in ambulatory restraints and placed in SHU.

At 3:20 p.m., an inmate on inmate fight was announced in a unit. Two inmates was
assaulting the other inmate. All inmates was secured and medically assessed and placed in SHU. The inmates were drinking and
the whole unit was breathalyzed. There was no staff injuries to report.
**At 5:35 p. m., a medical emergency was called in a unit. An inmate was found on his cell floor and was taken
to the institutions hospital to be medically assessed. The inmate was transferred to an outside hospital by ambulance with
major head trauma. There was no staff injuries to report.

At 3:40p.m., during yard recall, two inmates began fighting. Both inmates had weapons
and had to be physically separated by responding Staff. One inmate had to be taken to an outside hospital due to the extent
of his injuries. There was no staff injuries to report.
**At 7:10p.m., during a visual search, an inmate became combative toward staff. The inmate began throwing
punches with both hands toward staff. The inmate was restrained and escorted to SHU. The staff member involved was taken to
an outside hospital due to his injuries.

At 11:56 a.m., a fight was announced.. Two inmates assaulted one inmate by hitting him in the
back of the head with a lock in a sock. There was no staff injuries to report.
FCI- OXFORD- MARCH 5, 2010
Control Center announced a fight. Staff responded and separated the two inmates.
Both inmates were examined by medical staff and placed in SHU. The inmates involved in the fight were cell mates. There was
no staff injuries reported.

At 4:24p.m., staff called for assistance in Special Housing due to 2 inmates who were fighting
in their cell. Both inmates was placed in hand restraints and were removed from their cell and separated. There was no staff
injuries to report.

At 11:02a.m., a call for assistance was announced. An inmate on inmate broke out.
There was no weapons involved. Both inmates was placed in the Special Housing Unit. There was no staff injuries to report.

At approximately 9:20 p.m. an inmate was assaulted in a housing unit. The inmate sustained several
puncture wounds.

At 10:58a.m., a call for assistance was announced in unit d-2. An inmate became aggressive toward
staff and tried to assault a staff membe. The inmate was placed in special housing unit. There was no staff injuries to report.
USP- TERRE HAUTE- MARCH 3, 2010
An inmate was stabbed more than thirty times by another inmate in the housing unit.
The inmate was taken by ambulance to a local hospital and admitted to ICU due to multiple stab wounds in his chest and back.

At approximately 6:45 p.m., two staff members were assaulted by an inmate. The inmate was placed
in SHU.

At 9:57a.m., a body alarm was announced in the d-2 housing unit. An officer was assaulted by
a inmate. The inmate punched the officer in the face and head multiple times. The officer was taken to an outside hospital
for treatment.
USP- FLORENCE- MARCH 2, 2010
At 1:30p.m., an intoxicated inmate from another unit went into an officer’s
unit and hit her in the head area several times. After she was hit she backed up and hit her body alarm then put distance
between her and the inmate until additional staff arrives to help her. The inmate then went after the responding staff. The
staff member received a slight concussion and is doing fine. The inmate was moved to another institution. The institution
was also placed on lock-down status until futher notice.

At approximately 1:40 p.m., a body alarm was activated by a staff member. Two inmates were involved
in a one on one fight in the dining hall. The inmates were separated and escorted to SHU.

At 5:00p.m., an inmate was placed in SHU for indecent exposure and threatening staff.

At approximately 2:30 p.m., an inmate was assaulted on the recreation yard. The inmate sustained
several puncture wounds. He was transported to an outside hospital where he was treated and released.
FCI- OXFORD- MARCH 1, 2010
An altercation occurred between two inmates in an housing unit. The incident believed
to be between two gang member inmates. Later that evening an altercation was reported in recreation between 3 or 4 inmates.
This was also believed to be between gang member inmates. The institution was placed on lock-down status until mass interviews
are conducted.

At approximately 10:50 a.m., a staff member was assaulted by an inmate. The inmate was restrained
and escorted to SHU.

At 8:15a.m., a call for assistance was announced in the green corridor. An inmate was found
with a weapon. The inmate was placed in special housing unit. There was no staff injuries to report.

An officer at USP-2 was assaulted in SHU. The inmate through a tray and hit officer in the eye.
The officer was sent to a local hospital.
USP- TERRE HAUTE- FEBRUARY 21, 2010
Staff members found an inmate in his cell with blood on his back. The cell was occupied by two
inmates. The inmate was hospitalized for stab wounds to his back.

At 10:10a.m., a call for assistance was made in the gym. There was two inmates fighting one
on one. The inmates were in the gym due to mass shakedowns being conducted in the housing units. There was no staff injuries
to report. The institution is still on lock down status.

At 7:49 p.m., a call for assistance was made for the outside Unit D. An inmate was assaulted
with a weapon. The inmate was stabbed several times in the chest area. The inmate was taken to an outside hospital. There
was no staff injuries reported. The institution was placed on lock down status until further notice.

At
11:00a.m.,an inmate assaulted an officer by striking him in the face and attempted to gouge his eyes out with his fingers
when they were on the ground. The officer did not suffer from serious injuries. The inmate was locked up in SHU.

At 10:45 a.m., a fight was announced in E-1. It was a cell fight with 3 inmates on one. One
inmate sustained lacerations and abrasions to the face and body. There was no staff injuries reported.

At approximately 6:30p.m., the Recreation Officer announced a fight in progress on the Recreation
yard. Two inmates were fighting each other. Upon arrival staff the inmates were separated and escorted to SHU. There were
no staff injuries reported.

At 12:50 p.m., a fight was announced in front of C-1 Housing Unit. One inmate stabbed the other
inmate. The inmate sustained a few puncture wounds. The shank was recovered. There was no staff injuries reported.

At approximately 11:00 a.m., two inmates were fighting inside the chow hall. Staff
members came in and broke up the fight. The inmates sustained minor injures. There was no staff injured.
MCFP- SPRINGFIELD- FEBRYARY 8, 2010
At approximately 10:38a.m., an inmate from another unit assaulted another inmate. The OIC called
for assistance and the two inmates were separated and taken to lockup. The inmate that was assaulted received some bruises
to his facial area. There were no staff injuries reported.

At 3:20 p.m., a fight in F1 was announced. An inmate used an ab roller and struck
another inmate in head several times. Inmate was taken to outside hospital with serious head injuries. There was no staff
injuries reported.
FCI- BEAUMONT MEDIUM- FEBRUARY 7, 2010
At approximately 7:40a.m. the Recreation Officer announced a fight in progress on the recreation
yard. Two inmates were assaulting another. Upon the arrival of responding staff the inmates were separated and escorted to
SHU. There were no staff injuries reported.
USP- CANAAN- FEBRUARY 6, 2010
At approximately 12:15 p.m., in the Special Housing Unit an officer was collecting food service
trays when he was assaulted by an inmate. Inmate through feces on the staff member.
FCI- ELKTON- FEBRUARY 4, 2010
An officer was conducting a shake down of an inmate’s locker. While shaking down, the
officer found a leafy substance in an envelope. The inmate arrived at the area and made a lunging gesture towards the officer
in an attempt to grab the substance. The officer was required to use physical force to move the inmate away from him as he
activated his body alarm. The inmate was placed in the Special Housing Unit. There was no known injury that occurred to the
staff member.
USP- CANAAN- FEBRUARY 3, 2010
At 1:24 p.m., a body alarm announced in the education department. A fight had occurred between
inmates. There was no staff injuries reported.
FCC- COLEMAN- USP 1- FEBRUARY 2, 2010
At 2:00p.m., the tower officer advised that a group of gang rival inmates appeared to be squaring
off against each other on the rec yard. The compound officer requested additional staff to report to the rec yard. Responding
staff observed an inmate bleeding from the face area. He was immediately escorted off of the rec yard to health services.
A review of the Vicon security camera revealed one inmate striking another inmate with closed fist punches and knee kicks
to his upper torso and face area. The inmate was medically assessed and escorted to SHU without incident.
USP- HAZELTON- JANUARY 31, 2010
At 12:50p.m., an inmate threw urine in the face of an officer as he was making rounds in the
Special Housing Unit.
**At 09:10p.m., an officer was searching an intoxicated inmate in the Lieutenants complex. The
inmate became disruptive and struck the officer with his elbow. The officer received a contusion on his arm.
FCC- COLEMAN- USP-1- JANUARY 31, 2010
At approximately 8:32 p.m., an officer requested additional staff report to K-Unit.
The officer observed an inmate lunge towards him in front of the upper tier officers station. The officer perceived the inmate
lunged at him with a weapon after he heard something he perceived as metallic fall to the lower tier. The inmate’s attempt
to assault the officer failed when another inmate stepped between them and wrapped his arms around the inmate preventing the
assault. The inmate then escorted the other inmate back to his cell. Responding staff secured the unit as other inmates within
the unit were loud, unruly and resentful of being locked down. The inmate was removed from the cell, medically examined, and
placed in the Special Housing Unit.
USP- CANAAN- JANUARY 30, 2010
At approximately 11:30 a.m., the e-1 unit officer notified the operations lieutenant of a possible
fight in his housing unit. The unit was lock down and upper body searches were conducted. Two inmates were found to have injuries
both inmates were taken for medical assessments. At 1:15 pm one the inmates was taken to outside local hospital for injuries
he sustained from the fight. There was no staff injuries to report.
USP- HAZELTON- JANUARY 29, 2010
At 11:10 p.m., an inmate in SHU covered his window and broke the sprinkler head
in his cell. As a SHU officer was attempting to clear the water from in front of the cell door, the inmate threw a liquid
substance which smelled of feces on him. It hit the officer in his arm and leg.
USP- HAZELTON- JANUARY 28, 2010
At 05:27 p.m., a fight was announced in A-2 Unit. Several inmates were fighting and staff responded.
As an officer was attempting to pat search an inmate on the top tier of the unit, the inmate became combative and grabbed
the officer by the arm. The inmate then pushed the officer towards the top tier railing, while simultaneously reaching for
a weapon which was hidden in his waistband. Chemical agents were utilized on the inmate to stop the assault, and the inmate
was then restrained and the weapon recovered.
MCFP- SPRINGFIELD- JANUARY 26, 2010
At approximately 8:10p.m., an officer working the mental health unit found an inmate
unresponsive. His hands was tied and a ligature was around his neck. Two inmates were locked up for the homicide.
USP- HAZELTON- JANUARY 26, 2010
At 6:05p.m., while an officer was searching a cell in A-1 (SMU) unit,
an inmate entered the cell with her and punched her in the side of the head. The officer was referred to an outside hospital
for treatment. A sharpened weapon was recovered from the cell after the incident.
FCI- OXFORD- JANUARY 25, 2010
An inmate in an unit was found with injuries consistent with an assault. A second inmate was
identified as being involved. Both inmates were examined by medical staff, then placed in the Special Housing Unit. Inmates
in the unit were placed on cell restriction until mass interviews were conducted. There were no staff injuries reported.
FCC- COLEMAN- USP 1- JANUARY 22, 2010
USP-1 is on lock down, due to inmate found unconscious and stabbed in the recreation yard.
FCI- EL RENO- JANUARY 22, 2010
At approximately 6:00 an inmate was assaulted in a housing unit. The inmate was
transported to a local hospital for his injuries. The institution was locked down and 11 inmates were identified as possible
participants in the incident and were placed in SHU.
USP- LEE- JANUARY 21, 2010
At 3:44 PM a unit officer announced a fight in his unit with weapons involved. As responding
staff entered the unit the assault stopped and the inmate was secured in a tv room. The victim was restrained and removed
from the unit to the unit team area. The victim was then transported by stretcher to the health services area. At 4:21 PM
the victim was transported to an outside hospital where he was pronounced dead at 4:55 PM. Institution remains on lock down
status pending outcome of mass interviews.
FCI- BEAUMONT- MEDIUM- JANUARY 17, 2010
At 7:30a.m., an officer was assaulted by an inmate with a book. The inmate was escorted to S.H.U.
**At 11:00a.m. 26 inmates refused to leave the recreation cages in SHU. The inmates held the
recreation cages closed for several hours before complying with staff orders to exit the cages.
USP- LEWISBURG- JANUARY 17, 2010
At 3:13p.m., a cell fight broke
out in F Block. Gas was used to separate the inmates. Both inmates were removed safely and medically assessed. There was no
staff injuries reported.
FCI- BEAUMONT- MEDIUM- JANUARY 16, 2010
At approximately 6:00p.m., an inmate in SHU threw an unknown liquid substance
on an officer.
USP- LEWISBURG- JANUARY 16, 2010
An inmate was placed into ambulatory
restraints for assaulting a staff member in G Block. There was no other staff injuries reported.
FCC- COLEMAN- USP 2- JANUARY 14, 2010
At approximately 9:30a.m., an unit officer was monitoring the movement when he
told an inmate he cannot enter his unit. The inmate quickly jumped on the staff and they were wrestling to the ground. The
officer was taken to the hospital and pending more information. Another staff member received head injuries as well. He was
the first responder and he suffered some type of head trauma. The officer was also sent to hospital.
USP- CANAAN- JANUARY 14, 2010
At approximately 10:35 a.m., 200 hundred inmates that were in the gymnasium were observed grouping
up by staff. These inmates were placed in the gym due mass shake downs being conducted in c-1 and c-2. The inmates were ordered
by staff to lay on the ground and the inmates would not comply. At 10:50 a.m., flash bangs and chemical munitions were deployed
in the gym to take control of the dangerous situation. All inmates were then flexed cuffed and decontaminated by health services.
Body checks were conducted inmates were then locked down. There was no staff injuries to report and the institution is still
on lock down status.

At approximately 12:30p.m., A fight broke out on the recreation yard, among several
inmates. The was no contraband found but several inmates were blooded. There was no staff injuries reported.
USP- CANAAN- JANUARY 13, 2010
A call for assistance was announced at approximately 11:15 am. A fight broke out on the compound
in front of the housing units 2 inmates on 1. One inmate was stabbed multiple times. The inmate that was stabbed was taken
by ambulances to outside hospital. There was no staff injuries reported.
FCC- COLEMAN- USP 1- JANUARY 12, 2010
A fight broke out in the recreation yard between internal gangs. Staff fired two
percussion grandes to quell the fight. The institution was placed on lock down status until 01-13-10.
FCI- MIAMI- JANUARY 12, 2010
At approximately 12:50a.mm, a radio call came from an officer, stating that he needed a couple
of officers to respond to his housing unit. An inmate was beginning to get angry and acted like he wanted to fight. As officers
tried to restrain the inmate the inmate became combative and swung at one of the officers. The officers took down the inmate
and placed him in arm restraints. The inmate was placed in SHU. There was no staff injuries reported.
MCFP- SPRINGFIELD- JANUARY 12, 2010
At approximately 5:15p.m., in the upstairs TV room on ward 10-G, two inmates were watching TV.
Another inmate came in and turned the channel without asking the others. The channel changer and one of the inmates watching
TV had words. The one inmate went to his room and then came back and hit the channel changer in the side of his head. The
channel changer ran out of the room and the other inmate that had been watching TV with the first inmate, came to the aid
of the channel changer and hit the first inmate. The inmates began fighting. The first inmate eventually goes back to his
room and the second inmate follows him and again begins to fight with him. All three inmates were locked up. The channel changer
just had some red marks on his face where he got hit. The first inmate did not get injured and the second inmate had 2-3 small
lacerations to the head and a dislocated thumb.
FCI- EL RENO- JANUARY 11, 2010
At approximately 6:00p.m., there was an assault on the recreation yard. An inmate who had just
arrived on the bus was assaulted. He was transported to a local hospital and is currently in the I.C.U. There were no weapons
noted that were used in the assault. There were no staff injuries in the assault. The institution was placed on lockdown and
the lockdown was lifted on Jan 12, 2010 at approximately 9:30 AM
USP- LEWISBURG- JANUARY 11, 2010
At 8:37a.m., an assistance call was made in the
west recreation yard. Two inmates were attacking another inmates. The inmates were separated, medically assessed, and returned
to their cells. There was no staff injuries reported.
**At 9:09a.m., a body alarm was sounded in the Z block Recreation area because an inmate assaulted staff.
Responding staff restrained the inmate and he was placed into 4 point restraints. The staff member received minor injuries
to his face.
***At 12:33p.m., a body alarm was hit in the G block third floor because an inmate assaulted staff. The inmate
was placed into ambulatory restraints. There was no other staff injuries reported.
FCI- TEXARKANA- JANUARY 9, 2010
During the evening watch, the D-unit officer was making rounds and observed an inmate in the
D-2 restroom covered in blood. It appeared the inmate was kicked in the facial area several times and his facial area was
also smashed using a hard object. His bones from his face were broken in several different places that the inmate was transported
two hours from the institution to a specialist. There was no staff were injured.
FCI- EL RENO- JANUARY 8, 2010
At approximately 11:30a.m., there was a fight in the segregation unit between two
inmates who were celled together. It was an isolated incident and no staff were injured.
USP- CANAAN- JANUARY 6, 2010
At 7:10a.m., an inmate could not clear a medal detector at which time staff started
to pat search the inmate. He became combative he was placed on the ground and was found to have a shank hide in the sleeve
of his jacket. The inmate was placed in special housing unit. There was no staff injuries reported.
FCC- COLEMAN USP 2- JANUARY 6, 2010
An inmate was walking beside the operations lieutenant and grabbed her butt. Other staff members
responded and placed the inmate in SHU.
**At approximately 12:30 p.m., lock down was again ordered due to fences going
down and inoperable. The stun lethal fence at both institutions has gone down on a daily basis , thus interrupting the daily
running of the institution. Programs, recreation and other activities have been shut down due to fence problems. Which, in
turn, creates a hostile work environment for our officers who have to deal with irate inmates who are confined to there unit
without any activities to keep them occupied.
USP- LEWISBURG- JANUARY 6, 2010
At
3:35p.m., an assistance call was made for a cell fight in E Block. Both inmates were examined by medical staff. There was
no staff injuries reported.
FCC- COLEMAN- USP 2- JANUARY 5, 2010
At approximately 9:30 p.m., staff called for compound to stop by H-2 unit because an inmate
refused to step into his cell for lock down. The compound officer arrived and pat searched the inmate. As inmate was going
into his cell he turned around and slapped the officer in the face. The officer immediately wrestled with the inmate and placed
him in hand restraints.

The FCI Medium is currently locked down due to a fight among two rival gangs. A total
of 8 inmates were fighting in front of food service for the evening meal. Two of the inmates were sent to local hospital with
serious stab
wounds.
FCI- TEXARKANA- JANUARY 4, 2010
At 7:20 a.m., an inmate was assaulted by another inmate in a housing unit
due to a gambling bet. R & D staff heard some loud noises above their work area and notified the Lt's office. Lt's
responded to the area where the noise was heard and found one inmate laying on his bunk covered up with his sheets. The inmate
was kicked in the facial and upper body area. The institution was on lock down until inmates were counted and interviewed.
The institution lock down was lifted at 10:30a.m.
FCI- BENNETTSVILLE- JANUARY 4, 2010
At 12:10 p.m. a fight occurred on the recreation yard involving six inmates on two inmates,
with weapons involved. Two inmates were sent to an outside hospital for injuries. One inmate had multiple stab wounds and
the second inmate had head trauma and a broken nose and jaw.
